A speech brain-computer interface (BCI) turns neural activity into text by recording signals related to a person’s attempt or intention to speak, then using a trained decoder to predict speech units and assemble them into words. It does not simply read finished thoughts. The strongest current demonstrations use implanted electrodes, individualized training, and controlled communication tasks; they are experimental medical systems, not consumer mind-reading devices.
What does a speech BCI actually decode?
A speech BCI decodes patterns of activity associated with speech production or a defined speech-related task. Depending on the study, a participant may try to speak despite being unable to produce intelligible speech, silently articulate words, imagine speaking, or perform a controlled inner-speech task. Researchers pair the recorded activity with the words, sounds, or movements the participant intended, so a decoder can learn a mapping for that person and task.
This distinction matters: an attempted utterance, silent articulation, and private inner speech are not interchangeable inputs. A system trained on attempted speech cannot automatically be assumed to decode any thought, and results from one task do not establish performance on another. The phrase “mind reading” is misleading unless it is immediately qualified: these are limited, trained, user-dependent decoding systems, not general-purpose access to a person’s thoughts.

The conversion is a pipeline, not a direct translation from a single brain signal into a sentence. Researchers collect neural data during a defined task, process it into time-varying features, predict speech-related units, and use a sequence model to assemble those predictions into likely words.

- Record activity from speech-related regions. The highest-performing speech-to-text demonstrations covered here use microelectrode arrays implanted in the cortex or high-density electrodes placed on the cortical surface. They record activity from areas involved in planning and controlling articulators such as the lips, tongue, jaw, and larynx. These methods provide access to speech-related neural activity, but require surgery and remain part of clinical research.
- Define the communication task and collect paired examples. A participant performs a specified speech task while researchers record neural activity and know the intended words or sounds. Those paired examples provide training data. The decoder is therefore learned for a particular participant and communication setup rather than functioning as a universal translator.
- Process the changing signal. Neural recordings are filtered and transformed into features that preserve information about timing and speech-related activity. The information is distributed across neural populations; there is no single “word neuron” that simply emits a complete sentence.
- Predict intermediate speech units. Rather than mapping raw activity straight to a finished sentence, many systems estimate phonemes, articulatory features, acoustic units, or learned speech representations. These intermediate predictions give the next stage a structured sequence to interpret.
- Assemble the sequence into words. A recurrent neural network, transducer, language model, or related sequence decoder combines successive predictions into the most likely word sequence. Language structure can help resolve ambiguous or noisy evidence, but the resulting sentence is still a model’s best decoding of a trained task—not proof that every word was directly and unambiguously present in the neural signal.
- Present or speak the result. The output may appear as text, be sent to text-to-speech software, drive a personalized voice synthesizer, or control an animated face. Some newer approaches combine text and voice, allowing text to serve as a caption or fallback channel.
What have speech BCI demonstrations achieved?
Published demonstrations show that implanted systems can produce communication at useful rates, but their numbers describe different participants, tasks, vocabularies, metrics, and output modes. They should not be treated as a head-to-head ranking or a prediction of what an untrained user would experience.
| Demonstration | Recording and output | Reported result | What the result helps show |
|---|---|---|---|
| Willett et al. (2023) | Intracortical recording; brain-to-text | 62 words per minute; 9.1% word error rate on a 50-word vocabulary and 23.8% on a 125,000-word vocabulary | A large vocabulary can be decoded, but the error rate depends on vocabulary size. The demonstration involved one participant with ALS who could no longer speak intelligibly. |
| Metzger et al. (2023) | High-density cortical-surface recording; text, synthesized speech, and facial-avatar control | Median 78 words per minute and median 25% word error rate for text decoding | A single neural interface can support multiple forms of output, including expressive non-text output. |
| Card et al. (2024) | Speech neuroprosthesis; self-paced conversation | 97.5% accuracy after additional training; approximately 32 words per minute; more than 248 cumulative hours of use over 8.4 months | Training time and sustained use matter alongside headline accuracy. The accuracy figure is not the same metric as word error rate in the other reports. |
| Littlejohn et al. (2025) | Continuously streaming brain-to-voice system | Decoded in 80-millisecond increments, without explicit start-and-stop commands | Streaming can address conversational delay by producing output continuously rather than waiting for a complete sentence. |
| Wairagkar et al. (2025) | 256 implanted microelectrodes; real-time voice synthesis | Synthesized the participant’s voice, including changes in intonation and short melodies | Voice can carry expressive features beyond the words themselves, while still requiring implanted electrodes and individualized calibration. |
These figures answer different questions. Words per minute describes rate; word error rate describes the proportion of word-level errors under a study’s conditions; accuracy may use a different definition. Vocabulary size also matters: a fixed 50-word task is not equivalent to choosing among 125,000 words. To interpret any headline result, look for the participant and task, recording method, vocabulary, metric, training conditions, speed or latency, and whether the output was text or synthesized speech.
Can a BCI decode silent speech or inner speech?
Some research has investigated speech-related neural activity when a person is not producing audible speech, including controlled inner-speech tasks. Recent implanted research has decoded inner-speech-related activity in defined tasks, and single-neuron research has identified representations of internal speech in the human supramarginal gyrus. These findings are evidence that internal speech can have measurable neural correlates; they do not show that a device can freely transcribe a person’s unprompted stream of thought.
For practical communication, the system needs to know when the user intends to send a message. That is especially important for inner-speech decoding, because private cognition may not be meant for anyone else. Research on intended communication and user control is therefore not separate from the technology’s usefulness: a system must distinguish an intentional message from unrelated internal activity if it is to avoid turning private cognition into unintended output.

Are implanted BCIs the same as non-invasive brain-reading research?
No. The highest-performing speech-to-text demonstrations described here use implanted intracortical arrays or cortical-surface electrodes. Those approaches record speech-related activity at a level suited to communication decoding, but involve neurosurgery and are still tied to clinical research settings.
Non-invasive fMRI research has reconstructed aspects of the meaning of perceived or imagined language. That is better understood as semantic reconstruction research than as a practical speech-to-text communication prosthesis: fMRI is slow and equipment-intensive, rather than a low-latency interface for ordinary conversation. A result involving reconstructed meaning should not be presented as equivalent to streaming text from a portable BCI.
What still limits speech BCIs?
They are experimental and individualized. The strongest results generally come from one participant at a time and depend on calibration for that individual. A study result does not establish that another person, with a different condition or neural signal, would get the same speed or accuracy.

Implants bring a substantial deployment burden. The high-performing systems in these demonstrations require electrodes placed in or on the brain. That means they are not comparable to ordinary consumer electronics, and research performance alone does not establish broad availability or suitability for someone considering a medical procedure.

Errors and delays affect conversation. Word error rate, accuracy, vocabulary size, and words per minute describe separate aspects of performance. A fast output can still contain errors, while a low error rate on a constrained vocabulary does not guarantee the same result for open-ended conversation. Streaming work aims to reduce the need to wait for an entire sentence, but an 80-millisecond decoding increment is not, by itself, a guarantee of a particular end-to-end conversational delay.
Calibration and sustained use matter. A promising result after additional training may not describe what is available immediately after setup. The reported 248-plus cumulative hours over 8.4 months in one study is notable as a sustained-use result, but it does not establish uniform performance across users or all-day reliability in other settings.
Privacy and consent need special care. Attempted-speech systems are trained around intentional communication tasks; inner-speech research makes the boundary between intended messages and private cognition more sensitive. The demonstrations do not establish that a system can decode arbitrary thoughts, but designers still need user control and safeguards against unintended activation or disclosure.

For anyone who needs communication support now, a speech BCI is not the only relevant category. Augmentative and alternative communication (AAC) and speech-generating technology are distinct assistive approaches; they should not be mistaken for neural-signal-to-text systems. A clinician or speech-language professional can help identify appropriate communication options for an individual, while BCI research continues to address the burdens of surgery, calibration, accuracy, and sustained use.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why are researchers moving from text toward voice?
Text is a clear output channel, but spoken communication can convey more than words. Personalized voice synthesis can preserve characteristics such as a person’s voice and intonation, while avatar control can add a visual channel. The 2025 brain-to-voice demonstration using 256 implanted microelectrodes showed real-time synthesis that included intonation changes and short melodies; this illustrates expressive potential, not a removal of implantation or calibration requirements.
Streaming is another step toward natural interaction. A system that decodes continuously in short increments can begin producing output without waiting for an explicit stop signal or a complete sentence. That design addresses one source of conversational friction, though its practical value still depends on the accuracy, timing, and reliability of the complete system.

Frequently Asked Questions
Can a speech BCI send a message without someone intending to communicate?
The research described here does not establish reliable decoding of arbitrary thoughts or unintended messages. Because inner-speech work raises a sharper privacy concern, user control and a clear way to distinguish intended communication from other activity are important design questions.

Does a 97.5% BCI accuracy result mean only 2.5% of every conversation will be wrong?
Not necessarily. The reported accuracy figure is not directly interchangeable with word error rate, and a metric’s meaning depends on how a study defines and calculates it. Check the task, vocabulary, and measurement method before translating a study result into an expected conversation error rate.
Can someone use an implanted speech BCI at home today?
The demonstrations in this research are experimental medical neuroprostheses, not evidence of a generally available consumer product. A published research result alone does not establish access, eligibility, or suitability for an individual.
What can someone use while waiting for BCI research to advance?
AAC and speech-generating technology are separate assistive options that can support communication without being neural-signal decoders. A qualified clinician or speech-language professional can help match an option to a person’s needs and abilities.
