For most teams, start with a practical readiness toolkit that combines an anonymous questionnaire, scoring, interpretation and an action plan. The Leaders Drive Change Change Readiness Assessment & Action Toolkit is designed to reveal readiness strengths, pockets of resistance and existing change load. Choose the 12-item Organizational Readiness for Implementing Change (ORIC) instrument when you need a concise, defined implementation measure across roles. Use Prosci ADKAR to locate individual adoption bottlenecks, but pair it with team-level questions about leadership, communication, trust, resources and psychological safety.
No single score explains resistance. People may oppose a change because the case for it is unclear, because they do not trust sponsors, or because they lack time, staffing, systems or training. The best assessment therefore measures both individual adoption and the team conditions that make adoption possible.
What a change-readiness assessment should measure
Readiness is multidimensional. A useful assessment tests whether people understand the problem and the desired future state, believe the change is worthwhile, feel supported by leaders and managers, have the ability and resources to work differently, and feel safe raising concerns.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Resistance is often a signal rather than a character flaw. A low score can identify an unanswered question, a conflicting priority, weak sponsorship, poor peer norms, inadequate training or an unrealistic workload. Treating the signal as information produces a better intervention than labelling a team as “difficult.”

- Leadership and manager support: visible sponsorship, consistent messages and practical help.
- Trust and psychological safety: confidence that concerns can be raised without punishment.

A 2020 systematic review of organizational-readiness instruments mapped 897 of 1,370 survey items (68%) to the CFIR “inner setting” domain. The most frequent constructs were readiness for implementation, networks and communication, implementation climate, structural characteristics and culture. That finding explains why a purely individual attitude survey misses important causes of resistance. It also shows why questions should be tailored to the specific change rather than copied unchanged from a generic instrument.
The best tools, matched to the job
Leaders Drive Change Change Readiness Assessment & Action Toolkit: best all-round practical option
This paid digital download is built for leadership teams, departments and whole organizations. It includes a PDF assessment, a Sheets/Excel scoring model, an insights and themes guide, and an action-planning template. Its focus is practical: where are the strengths, where are the pockets of resistance, and how much change load are people already carrying?
Choose it when the assessment must lead directly to a facilitated conversation and an owned action plan. The scoring workbook can help a sponsor compare themes and decide where to intervene, while the interpretation guide reduces the risk of treating a single average as the whole story. It is also suited to repeating the pulse as a rollout progresses. Current price, licensing limits and program terms should be checked with Leaders Drive Change before purchase because those commercial details can change.
ORIC: best concise, research-based implementation measure
PhenX describes Organizational Readiness for Implementing Change (ORIC) as a 12-item instrument. Respondents rate agreement on a 1-to-5 scale. It is intended for pre-implementation studies and is most useful when you need a short, defined measure that can be administered consistently across several roles.
Collect responses from multiple roles rather than asking only the project team. Comparing managers, frontline staff, support functions and affected specialists can reveal barriers that disappear in an organization-wide average. ORIC is deliberately brief and focused on implementation readiness; it does not, by itself, provide a detailed action-planning workbook or a full diagnostic of trust, workload or psychological safety. Add those team-level questions when they matter to the change.
Prosci ADKAR plus team questions: best for individual adoption bottlenecks
Prosci ADKAR describes five observable elements of individual change:

- Awareness of the need for change.
- Desire to participate and support it.
- Knowledge of how to change.
- Ability to apply the new skills and behaviours.
- Reinforcement to sustain the change.
ADKAR helps a change lead identify where adoption is stalling for a particular person, role or group and match support to that stage. For example, a knowledge gap calls for instruction, while an ability gap may require coaching, practice time or system changes.
ADKAR is an individual-change model within a broader methodology. Used alone, it cannot measure organizational climate, cross-team capacity, communication networks or cultural conflict. Pair it with questions about manager support, leadership visibility, trust, resources, peer expectations and psychological safety to explain why a whole team is stuck.
National, government and university templates: best free adaptable references
The National Collaborating Centre for Methods and Tools lists a Change Assessment Instrument, a Change Readiness Survey and a survey covering opinions about change goals, anticipated impact, planning and implementation. These references are useful when you need adaptable prompts without buying a packaged toolkit.
The Government of Zambia’s tool defines readiness across four areas: strategy, methodology, capability and capacity. It is designed to locate conflicts among personnel, teams, departments or organizational cultures. That makes it useful for a complex program in which resistance may come from competing mandates or incompatible ways of working, not just from an individual’s attitude.
California’s organizational change-management framework provides a readiness template for executives, managers, employees and project teams, along with sponsorship and team-dynamics resources. It is particularly useful when different audiences need different versions of the assessment and when sponsor behaviour must be assessed alongside employee readiness.

How to choose without overbuying
- Define the decision you need to make. If the question is “Can we begin?”, use ORIC or a broad readiness toolkit. If it is “Why is this role not adopting the new process?”, use ADKAR. If it is “Which departments are in conflict?”, use a framework that covers culture, capacity and competing priorities.
- Choose the smallest instrument that answers that decision. A 12-item ORIC pulse is easier to repeat than a long diagnostic. A fuller toolkit earns its place when interpretation and action planning would otherwise be improvised.
- Set the unit of analysis before collecting data. Decide whether results will be reported for the whole organization, each team, each role or a combination. Do not promise team-level findings if some teams are too small to protect anonymity.
- Plan the action meeting first. Name the sponsor, facilitator and owners who will respond to low scores. An assessment without a decision and follow-up process teaches employees that speaking up changes nothing.
- Schedule a repeat measurement. Take a baseline before rollout, reassess after early implementation and repeat after a defined intervention. Keep core items unchanged so movement is interpretable, while adding a few phase-specific questions when necessary.
A practical 15-item pulse survey
Use a five-point agreement scale such as 1 = strongly disagree, 3 = neither agree nor disagree and 5 = strongly agree. Add “not enough information to answer” where a respondent may genuinely lack visibility; do not force a neutral answer to an information gap.
| Dimension | Sample statement | What a low result suggests |
|---|---|---|
| Awareness | I understand why this change is needed now. | The case for change or timing is unclear. |
| Future state | I understand what will be different in our team after implementation. | The target process or outcome is not concrete. |
| Personal impact | I understand how this change will affect my role and priorities. | People are filling an information gap with assumptions. |
| Team impact | Our team has discussed how the change will affect customers and handoffs. | Cross-team consequences have not been worked through. |
| Desire | I believe the benefits of this change outweigh its disruption. | Benefits are unconvincing or costs feel excessive. |
| Leadership | Senior leaders are visibly consistent in supporting this change. | Sponsorship is weak, contradictory or remote. |
| Manager support | My manager supports the change and makes time for questions. | Local coaching or permission to raise concerns is missing. |
| Trust | I trust the people leading this change to act on serious concerns. | Past decisions or communication have damaged credibility. |
| Psychological safety | I can disagree with the change without negative consequences. | Resistance may be hidden rather than absent. |
| Knowledge | I know what I need to learn to work in the new way. | Training needs are undefined or unavailable. |
| Ability | I will have enough practice and help to perform the new process. | Instruction alone will not overcome a capability gap. |
| Resources | We will have the time, staffing and systems required to adopt this change. | Capacity or infrastructure is the binding constraint. |
| Peer norms | People whose opinions matter on our team support trying the new way. | Informal influencers may undermine adoption. |
| Reinforcement | Our measures, recognition and routines will reinforce the new behaviour. | The organization may revert after launch. |
| Participation | I know how to raise a problem or suggest an improvement during rollout. | Feedback channels and ownership are unclear. |
Add two open questions at the end: “What is the biggest risk to successful adoption?” and “What is one action leaders should take before rollout?” Code comments into themes rather than publishing identifiable quotations.
Protect anonymity while segmenting results
Segmenting by role or team is valuable because an organization-wide average can hide a serious pocket of resistance. It also creates a privacy risk. Set a minimum reporting group size before launch, suppress small cells, and combine similar teams when necessary. Do not show a manager a cut of responses that can be inferred from one or two people.
Recommended Free Tools
Explain who can see raw responses, who receives only aggregated results, how long data will be retained and whether open-text comments will be edited for identifying details. Use a neutral survey administrator when employees may fear that their manager will recognize them. Report response counts beside every score so readers can judge how stable a result is.
Compare distributions, not only means. A team with an average of 3.2 could be uniformly uncertain, or it could contain a group of strong supporters and a smaller group experiencing severe risk. Cross-tabulate role, location or shift only where the sample is large enough to preserve confidentiality.
Score and interpret the results
Use item and dimension patterns
Calculate an average for each dimension only after checking response counts and missing answers. Look for clusters: low Awareness plus low Future State points to communication; high Awareness but low Resources points to capacity; high Desire but low Ability points to training, practice or system friction. Read open comments alongside the numbers to identify the specific barrier.

Use thresholds as guidance, not a verdict
The California readiness guide says an average below 4.0 in a category indicates that the organization is not fully prepared and should focus effort there. Apply that benchmark only in the context of that guide. It is not a universal pass mark for every instrument, culture or change. ORIC’s 1-to-5 responses should be interpreted with the instrument’s intended construct and with role-level comparisons, not by borrowing a cutoff from another framework.
Separate resistance from lack of information
A low score on “I understand how this affects my role” is not evidence of opposition. First provide the missing information, then measure again. A low score on trust or psychological safety requires a different response, such as visible follow-through on concerns or a safer feedback channel. Labeling both conditions “resistance” will send the wrong intervention to both groups.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Turn findings into an action loop
- Share the themes quickly. Within an agreed time, publish the response rate, major strengths, lowest-scoring dimensions and the questions leaders cannot yet answer. Do not publish identifiable comments.
- Prioritize a small number of barriers. Select the two or three issues that most threaten adoption, considering both score and operational impact. A low score that blocks a critical handoff may deserve priority over a lower score in a less consequential area.
- Assign named owners and dates. Each action needs an accountable owner, a deliverable and a review date. “Improve communication” is not an action; “hold two role-specific workflow demonstrations and publish the unanswered-questions log by Friday” is.
- Match the intervention to the diagnosis. Clarify the case for Awareness problems, involve affected staff when Desire is weak, provide training for Knowledge gaps, create supervised practice for Ability gaps, and change measures or recognition when Reinforcement is low. Address staffing, systems and conflicting priorities directly rather than asking for more enthusiasm.
- Reassess before the next gate. Repeat the core pulse before rollout, after early implementation and after the first corrective actions. Escalate issues that remain low across two measurements instead of declaring them normal.
- Use readiness to sequence implementation. AHRQ states that readiness assessment should be a team activity because team buy-in is important to implementation success. Its guidance also recommends culture surveys to identify strengths and weaknesses and considering incremental implementation in units that appear more ready. A pilot can therefore be a risk-control decision, not a reward for one team.
Common failure modes and fixes
Running a survey with no visible follow-through
Symptom: response rates fall and open comments become cynical. Fix: publish the decision timeline before launch, report themes promptly and show which owner is responsible for each response.
Using one average for every audience
Symptom: executives see readiness while a frontline shift is struggling. Fix: collect role- and team-level data, apply minimum group sizes and investigate meaningful differences.
Calling every low score resistance
Symptom: more messaging is sent when the real problem is staffing, system access or training. Fix: map each low item to Awareness, Desire, Knowledge, Ability, Reinforcement, leadership, resources or safety before selecting an intervention.

Borrowing a cutoff from another instrument
Symptom: a team is labelled ready because it cleared an arbitrary number. Fix: follow the selected tool’s scoring guidance, examine distributions and compare the same items over time.
Frequently asked questions
Frequently Asked Questions
Which tool should a small team use first?
Use a short anonymous pulse based on the 15 sample statements, or ORIC when a defined 12-item implementation measure is sufficient. Add open comments and a scheduled action meeting so the result leads to a decision.
Can ADKAR measure team resistance by itself?
No. ADKAR diagnoses individual adoption stages. Add team questions about leadership, communication, trust, resources, peer norms and psychological safety to explain collective patterns.
How many responses are needed to report a team score?
There is no universal number. Set a minimum reporting group size before launch, suppress small cells and combine groups when individual responses could be inferred.
Should readiness be measured once or repeatedly?
Measure a baseline before rollout, repeat after early implementation and reassess after corrective actions. Keep core items unchanged so movement can be compared over time.
