A biological indicator (BI) verifies whether a steam cycle inactivated a known population of highly resistant spores. A chemical indicator (CI) verifies that its specified process conditions—such as heat, time, moisture, or steam contact—were reached. A CI can show that a package was exposed and, when placed inside it, help assess sterilant penetration. It cannot prove that microorganisms were killed. Safe release decisions use mechanical records, chemical indicators, and biological indicators together; no single result proves that every item in a load is sterile.

The essential difference at a glance

Question Biological indicator Chemical indicator
What does it respond to? Survival or inactivation of a standardized population of resistant spores. One or more specified physical or chemical sterilization parameters, depending on the indicator design.
What does it tell you? Whether the biological challenge was inactivated under the test conditions. Whether the indicator experienced the conditions it was designed to detect; an internal CI also helps assess conditions inside a package.
When is the result available? After processing and incubation or readout according to the product instructions. Some systems provide rapid readout. Usually immediately after the cycle.
Routine role in CDC guidance At least weekly, and in every load containing implantable items. Mechanical and internal/external chemical monitoring for each load, with placement determined by the guidance and package design.
What it cannot establish A negative BI does not prove that every item in the load is sterile or was adequately exposed. A passed CI does not prove sterilization or microbial kill and cannot replace a BI.

These are complementary tests, not competing versions of the same test. The Centers for Disease Control and Prevention (CDC) describes biological indicators as the only process indicators that directly monitor the lethality of a sterilization process, while chemical indicators indicate exposure to the process. See the CDC explanations in Sterilizing Practices and Steam Sterilization.

What a biological indicator verifies

A deliberately difficult biological challenge

A steam BI contains a standardized population of resistant spores. For steam sterilization, CDC identifies Geobacillus stearothermophilus as the challenge organism. The spores are selected because they are harder to inactivate than many ordinary contaminating organisms. If the prescribed cycle inactivates that challenge under the test conditions, the BI supplies direct evidence of microbial lethality rather than merely evidence that a gauge reached a number.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What a negative and positive result mean

A negative BI means the spores did not survive the test and the indicator system reported no growth or other positive signal, as defined by its instructions. A positive BI means the challenge was not demonstrated to be inactivated. Read the result only with the product’s incubation, readout, control, and interpretation instructions; different commercial systems have different readout workflows.

A BI result is evidence about the indicator’s location and conditions, not a certificate covering every item. CDC’s glossary cautions that a negative BI does not prove that every item in a load is sterile or that all items experienced adequate sterilization conditions. Load configuration, packaging, air removal, moisture, instrument design, and placement still matter. The BI therefore strengthens a monitoring program without eliminating the need for mechanical and chemical checks.

When the answer is available

Traditional BIs require processing followed by incubation and interpretation. Rapid-readout products can shorten the time to a result, but the facility must follow the exact labeled procedure, including controls and any required incubation period. Do not infer a pass from an unread indicator or substitute a visual color change that the manufacturer does not define as a final result.

What a chemical indicator verifies

Response to process conditions

A CI contains a chemical system that changes when it encounters one or more specified sterilization conditions. Depending on its type, the response may represent exposure to a single parameter or a combination of parameters such as temperature, time, and steam. The indicator’s label and instructions define what its endpoint means; a color change should not be interpreted more broadly.

External indicators: identifying processed packs

An external strip, label, or tape indicator provides a quick visual distinction between a package that has been through a process and one that has not. It is useful for workflow control and prevents an unprocessed pack from being mistaken for a processed one. It does not show that the sterilant reached the inside of the package or that the load achieved microbial kill.

Internal indicators: checking conditions inside the package

An internal CI is placed within a pouch, wrapped set, container, or other package. Its response gives immediate information about the conditions at that location and can reveal a penetration problem that an external mark would miss. In dental settings, CDC emphasizes placing an internal chemical indicator in each package and using the result before the item is used; see Dental Sterilization and Disinfection.

If an internal indicator is visible from outside the package, CDC’s listed recommendation says a separate external indicator is not needed for that package. Facilities should still follow their written policy and the sterilizer and packaging instructions.

The immediate-result advantage—and its boundary

CIs are normally read at the end of the cycle, so they can support same-day package decisions while a BI is incubating. That speed makes them valuable for every load, but speed does not change the measurement: a CI reports its specified exposure response, not sterility. CDC says chemical indicators are convenient and inexpensive and indicate exposure; the same guidance says they should be used with biological indicators rather than replacing them.

Why neither result alone proves a sterile load

Sterility is a property of an item after an effective process, adequate packaging, correct handling, and protection from recontamination. A CI can respond even when the package interior was not adequately exposed, if its placement or type does not represent the hardest-to-sterilize location. A BI samples one challenge at one location and cannot inspect every hinge, lumen, wrapped layer, or container in the load. Mechanical records can also miss a local penetration problem if the chamber sensors show acceptable values.

For that reason, a negative BI should be described as a negative result for the tested indicator under the stated conditions—not as proof that every item is sterile. Likewise, a passed internal CI supports evidence of conditions at its location but does not establish kill. Release decisions should consider all available evidence and the facility’s validated procedures.

How mechanical, chemical, and biological monitoring fit together

Monitoring layer Primary evidence Typical question answered Characteristic limitation
Mechanical Recorded cycle time, temperature, pressure, and other sterilizer readings. Did the equipment record the programmed physical conditions? A chamber reading may not reveal poor air removal, a blocked lumen, or a cold spot inside a package.
Chemical External and internal indicator responses. Did the indicator experience its specified process conditions, including at the package interior when placed there? It does not directly demonstrate microbial kill.
Biological Inactivation or survival of a resistant spore challenge. Was lethality demonstrated for the tested challenge at its location? It does not prove that every item in the load was sterile or adequately exposed.

Using the layers together gives different kinds of information: mechanical monitoring can expose a cycle deviation, CIs provide immediate package-level feedback, and BIs challenge the process biologically. A disagreement is a signal to stop and investigate, not a reason to select whichever result is most convenient.

CDC monitoring frequencies and special cases

Situation CDC recommendation Operational implication
Every sterilizer load Record mechanical readings and use chemical monitoring, including internal and external indicators as applicable. Review the cycle record and indicator results before releasing the load.
Routine biological monitoring Use an FDA-cleared commercial spore preparation at least weekly, intended for the sterilizer type and cycle parameters. For steam, the example organism is Geobacillus stearothermophilus. Schedule BI testing at least weekly even when no implantable items are processed.
Load containing implantable items Use a BI for every such load and quarantine whenever possible until the BI is negative. Do not treat a passed CI as a substitute for the BI or bypass the facility’s quarantine process.
Internal CI visible from outside the package An external indicator is not needed under the listed CDC recommendation. Ensure the internal indicator is actually visible and interpretable without opening the package.

These are recommendations, not measured accuracy statistics comparing one indicator type with another. Apply the current CDC guidance at Recommendations for Disinfection and Sterilization in Healthcare Facilities together with the sterilizer, indicator, packaging, and facility instructions.

Step-by-step: monitoring a steam sterilization load

  1. Confirm the cycle and load. Identify the sterilizer, cycle parameters, load contents, packaging system, and whether the load includes implantable items. Use only indicators labeled for that sterilizer type and cycle.
  2. Check the sterilizer before loading. Verify that required maintenance is current, the chamber and drain are in the condition specified by the manufacturer, and the selected program matches the load. Do not alter validated settings to force a result.
  3. Choose the BI correctly. Select an FDA-cleared commercial steam BI intended for the cycle parameters. Follow the product instructions for placement, controls, incubation, and readout.
  4. Place chemical indicators where they provide useful evidence. Apply an external indicator when required by policy or when the package has no visible internal indicator. Place an internal CI in each package or set as directed, especially at a location considered difficult for steam penetration.
  5. Load without blocking exposure. Arrange packs, pouches, containers, and instruments according to the sterilizer and packaging instructions. Avoid overloading, contact that impedes steam circulation, or configurations that conceal the internal indicator.
  6. Run the validated cycle and retain the mechanical record. Record the cycle identification and the displayed time, temperature, pressure, and any alarms or aborts. An interrupted or alarmed cycle is not a successful cycle merely because an indicator changed.
  7. Read the CIs before release. Confirm the expected endpoint on every required external and internal indicator. If an indicator is incomplete, abnormal, missing, or unreadable, hold the package and follow the facility’s failure procedure.
  8. Process and interpret the BI. Incubate or read the BI exactly as labeled, include the required control, and document the result with the load record. For implantable loads, quarantine whenever possible until the BI is negative.
  9. Release, hold, or reprocess based on the complete record. Release only when the mechanical record, required CIs, packaging condition, and BI policy support release. A torn, wet, opened, or otherwise compromised package is not protected by a favorable indicator result.

What to do when an indicator fails

An external or internal CI does not show the expected endpoint

  • Do not use the item or release the affected package.
  • Check whether the correct indicator was used, whether it was placed and read as directed, and whether the package was exposed to the intended cycle.
  • Review the mechanical record for an abort, alarm, incorrect cycle, abnormal time, temperature, pressure, or other deviation.
  • Notify the responsible supervisor or infection-prevention contact according to facility policy, identify other packages from the load, and reprocess or investigate as directed.

A CI failure is an immediate warning about the package or process conditions. It is not proof by itself that every microorganism survived, but it is sufficient reason not to use the item until the problem is resolved.

A steam BI is positive

CDC’s protocol for a positive steam biological indicator calls for a controlled investigation rather than an automatic assumption that every prior load must be recalled. Take the sterilizer out of service, notify the relevant supervisor or infection-control function, and check the cycle settings, loading and indicator use, maintenance status, steam supply, and BI interpretation. Confirm that the BI was the correct product for the sterilizer and cycle and that controls were valid.

Repeat BI testing in three consecutive cycles according to the CDC protocol. If those repeat indicators are negative, the sterilizer may return to service under the protocol and local requirements. If repeat tests remain positive, treat implicated items as nonsterile, recall and reprocess them, and obtain maintenance and manufacturer review as needed. The protocol is specific to steam; do not transfer it automatically to other sterilization methods. Details are in CDC’s Protocol for a positive steam biological indicator.

CDC also states that a single positive steam spore test does not require recall of non-implantable objects unless a sterilizer defect, procedure defect, or inappropriate cycle settings are identified. Implantable items and any situation involving a confirmed defect require the more conservative actions in the facility’s policy and the CDC protocol.

How to select and place indicators

Match the label to the equipment and cycle

Indicator compatibility is not generic. Verify the labeled sterilizer type, cycle parameters, intended use (steam), storage conditions, expiration, and readout method. A product listing that merely says “spore test” or “sterilization tape” is not evidence that it is FDA-cleared or compatible with a particular cycle. Use the manufacturer’s instructions and retain the labeling required by your quality system.

Represent the hardest-to-sterilize location

Place the BI and internal CI where steam penetration is most challenging for the validated load configuration, not where they are easiest to see. Follow the packaging and sterilizer instructions for lumened instruments, wrapped sets, containers, and pouches. The indicator must remain readable after the cycle and must not be positioned so that its response is masked by a label, fold, or container wall.

Document enough to reconstruct the decision

Record the sterilizer identifier, cycle, date, load contents or tracking number, mechanical results, CI outcomes, BI lot or identifier, operator, and any corrective action. Documentation allows an abnormal result to be connected to the affected packages and prevents a later reviewer from mistaking an untested or unreadable indicator for a pass.

Common mistakes and their fixes

  • Using tape as proof of sterility: Tape is generally an external exposure marker. Use an appropriate internal CI and the required BI program.
  • Skipping internal indicators: An external change cannot show what happened inside a dense or wrapped package. Place the internal indicator as required.
  • Running BIs only after a problem: CDC recommends routine BI monitoring at least weekly, not only during troubleshooting.
  • Using a BI for the wrong cycle: Select a product labeled for the actual steam cycle parameters and follow its interpretation instructions.
  • Releasing an implantable load before the BI result: Quarantine whenever possible until the BI is negative.
  • Ignoring an abnormal mechanical record because the CI changed: A CI cannot override an aborted cycle, alarm, or incorrect setting.
  • Calling a negative BI a guarantee: Describe it as a negative result for the tested indicator and interpret it with the other monitoring evidence.
  • Repeating a positive BI without taking the sterilizer out of service: Follow the CDC positive-indicator protocol, including investigation and three-cycle repeat testing.
  • Assuming a dry, intact package is sterile: Package appearance is only one release check and does not replace monitoring results.
  • Reusing or storing indicators outside their instructions: Expired, damaged, improperly stored, or unrecorded indicators can invalidate the monitoring decision.

Practical examples

External CI passes, internal CI fails

The outside tape shows exposure, but the internal indicator did not reach its specified endpoint. Hold the package, review loading and cycle records, and reprocess or investigate according to policy. The external mark does not cancel the internal failure because the two indicators answer different questions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mechanical record is normal, CI and BI are negative

This is the expected combination for a monitored load: the sterilizer recorded the programmed physical conditions, the package indicators responded as specified, and the biological challenge was negative. Document all three layers and still inspect packaging integrity before use. The BI result remains evidence about its test location, not a guarantee for every item.

One positive BI in a non-implantable load

Remove the sterilizer from service and begin the CDC investigation. Do not automatically recall every non-implantable item solely because of that one result; CDC’s recommendation distinguishes a single positive from a confirmed equipment, procedure, or settings defect. Follow the repeat-testing and disposition steps, and involve infection prevention and maintenance.

Positive BI repeats in three cycles

Persistent positives indicate that the sterilizer or process cannot be relied upon. Treat implicated items as nonsterile, recall and reprocess them, and arrange technical evaluation and manufacturer review. Keep the sterilizer out of service until the cause is corrected and the required testing supports return.

Where to verify current requirements

Facility policies should be based on the current instructions for the specific sterilizer, packaging system, and indicator, plus applicable regulation and professional guidance. CDC’s Glossary defines indicator terminology and explains the limits of a negative BI. The broader monitoring discussion appears in Sterilizing Practices, while steam-specific context is available in Steam Sterilization.
