Short answer: A blood test is usually the easier first biomarker check, while an amyloid or tau PET scan is a more resource-intensive way to image Alzheimer’s-related pathology in the brain. A validated blood test can be accurate enough for triage—and, in specialized memory-care settings, may substitute for PET or cerebrospinal-fluid testing when it meets strict performance thresholds. Neither test is a stand-alone diagnosis: results must be interpreted with the person’s history, cognitive examination and other medical information.

What an Alzheimer’s blood test and a PET scan actually measure

These tests answer related but different questions. Blood testing measures Alzheimer’s-associated proteins circulating in plasma. PET (positron-emission tomography) uses an injected radioactive tracer and a scanner to show whether amyloid plaques or tau pathology are present in the brain.

Blood testing: plasma amyloid and phosphorylated tau

Modern blood tests generally measure a combination of amyloid-beta and phosphorylated tau (p-tau). One FDA-cleared example, the Lumipulse G pTau217/β-Amyloid 1-42 Plasma Ratio, combines p-tau217 with the amyloid-beta 1-42 measurement in a ratio. The FDA’s 2025 clearance covers adults aged 55 and older who have signs or symptoms of cognitive decline. It is an aid to diagnosis, not a population-screening test and not a stand-alone diagnostic test.

A tube of blood is collected by routine phlebotomy and sent for laboratory analysis. The laboratory’s result is a probability or classification generated by a particular assay and cutoff; it is not a direct count of plaques in the brain. Performance can change substantially with the assay, cutoff, patient population and comparator used, so the result from one platform cannot be generalized to every commercial “Alzheimer’s blood test.”

PET: imaging amyloid or tau in the brain

For an amyloid PET scan, a radiotracer is injected and allowed to distribute before images are acquired. An amyloid-positive scan indicates that the tracer detected a level of amyloid pathology considered abnormal for that test. Tau PET uses a different tracer and addresses tau distribution rather than amyloid burden. Use of PET is guided by appropriate-use criteria and generally requires a specialist center with the scanner, tracer supply and trained interpretation.

PET is an in-vivo biomarker reference because it images pathology in the organ where it occurs. However, a positive scan does not prove that Alzheimer’s disease is the only cause of a person’s memory or thinking symptoms, and it does not by itself determine disease stage or future rate of decline.

Are blood tests as accurate as PET scans?

There is no single accuracy number for “an Alzheimer’s blood test.” Accuracy claims must identify the exact assay, cutoff, study population and reference test. The 2025 Alzheimer’s Association clinical practice guideline sets practical thresholds for blood tests used in symptomatic patients:

  • Triage use: at least 90% sensitivity and 75% specificity.
  • Possible substitution for amyloid PET or cerebrospinal-fluid (CSF) testing in specialized memory care: at least 90% sensitivity and at least 90% specificity.

Sensitivity describes how often a test is positive when the target condition is present. Specificity describes how often it is negative when the target condition is absent. A test can be highly sensitive yet produce too many false positives for a high-stakes treatment decision, which is why the guideline uses separate thresholds for triage and substitution.

What the thresholds mean in practice

A blood test that reaches the triage threshold can help a clinician decide who should move forward to a more definitive biomarker evaluation and who may not need immediate PET or CSF testing. A test meeting both 90% thresholds may be used instead of PET or CSF in a specialized memory-care setting, provided the clinical team follows the guideline’s intended use and has validated the assay in the relevant population.

Many commercially marketed blood tests do not meet both thresholds at a single cutoff. Some report an intermediate zone rather than a simple positive or negative result. An intermediate result is not evidence that Alzheimer’s pathology is absent; it means the blood measurement alone is not sufficiently decisive and another biomarker may be needed.

Why PET is not a perfect “yes or no” answer

PET directly images amyloid or tau, but the scan still has to be interpreted in context. Amyloid pathology can be present in people who do not yet have dementia, and a positive amyloid scan does not identify every contributor to cognitive symptoms. A negative amyloid PET makes Alzheimer’s amyloid pathology less likely as the explanation for current symptoms, but clinicians may still investigate vascular disease, medication effects, sleep disorders, depression, other neurodegenerative diseases or reversible medical causes.

Cost, access and insurance

Exact prices are not universal for either test. They vary by location, laboratory or imaging facility, clinician fees, tracer availability, insurer and whether prior authorization is required. Ask for a written estimate that separates the test, professional interpretation and any visit or facility charges.

Factor Blood biomarker test Amyloid or tau PET
Collection Routine blood draw Radiotracer injection followed by imaging
Equipment Phlebotomy and a qualified laboratory PET scanner, radiotracer supply and specialist interpretation
Relative logistics Usually easier to schedule and access More resource-intensive; availability depends on specialist centers and tracer supply
Published universal consumer price Not established by the FDA clearance Not established; facility and regional charges differ
Radiation No ionizing radiation from the blood draw Yes; exposure comes from the injected radiotracer
Insurance coverage Depends on the assay, indication and plan Depends on policy, indication and plan

FDA clearance of a test does not set a national consumer price. Before paying for a blood test, confirm that the laboratory is offering the cleared or otherwise clinically validated assay your clinician ordered, ask whether the charge is billed to insurance, and request the cash price if it is not. For PET, verify that the imaging center is in-network, that the tracer and interpretation are included, and that authorization has been approved before the appointment.

The Centers for Medicare & Medicaid Services (CMS) retired its former 2013 national restriction on amyloid PET in October 2023. That change did not create automatic coverage for every scan. Medicare contractors, Medicare Advantage plans and commercial insurers can apply their own medical-necessity, documentation and authorization rules.

When should you use a blood test, PET scan or spinal tap?

The choice is usually a sequence rather than a competition between three interchangeable tests.

  1. Begin with a clinical assessment. A clinician takes a history, performs a cognitive examination, reviews medicines and evaluates potentially reversible contributors. Biomarkers are most useful when the symptoms and examination create a specific question about Alzheimer’s pathology.
  2. Use a validated blood test for triage when appropriate. For a symptomatic person in the intended age and clinical population, a blood test that has been validated for the laboratory’s stated cutoff can help determine whether Alzheimer’s biomarker testing is warranted and whether the result is strongly negative, strongly positive or indeterminate.
  3. Confirm when the decision requires greater certainty. If the blood result is indeterminate, conflicts with the clinical picture or does not meet the performance standards for substitution, the specialist may order amyloid PET or CSF testing. PET is also reasonable when direct brain imaging is important to the diagnostic question.
  4. Use an amyloid-confirming biomarker before certain treatment decisions. Disease-modifying therapy decisions may require evidence of amyloid pathology. The treating specialist determines whether a validated blood result is sufficient under current guidance and local protocol or whether PET or CSF confirmation is required.
  5. Investigate other causes regardless of the biomarker result. A positive biomarker does not eliminate the need to assess vascular, medication, psychiatric, sleep, metabolic or other neurologic explanations for symptoms.

When a blood test is a sensible first step

  • The person has symptoms or signs that warrant an Alzheimer’s biomarker assessment.
  • The assay is validated for the person’s age, clinical setting and intended use.
  • Access to PET or CSF is limited, delayed or burdensome.
  • The clinician is using the result for triage rather than treating it as an autonomous diagnosis.

When PET or CSF is more appropriate

  • The blood result falls in an indeterminate range.
  • The result conflicts with the examination, history or another test.
  • The blood assay does not meet the guideline thresholds required for the intended decision.
  • A treatment or research protocol requires a specified amyloid confirmation method.
  • The specialist needs information that a plasma ratio cannot provide, such as direct visualization of amyloid or tau pathology.

Can a blood test diagnose Alzheimer’s or only show risk?

A blood test can provide evidence of Alzheimer’s-related biology, but it is not equivalent to diagnosing dementia from a laboratory number alone. The FDA labeling for the 2025-cleared plasma ratio explicitly says it is intended for people aged 55 and older with signs or symptoms of cognitive decline and is not intended as a stand-alone diagnostic test.

That distinction matters because Alzheimer’s biology and clinical disease are not identical. A person can have biomarker evidence without substantial day-to-day impairment, while someone with cognitive symptoms may have a different or additional cause. Diagnosis requires integrating the biomarker with functional history, cognitive findings and the evaluation for other conditions.

Consumer-facing “risk” language can also be misleading. A result may indicate a higher or lower likelihood that the tested pathology is present at the assay’s cutoff; it does not reliably predict an individual’s exact course, when symptoms will progress or whether a particular treatment will work.

Limitations and risks to discuss before testing

Blood-test limitations

  • False positives and false negatives: No assay is perfect, and results near a cutoff are especially difficult to classify.
  • Population limits: Validation in one age range, ancestry group, disease stage or specialty clinic does not automatically establish the same performance elsewhere.
  • Assay variation: Different platforms measure different analytes or use different calibration and cutoff rules.
  • Clinical context still required: FDA clearance does not remove the need for examination, history and differential diagnosis.

PET limitations and risks

  • Radiation and injection: PET involves an injected radiotracer and ionizing-radiation exposure. The imaging center should explain the expected exposure and any relevant precautions.
  • Availability: Scheduling depends on scanner capacity, tracer delivery and specialist interpretation.
  • Pathology is not the whole diagnosis: A positive scan confirms the targeted pathology but does not explain every symptom or establish stage by itself.
  • Incidental findings: Imaging can reveal unexpected findings that may require separate evaluation.

CSF (spinal-tap) testing

CSF testing is another established biomarker route. It may be considered when a blood result is inconclusive or when the clinical decision requires a validated CSF profile. It is more invasive than a blood draw, so the clinician weighs its additional information against the procedure’s burden and the availability of PET or a sufficiently accurate plasma assay.

How to prepare for a biomarker appointment

  1. Bring a symptom timeline. Note when memory, language, judgment or navigation changes began and how they affect daily activities.
  2. List every medicine and supplement. Include nonprescription sleep aids, antihistamines and other products that can affect cognition.
  3. Ask which assay and cutoff will be used. Request the laboratory’s intended population, validation data and how it reports positive, negative and intermediate results.
  4. Clarify the clinical decision. Ask whether the result is for triage, diagnostic clarification or treatment eligibility; the required certainty may differ.
  5. Check logistics and coverage. Confirm fasting or other preparation instructions, turnaround expectations, prior authorization and the total out-of-pocket estimate.
  6. Plan interpretation with the ordering clinician. Do not make medication, driving, employment or care-planning decisions from an unreviewed result.

How to read a result without overinterpreting it

A report should identify the analytes, measurement method, cutoff or reference range and the population for which the interpretation was validated. “Positive” means the measured pattern crossed that assay’s threshold; it does not mean the test has identified every cause of the person’s symptoms. “Negative” lowers the likelihood of the targeted Alzheimer’s pathology but does not rule out other neurologic disease or guarantee normal cognition. “Indeterminate,” “borderline” or a similar category means the result should not be forced into a yes-or-no conclusion.

Ask the clinician what would change if the result were wrong. If a false positive could expose someone to an unnecessary treatment or a false negative could delay an important decision, confirmatory PET or CSF testing may be more appropriate than relying on the plasma result alone.

A practical decision table

Clinical situation Usually considered Reason
Symptoms warrant evaluation; access to specialty imaging is limited Validated blood biomarker for triage Convenient first estimate of Alzheimer’s pathology likelihood
Blood result is intermediate or does not meet substitution thresholds Amyloid PET or CSF Higher-certainty confirmation is needed
Blood result conflicts with symptoms or examination Specialist review plus PET or CSF as indicated Discordance may reflect assay limits or another condition
Considering a disease-modifying treatment Validated amyloid confirmation accepted by the treatment protocol Eligibility may require documented amyloid pathology
No cognitive symptoms and a request for general screening Clinical discussion rather than an automatic biomarker test The FDA-cleared use described here is for adults 55 and older with signs or symptoms, not stand-alone population screening

What a careful answer looks like

Blood tests can be a convenient first biomarker check; PET is a specialized imaging option when the answer must be especially certain. A blood result is most useful when the assay, cutoff and intended population are known and a clinician is prepared to act on an uncertain result. PET adds direct evidence of brain pathology but brings injection, radiation, scheduling demands and variable coverage. The right choice depends on the clinical question, the test’s validated performance and what decision will follow—not on the word “positive” or “negative” alone.
