Not yet, as a broad category. Artificial blood is a group of experimental oxygen-delivery technologies, not one finished product. Human trials of hemoglobin-based oxygen carriers (HBOCs) found serious cardiovascular and organ toxicities, and no HBOC has U.S. Food and Drug Administration approval for routine human use. Perfluorocarbon-based oxygen carriers (PFOCs) are also investigational. Safety has to be demonstrated separately for each product, intended use and regulatory jurisdiction.
What “artificial blood” actually means
Conventional donated blood does several jobs at once: red cells carry oxygen, plasma supports fluid balance and clotting, and other components contribute to immunity and hemostasis. Most products called artificial blood are narrower substitutes designed primarily to transport oxygen when red cells are unavailable or cannot be given. They do not automatically replace platelets, clotting factors or plasma.
Hemoglobin-based oxygen carriers (HBOCs)
HBOCs use purified or modified hemoglobin outside intact red blood cells. The molecule can be chemically cross-linked, polymerized, encapsulated or otherwise engineered to remain in the circulation and bind oxygen. Removing the red-cell membrane creates potential advantages such as compatibility with blood groups and simpler storage, but it also exposes hemoglobin to tissues in a form that behaves differently from hemoglobin inside a cell.
Perfluorocarbon-based oxygen carriers (PFOCs)
PFOCs use synthetic perfluorocarbon compounds, usually formulated as emulsions, to dissolve and transport oxygen and carbon dioxide. Their performance depends on particle formulation, dose and the oxygen available in the lungs. They are pharmacologically active formulations rather than bags of donor red cells, and their persistence, clearance and emulsion behavior must be established for each design.

Why one safety answer is impossible
An HBOC and a PFOC have different chemistry, circulation time, metabolism and toxicology. A product intended for emergency trauma may have a different risk-benefit balance from one intended for planned surgery or chronic anemia. A product can also have a different legal status in different countries. “Artificial blood” therefore describes a research field, not a single approved treatment.
Where approval stands today
FDA scientists reported in 2018 that every HBOC tested in humans had failed clinical studies because of toxicity and that the FDA had not approved an HBOC for human use. The NIH Blood-Based Products Regulatory Knowledge Guide likewise states that no FDA-approved oxygen-carrying blood-substitute product exists commercially. A 2024 review reported no FDA- or EMA-approved HBOC or PFOC at the time of publication.
| Product or class | Status described by the cited sources | What that status does not mean |
|---|---|---|
| HBOCs as a class | No FDA approval for clinical use; human trials showed toxicity and/or failure to meet required endpoints. | It does not prove that every future molecular design will fail, but it does mean safety cannot be inferred from oxygen-carrying performance alone. |
| PFOCs as a class | Investigational; no FDA or EMA approval was identified in the 2024 review. | “Experimental” does not establish either safety or effectiveness for routine transfusion. |
| Hemopure (HBOC-201, hemoglobin-glutamer-201) | Described in a 2025 review as approved for canine anemia in the United States and European Union, approved for human use in South Africa in 2001 and Russia in 2006, and available for expanded-access use in the United States. | Those permissions are not FDA commercial approval for routine human transfusion in the United States. |
Approval is indication-specific as well as product-specific. A permission for veterinary use, a foreign human authorization or a U.S. expanded-access case does not establish that the same product can be prescribed or marketed as a standard blood replacement in the United States.
Why early HBOCs caused safety problems
Free hemoglobin changes vascular signaling
Inside red cells, hemoglobin is physically separated from the vessel lining and from many circulating molecules. Once released into plasma, it can react with nitric oxide, an important signaling molecule that normally helps blood vessels relax. Nitric-oxide scavenging can produce intense vasoconstriction and hypertension. The FDA describes possible consequences including heart attack and stroke.
Oxidation and reactive injury
After releasing oxygen, cell-free hemoglobin can oxidize into chemically reactive forms. Those species can damage lipids, proteins and cell membranes and can trigger inflammatory responses. The resulting injury is not limited to one organ: studies and regulatory discussions have raised concerns involving the kidneys, heart, brain and lungs.

Oxygen delivery is not the same as clinical benefit
Several development programs focused on biochemical or physiological oxygen-delivery measures. Clinical trials nevertheless showed elevated adverse-event rates and/or failed to meet prespecified endpoints. A carrier may raise measured oxygen content while worsening blood pressure, perfusion or organ function. Historical HBOC experience established that an oxygen-delivery endpoint cannot substitute for outcomes such as survival, cardiovascular events, kidney injury and functional recovery.
Heterogeneous products create unpredictable risk
FDA scientific work emphasizes that HBOCs are chemically heterogeneous. Molecular size, cross-linking, polymer distribution, impurities and other structural features affect oxygen binding, release, circulation and toxicity. Two products both labeled “hemoglobin-based” can therefore have materially different risk profiles. Regulators need a defined composition and a mechanistic explanation for how each component behaves in the body.

PFOCs avoid some problems associated with free hemoglobin, but they introduce their own uncertainties. A 2024 review identified pharmacologic side effects, nanomaterial and formulation complexity, persistence in the body and weak clinical-trial designs as major barriers to translation.
Formulation and particle behavior
PFOCs are commonly administered as emulsions. Droplet size, surfactants, stabilizers, concentration and manufacturing conditions can change distribution and tolerability. Developers must show that the formulation remains stable during storage and after administration, and that particles do not create unacceptable inflammatory, vascular or organ effects.
Persistence and clearance
Long residence in the body may help maintain oxygen transport, but it can also prolong adverse effects. A sponsor must characterize where the material goes, how it is metabolized or eliminated, whether repeated doses accumulate and how impaired kidney, liver or lung function changes exposure.

Clinical evidence remains limited
The absence of an approval is not a finding that all PFOCs are ineffective. It means the available evidence has not established a product-specific balance of benefit and risk acceptable to the relevant regulator. Better-designed trials, clearer indications and complete follow-up are required before a PFOC could be considered for routine use.

What regulators require before human use
FDA oxygen-therapeutics guidance and EMA blood-product guidance frame development as a sequence of linked demonstrations. A promising laboratory result cannot compensate for an undefined product, inadequate toxicology or an uninformative trial.
1. Define the product and its mechanism
- Specify the chemical composition, molecular or particle-size distribution, impurities and degradation products.
- Measure oxygen-binding and oxygen-release behavior under clinically relevant conditions, not only in a simplified laboratory solution.
- Characterize stability, storage limits, container compatibility and changes after dilution or preparation.
- Explain expected interactions with vessel walls, nitric-oxide signaling, blood cells, plasma proteins and immune pathways.
For HBOCs, the submission must connect structural features to oxygen transport and toxicity. For PFOCs, it must also explain emulsion architecture, particle behavior and clearance.
2. Complete a relevant preclinical safety package
Animal and laboratory studies should examine blood pressure, vasoconstriction, oxidative stress, inflammation, thrombosis and complement or immune activation. Organ-specific assessments normally include kidney, heart, brain and lung injury, along with laboratory markers and tissue examination.
Testing must also address pharmacokinetics, dose response, distribution, metabolism and clearance. Risk can change in people with aging, diabetes, atherosclerosis, sepsis, kidney disease or other conditions common in the proposed population, so models should test clinically relevant comorbid states where feasible. FDA discussions have identified unresolved safety questions for both hemoglobin products and perfluorochemical emulsions.

3. Establish manufacturing and quality controls
A sponsor must manufacture reproducible lots and demonstrate sterility, identity, purity, potency, stability and dose control. Critical process parameters and release tests should detect changes in molecular size, oxidation state, particle distribution, residual reagents, endotoxin or other impurities. Chemistry, manufacturing and controls evidence forms a core part of a biologic or drug submission; a product that cannot be made consistently cannot be evaluated consistently.
4. Choose an ethically justified indication and comparator
Trials must specify when the product is intended to be used, what conventional care patients would otherwise receive and why exposing participants is reasonable. The comparator might be standard red-cell transfusion, another accepted treatment or a clearly justified control, depending on the indication. Emergency use does not remove the need to define eligibility, dosing, rescue treatment and consent procedures.
5. Predefine meaningful efficacy and safety outcomes
Endpoints should reflect patient benefit, such as survival, avoidance of organ failure, recovery or transfusion reduction, rather than relying only on oxygen content or a short-term laboratory marker. Sample size must be large enough to detect clinically important harms. Serious adverse events, cardiovascular events, blood-pressure changes, kidney injury, neurologic outcomes, pulmonary complications and delayed toxicity require systematic collection and independent review.
6. Submit for marketing-authorisation review
FDA review assesses whether the evidence supports safety and efficacy for the proposed use and whether manufacturing controls are adequate. EMA guidance helps developers prepare marketing-authorisation applications and evaluate clinical efficacy and safety for blood-related products. A regulator can restrict an indication, require additional studies or decline authorization when benefits do not outweigh risks.
How a credible clinical-trial program should work
- First-in-human studies: Start with carefully selected participants, conservative dosing and intensive monitoring of blood pressure, oxygenation, cardiac status, kidney function and laboratory toxicity. Escalation should stop when predefined safety limits are reached.
- Early patient studies: Test the intended population and dosing schedule while continuing pharmacokinetic and organ-safety assessments. Investigators should document rescue transfusions and protocol deviations rather than treating them as irrelevant failures.
- Confirmatory trials: Use an adequately powered, prespecified comparison with standard care and endpoints that matter to patients. Mortality, major cardiovascular events and organ injury must be analyzed even if the product meets an oxygen-delivery target.
- Follow-up and surveillance: Track delayed effects, repeated exposure, immune reactions and outcomes in patients excluded from early trials. Rare harms may only become visible after broader use, so post-authorisation monitoring is part of the safety case.
Trial design is especially important in trauma and other emergencies, where illness severity, coexisting shock and rapidly changing treatment can obscure both benefits and harms. Randomization, transparent stopping rules and independent safety oversight help distinguish a useful carrier from one that merely changes laboratory measurements.

Artificial carriers versus donated red cells
| Comparison point | Artificial oxygen carriers | Donated red-cell transfusion |
|---|---|---|
| Oxygen delivery | Depends on the product’s binding or dissolving chemistry, dose and circulation time; performance must be demonstrated clinically. | Provided by intact donor red cells with established oxygen transport, subject to compatibility and storage considerations. |
| Cardiovascular and organ toxicity | HBOCs have shown vasoconstriction, hypertension, oxidative injury and organ-toxicity concerns; PFOC risks are formulation-specific and remain under study. | Has known transfusion risks that are managed through screening, matching, storage controls and monitoring. |
| Immunogenicity | May avoid some red-cell antigen matching, but proteins, particles, impurities or excipients can trigger immune or inflammatory reactions. | Compatibility testing and blood-group matching are required; reactions remain possible. |
| Circulation and clearance | Must be established for each molecule or formulation, including persistence and repeated-dose accumulation. | Red-cell survival and clearance are characterized clinically, though they vary with storage and recipient condition. |
| Storage and logistics | Could offer logistical advantages if long-term stability is proven, but no broadly approved commercial substitute currently provides that solution. | Requires blood collection, testing, cold-chain management and inventory planning. |
| Regulatory evidence | Requires product-specific chemistry, toxicology, manufacturing, efficacy and serious-adverse-event evidence. | Operates under established transfusion standards and blood-safety systems. |
What Hemopure can—and cannot—tell patients
Hemopure is often cited because it is a named HBOC with regulatory history outside the United States. The 2025 review describes Hemopure as approved for canine anemia in the United States and European Union, authorized for human use in South Africa in 2001 and Russia in 2006, and available through U.S. expanded access.
Expanded access allows use of an investigational product for a patient who may have no satisfactory alternative under specific regulatory and clinical conditions. It is not the same as FDA approval for routine marketing. A clinician cannot treat Hemopure as an interchangeable substitute for standard transfusion merely because it has a foreign authorization or veterinary approval. The indication, consent process, institutional oversight, supply and emergency plan all matter.
What happened to the promise of blood substitutes?
The original appeal was practical: a product that could be stored longer, transported without matching every blood group and supplied when donated blood was scarce. Those advantages remain attractive for disaster response, remote medicine and severe bleeding. Development stalled because the same modifications intended to keep oxygen carriers in circulation also changed how they interacted with blood vessels and organs.

Elevated adverse-event rates, cardiovascular toxicity and failures on specified endpoints prevented a class-wide breakthrough. Current work has shifted toward more controlled molecular engineering, encapsulation, better formulations and narrower indications, but each candidate still has to clear the same product-specific evidence threshold.
How to evaluate a new “synthetic blood” claim
- Identify the technology: HBOC, PFOC or another oxygen-therapeutic approach.
- Check the regulator and jurisdiction. A foreign approval, veterinary label or expanded-access pathway is not U.S. FDA commercial approval.
- Look for a registered clinical trial with a defined indication, comparator, sample size and prespecified endpoints.
- Ask whether results include mortality, cardiovascular events, kidney injury and other serious harms, not just oxygen measurements.
- Confirm that manufacturing consistency, sterility, stability, dose and impurity controls are described.
- Distinguish a peer-reviewed review or press release from an actual marketing authorisation.
For patients and families, the practical question is not whether a product is called artificial blood. It is whether a named product is authorized for the proposed use, supported by adequate human evidence and available through a clinician who can provide the required monitoring and rescue care.
Bottom line
Artificial blood products cannot currently be described as broadly safe for routine transfusion. Human-tested HBOCs exposed serious risks from free hemoglobin, including vasoconstriction, hypertension, oxidative injury and organ toxicity; PFOCs remain investigational with unresolved pharmacology, formulation, persistence and trial-design issues. Regulators require a complete, product-specific chain of evidence—from molecular characterization and preclinical toxicology through controlled clinical trials, manufacturing validation and long-term safety monitoring—before marketing authorization is justified.
