Neither a baby wrap nor a structured carrier is automatically better or safer for every newborn. Choose a specific model whose instructions allow your baby’s current size, then make sure you can fit it correctly, keep the baby’s face visible and airway clear, and support the baby’s body. A wrap may suit a caregiver comfortable tying fabric; a structured carrier may suit someone who prefers a pre-shaped carrier. Those are practical preferences, not proven category-wide advantages.
How to compare the two carrier types

Keep the newborn’s airway clear
The AAP warns that some sling positions can curl a baby into a C-shape, which may increase breathing problems. In any carrier, keep the face visible, the neck straight, the chin away from the chest, and the nose and mouth clear; check frequently for blocked airflow. The AAP’s guidance says the U.S. Consumer Product Safety Commission (CPSC) warns of suffocation hazards in infant slings, particularly for babies younger than four months. This is a reason to be especially attentive with a newborn, not evidence that every wrap is unsafe or that a structured carrier is automatically safe.
The AAP also says premature infants and infants with respiratory problems should not be placed in backpacks or other upright positioning devices because the position may make breathing harder. Ask your child’s clinician for advice about an individual baby’s needs.
Support the back and hips
For hip positioning, IHDI recommends letting the hips spread naturally, with the thighs supported and the hips and knees bent. It says wraps generally promote this position, but warns that wraps and other devices may interfere with positioning if used incorrectly, especially for extended periods. Fit should match the infant’s size; IHDI says little is known about intermediate positions and that short periods are unlikely to cause harm.

IHDI summarizes an April 20, 2023 study in the Journal of Orthopedic Research involving 13 full-term infants: 10 with healthy hips and three with mild hip dysplasia. Researchers assessed wide-base carriers that supported the thighs to the knees and narrow-base carriers that supported the baby in the groin area. IHDI reports that the hips were more fully in the socket in wide-base carriers, with position differences more noticeable for infants with hip dysplasia. This small study does not establish that every structured carrier has a wide base or that every wrap provides hip-healthy positioning. IHDI’s summary of the study

Choose by the baby’s fit and your ability to use it
- Check the exact model’s instructions. Confirm that its minimum size or weight and newborn configuration cover your baby. Do not assume every wrap or structured carrier is suitable from birth.
- Try the fit with your baby. The AAP advises taking the baby along when purchasing. Check that the back is supported and the leg openings are small enough to prevent slipping through.
- Check the position after fitting. Make sure the face remains visible, the nose and mouth clear, the chin off the chest, and the thighs supported with hips and knees bent.
- Inspect the carrier. Periodically check seams and fasteners for damage, as the AAP advises.
- Use the style you can adjust reliably. A wrap involves tying and adjusting fabric; a structured carrier is pre-shaped. Neither shape removes the need to follow its instructions and monitor the baby.
Which one should you choose?
Choose a wrap if you are comfortable learning its ties and can maintain a secure fit and clear airway. Choose a structured carrier if you prefer its pre-shaped design and the particular model fits your newborn as instructed. If either option leaves the baby’s face difficult to see, the position uncertain, or the fit insecure, it is not the right choice for that use.
