There is no single substitute for testosterone that treats every menopause symptom. The best alternative depends on what is lowering sexual desire: vaginal dryness or pain, hot flashes and poor sleep, depression or anxiety, medication effects, relationship factors, or a separate medical condition. Vaginal oestrogen or non-hormonal moisturisers can relieve painful sex; menopause-specific cognitive behavioural therapy (CBT) can help symptom distress and sleep; hormone therapy (HRT/MHT) is the most effective treatment for vasomotor symptoms when appropriate; and prescription medicines such as flibanserin, prasterone, ospemifene or fezolinetant may fit particular situations. A clinician should assess the cause before prescribing, especially when HRT is contraindicated or the choice is complex.

Start with the reason desire has changed

“Low libido” is a symptom, not a diagnosis. Desire commonly falls because sex has become uncomfortable, because night sweats and insomnia leave little energy, or because mood, medicines, health conditions and relationship circumstances have changed. A product aimed only at sexual desire will not correct pain, severe sleep loss or an untreated illness.

Questions that change the treatment

  • Is there dryness, burning, tearing or pain with penetration?
  • Did hot flashes, night sweats or insomnia begin at the same time?
  • Are depression, anxiety, stress or relationship difficulties affecting interest?
  • Could an antidepressant, blood-pressure medicine, hormonal contraceptive or another drug be contributing?
  • Are there symptoms of thyroid disease, anaemia, diabetes or another condition?
  • Is there postmenopausal bleeding, a history of breast or endometrial cancer, clotting, stroke or liver disease?

NICE recommends an individualized assessment and menopause-specialist input when HRT is contraindicated or the best option is uncertain. Persistent or unexplained bleeding after menopause needs medical assessment before starting or continuing treatment.

Compare the main alternatives

Option Main target Hormonal exposure Key cautions or monitoring How it is obtained
Vaginal oestrogen Dryness, burning and painful sex from genitourinary syndrome of menopause Local treatment; systemic absorption is minimal compared with systemic HRT Review unexplained bleeding and cancer history with a clinician; serious adverse effects are very rare Prescription
Moisturiser or lubricant Regular tissue hydration or friction reduction during sex Non-hormonal Choose products that do not irritate; comfort improvement does not treat every cause of low desire Self-care or pharmacy product
Vaginal prasterone (DHEA) Dryness and pain when local oestrogen or non-hormonal products fail or are not tolerated Locally administered hormone precursor Not considered cost-effective as a first-line option in NICE’s rationale; clinician review is needed Prescription
Ospemifene Moderate-to-severe dyspareunia or vaginal dryness when local treatment is impractical Oral selective oestrogen-receptor modulator Boxed warnings for endometrial cancer and cardiovascular disorders; contraindicated with undiagnosed abnormal genital bleeding or active/prior thromboembolism Prescription
HRT/MHT Hot flashes, night sweats and other systemic symptoms; may indirectly improve desire Systemic oestrogen, with progestogen when a uterus is present Benefit-risk assessment depends on age, timing, bleeding, cancer, clot and liver history Prescription
Menopause-specific CBT Distress, sleep problems, mood symptoms and coping with hot flashes; can support sexual wellbeing Non-drug Requires a trained practitioner and active participation Clinician referral or self-management programme
Fezolinetant Moderate-to-severe hot flashes when a non-hormonal prescription is wanted Non-hormonal neurokinin-3 receptor antagonist Liver tests before treatment, monthly for the first three months, then at months 6 and 9; stop and seek care for liver-injury symptoms Prescription
Flibanserin (Addyi) Acquired, generalized hypoactive sexual desire disorder (HSDD) causing distress or interpersonal difficulty Non-hormonal central nervous-system medicine 100 mg at bedtime; boxed warning for hypotension and syncope, especially with alcohol, moderate/strong CYP3A4 inhibitors or hepatic impairment; stop after eight weeks without improvement Prescription

If dryness or painful sex is suppressing desire

Vaginal oestrogen is the usual first-line medical option

NICE recommends offering vaginal oestrogen for menopausal genitourinary symptoms, including for people already using systemic HRT. It is available as a cream, gel, tablet, pessary or ring. Because absorption into the bloodstream is minimal compared with systemic HRT, it targets vaginal and vulval tissues rather than treating hot flashes. Serious adverse effects are very rare, but a clinician should review any unexplained bleeding and discuss treatment in the context of a personal history of breast or endometrial cancer.

Improvement is usually judged by comfort: less dryness, burning and pain with sex. Desire may return when sex is comfortable, but vaginal oestrogen is not a universal libido medicine.

Use moisturisers regularly and lubricants during sexual activity

A vaginal moisturiser is applied on a regular schedule to help hydrate tissue between sexual encounters. A lubricant is used immediately before or during sexual activity to reduce friction. Water-based products are a practical starting point; stop any product that causes burning or irritation. These options are appropriate when oestrogen is contraindicated or not preferred, and they can be used while a clinician evaluates other causes of low desire.

When prasterone may be considered

Vaginal prasterone (DHEA) can be considered when vaginal oestrogen or non-hormonal moisturisers and lubricants have been ineffective or are not tolerated. NICE’s evidence review found improvement in dryness and pain with sex, but its rationale did not consider prasterone cost-effective as a first-line treatment. It is a prescription option, not an over-the-counter libido supplement.

When an oral option is needed: ospemifene

Ospemifene is an oral alternative when locally applied treatment is impractical. The FDA label revised in February 2025 specifies 60 mg once daily with food for moderate-to-severe dyspareunia or vaginal dryness caused by menopausal vulvar and vaginal atrophy. The label carries boxed warnings about endometrial cancer and cardiovascular disorders. It is contraindicated with undiagnosed abnormal genital bleeding and with active or prior thromboembolism. A prescriber must check bleeding history, clot risk, cardiovascular risk and other medicines before use.

Do not substitute vaginal laser outside a trial

NICE advises not offering vaginal laser treatment for menopausal genitourinary symptoms unless it is delivered as part of a randomized controlled trial. Marketing claims about permanent vaginal “rejuvenation” should not replace treatments with established guidance and safety information.

If hot flashes and sleep loss are driving the problem

HRT/MHT remains the most effective systemic treatment when suitable

NICE recommends offering HRT for vasomotor symptoms and selecting dose, formulation and route through an individualized benefit-risk discussion. Someone with a uterus generally needs combined oestrogen and progestogen to protect the endometrium. After a total hysterectomy, oestrogen-only HRT is generally used. HRT may improve energy, sleep and comfort indirectly, but it is not prescribed solely as a general-purpose libido booster.

Menopause-specific CBT

NICE recommends considering menopause-specific CBT alongside HRT, or when HRT is contraindicated or declined. Therapy can address the cycle of hot-flash worry, insomnia, fatigue, anxiety and reduced interest in sex. It may also be useful for mood symptoms and the emotional impact of painful sex. CBT does not require hormone exposure and can be combined with local vaginal treatment or a non-hormonal hot-flash medicine.

Fezolinetant for moderate-to-severe hot flashes

Fezolinetant is a non-hormonal prescription for moderate-to-severe menopausal hot flashes. Liver safety is central to treatment: the FDA recommends liver tests before starting, monthly during the first three months, and again at months 6 and 9. Stop the medicine and seek medical care for possible liver-injury symptoms such as yellowing of the skin or eyes, dark urine, unusually pale stools, severe nausea or vomiting, unusual itching, or pain under the right ribs. It treats vasomotor symptoms, not vaginal dryness or HSDD.

If low desire remains after pain and vasomotor symptoms are addressed

Psychosexual, relationship and medical care come first

Assessment should include mood, anxiety, trauma, relationship context, sleep, medication effects and medical conditions. Counseling, sex therapy, couples therapy or CBT may be appropriate. Reviewing a suspected medicine with the prescribing clinician is safer than stopping it abruptly. A physical examination and laboratory testing are guided by symptoms rather than ordered automatically for every person.

Flibanserin (Addyi) has a narrow indication

The FDA label revised in December 2025 indicates Addyi for women younger than 65 with acquired, generalized HSDD that causes distress or interpersonal difficulty and is not due to another medical or psychiatric condition, relationship problems or a medication. The dose is 100 mg at bedtime. Treatment should be stopped after eight weeks if there is no improvement.

The boxed warning concerns hypotension and syncope. Risk is particularly important with alcohol, moderate or strong CYP3A4 inhibitors and hepatic impairment. The prescriber and pharmacist need a complete list of medicines, supplements and alcohol use before treatment. Flibanserin is not indicated for low desire caused by pain, hot flashes, relationship conflict or another identifiable condition.

How testosterone fits into the decision

NICE says to consider testosterone supplementation for people with low sexual desire associated with menopause if HRT alone is not effective. This is a specialist, indication-limited option. Testosterone is not a general treatment for hot flashes, mood symptoms, vaginal pain or every form of menopause-related low libido. If discussing alternatives, clarify whether the goal is to avoid testosterone entirely, avoid systemic hormones, or treat a symptom that testosterone would not be expected to fix.

A practical decision pathway

  1. Record the pattern. Note when desire changed, whether pain or dryness is present, how often hot flashes occur, sleep quality, mood, medicines and the effect on personal wellbeing or a relationship.
  2. Check for red flags. Arrange medical assessment for postmenopausal bleeding, new pelvic pain, breast or genital symptoms, severe depression, symptoms of liver disease or a sudden unexplained change in sexual function.
  3. Treat the dominant symptom. Use vaginal oestrogen or non-hormonal comfort products for genitourinary symptoms; discuss HRT, CBT or fezolinetant for vasomotor symptoms; and address mood, sleep, medicines and relationship factors directly.
  4. Review response. Agree on a follow-up interval and define success, such as pain-free sex, fewer night sweats, improved sleep or restored interest. Lack of improvement should trigger reassessment rather than automatic addition of another product.
  5. Escalate selectively. Consider prasterone, ospemifene or flibanserin only when the indication fits and contraindications, interactions and monitoring have been reviewed. Seek menopause-specialist input when HRT is unsuitable or several symptoms overlap.

Safety checklist before choosing a prescription

  • Age and years since the final menstrual period.
  • Whether the uterus is present and whether bleeding is fully explained.
  • Personal or strong family history of breast or endometrial cancer.
  • Previous blood clot, stroke, heart disease or uncontrolled cardiovascular risk.
  • Liver disease and current liver-test results when relevant.
  • All prescription medicines, over-the-counter products, supplements and alcohol intake.
  • Pregnancy possibility where relevant and the need for contraception.
  • What monitoring is required: bleeding review, blood pressure, interaction checks or scheduled liver tests.

What to avoid or treat cautiously

Unregulated “bioidentical” or herbal hormone products

NICE says the efficacy and safety of unregulated hormone preparations are unknown. Constituents, quality and purity can vary, and interactions with medicines have been reported. “Natural” does not establish a safe dose, reliable hormone content or suitability after cancer or clotting problems.

Single-product promises

A supplement or prescription aimed at desire cannot be expected to resolve pain, insomnia, depression, medication effects or relationship distress. Be cautious of products that promise to balance every menopause symptom without a diagnosis, ingredient disclosure or follow-up plan.

When to seek prompt medical help

  • Any new, heavy or unexplained bleeding after menopause.
  • Chest pain, sudden shortness of breath, one-sided leg swelling or neurological symptoms.
  • Fainting, severe dizziness or repeated falls after starting a medicine.
  • Yellow skin or eyes, dark urine, pale stools or severe right-upper-abdominal pain while taking fezolinetant.
  • Severe depression, suicidal thoughts or loss of safety at home.
  • New breast, vulval or vaginal lumps, sores or persistent discharge.

What the evidence says about use and access

Menopause symptoms affect a large potential treatment population, but prescribing is much less common: an FDA announcement in 2026 summarized 2020 U.S. data as about 41 million women aged 45–64, while about 2 million women aged 46–65 received a hormone-therapy prescription that year. That contrast does not show that one treatment is right for everyone; it underscores why symptom-specific assessment matters.

For fezolinetant, FDA-described Veozah Trial 1 and Trial 2 randomized 1,028 postmenopausal participants (1,022 received treatment), and Trial 3 randomized 1,831 participants. Trial participation demonstrates how the medicine was studied; it does not guarantee the same benefit or safety profile for an individual.

Bottom line for choosing an alternative

Match the treatment to the symptom: local vaginal therapy or moisturiser and lubricant for dryness and painful sex; HRT, CBT or fezolinetant for hot flashes and sleep disruption; and psychosexual or medical care, with narrowly indicated flibanserin in eligible women, when low desire persists without another driver. Prasterone and ospemifene are specific options for genitourinary symptoms, each requiring clinician review. Do not use unregulated hormones or vaginal laser outside a randomized trial, and involve a menopause specialist when HRT is contraindicated, symptoms overlap or safety risks are substantial.
