For a new episode of mild depression (called less severe depression in NICE guidance), antidepressants are not usually the first treatment offered. NICE recommends discussing options and matching care to your clinical needs and preferences; an antidepressant may still be appropriate when it is your informed preference. Effective non-drug choices include guided self-help, cognitive behavioural therapy (CBT), behavioural activation, and, when those formats do not suit you, group mindfulness, structured exercise, or interpersonal psychotherapy.
“Mild” describes symptom severity, not whether the situation is safe or deserves care. Arrange an assessment before choosing a treatment so a clinician can check the diagnosis, suicide risk, bipolar symptoms, substance use, medical contributors, pregnancy considerations, and medicines. If you already take an antidepressant, do not stop or taper it without the prescriber’s plan.
What mild depression means—and why assessment still matters
NICE uses the term less severe depression for what many people call mild depression. Symptoms can include persistent low mood, loss of interest, reduced energy, sleep or appetite changes, slowed thinking, guilt, and difficulty functioning. The label does not predict exactly how you will feel from day to day, and it does not rule out suicidal thoughts or rapid deterioration.

This assessment is not a requirement to take medication. It is what makes a non-drug plan safe, appropriately targeted, and easier to adjust if symptoms persist.
How to choose an alternative to antidepressants
NICE recommends shared decision-making for a new episode: discuss the available treatments, then select the option that fits both clinical need and personal preference. Compare choices on more than whether they involve a prescription.
| Decision factor | Questions to ask |
|---|---|
| Practitioner contact | Would you prefer self-guided work with brief support, a group, or one-to-one sessions? |
| Structure | Can you commit to regular sessions, exercises, or planned activities, or do you need a more flexible approach? |
| Access | Is the service available in person, by telephone, or online, and can you attend reliably? |
| Preference and comfort | Would discussing thoughts, focusing on activity, working with relationships, or practising meditation feel acceptable? |
| Physical health | Do pain, mobility limits, cardiovascular concerns, fatigue, or another condition require a modified plan? |
| Time and resources | What session schedule, travel, privacy, and cost can you realistically sustain? |
| Monitoring | How will you and the practitioner measure change and decide whether to continue, adapt, or step up care? |
Guided self-help is generally the least resource-intensive option. Group and individual therapies involve more practitioner contact. A lower-intensity treatment is not “doing nothing”; it is a structured intervention with a defined review plan.

Guided self-help combines a printed workbook or digital course with regular support from a trained practitioner. NICE describes a typical course as six to eight structured sessions. Materials may teach CBT, behavioural activation, problem-solving, or psychoeducation. The support is usually briefer than full therapy, but it is not simply being handed a book and left alone.
What the methods teach
- CBT-based self-help: identify links among situations, thoughts, feelings, beliefs, and behaviour; test unhelpful predictions; and practise alternative responses.
- Behavioural-activation self-help: map how avoidance and activity patterns affect mood, then schedule manageable, meaningful, or rewarding activities.
- Problem-solving: define a specific problem, generate options, choose a workable step, and review the result.
- Psychoeducation: learn how depression can affect sleep, motivation, concentration, and behaviour so that symptoms are less confusing and easier to plan around.
How to use it effectively
- Choose a structured program. Look for a clearly described CBT, behavioural-activation, problem-solving, or psychoeducational course rather than unstructured motivational material.
- Set the support schedule. Agree how often you will speak with the trained practitioner, what channel will be used, and what to do if symptoms worsen.
- Work between sessions. Reserve a small, predictable block of time for reading, worksheets, activity planning, or experiments. Completing every exercise perfectly is not the goal; consistent practice is.
- Bring obstacles to review. Tell the practitioner when fatigue, shame, concentration problems, caregiving, or an unsafe home situation prevents an assignment. The plan can be simplified or adapted.
- Review progress at the end of the course. Decide whether symptoms and functioning have improved enough to continue independently, whether another therapy is preferable, or whether a higher level of care is needed.
Guided self-help suits people who want privacy and flexibility and can complete regular tasks. It may be a poor fit when concentration is severely impaired, risk is high, symptoms are rapidly worsening, or there is no way to obtain support and review.

Cognitive behavioural therapy (CBT)
CBT is a goal-oriented, structured therapy. It examines patterns linking situations, interpretations, emotions, bodily responses, and actions. Sessions commonly include a shared agenda, practice in the session, and homework between sessions. The aim is to build skills you can continue using after therapy ends, not to prove that your feelings are irrational.
Group CBT
NICE identifies group CBT as a first-line option for less severe depression. A group can provide a planned curriculum, repeated skills practice, and the normalising experience of meeting others with similar difficulties. Ask about group size, confidentiality rules, accessibility, and whether sessions are in person or remote. Group work is not suitable for everyone; discomfort speaking in front of others, scheduling constraints, or a need for highly individualised attention may make one-to-one care preferable.

Individual CBT
Individual CBT is useful when a group is unavailable, unsuitable, or not your preference. You and the therapist can tailor examples and homework to work, relationships, health conditions, or specific avoidance patterns. More individual attention does not guarantee faster improvement; attendance, therapeutic fit, and practising skills between sessions matter.
Questions to ask a CBT provider
- What training and supervision do you have for depression?
- How many sessions are usually planned, and how are goals measured?
- What happens if homework is too difficult or symptoms worsen?
- Can sessions be adapted for disability, neurodivergence, language, faith, or cultural needs?
Behavioural activation (BA)
Behavioural activation focuses on the relationship between activity and mood. Depression often narrows a person’s routine: enjoyable or meaningful activities drop away, avoidance brings short-term relief, and low activity then reinforces low mood. BA makes that cycle visible and uses gradual, planned action to interrupt it.
What a BA plan contains
- Map the current pattern. Record activities, mood, energy, avoidance, and what happens immediately afterward.
- Identify values and functions. Choose areas that matter—such as connection, learning, health, care of a home, or creativity—rather than chasing a perfect mood.
- Break tasks down. Convert “exercise” into a ten-minute walk, “see friends” into a text or short call, or “clean the house” into one surface.
- Schedule actions. Put selected activities on a calendar with a time, place, backup plan, and expected difficulty.
- Review and adjust. Notice what was completed, what barriers appeared, and whether the activity brought pleasure, mastery, connection, or another useful outcome.
BA can be delivered individually, in a group, or through guided self-help. It does not require waiting to feel motivated; motivation often follows small, repeated actions. The plan should still respect pain, fatigue, disability, caring duties, and financial or transport limits.

Mindfulness-based cognitive therapy and meditation groups
NICE lists group mindfulness or meditation as an alternative when CBT or behavioural activation is not acceptable or suitable. Structured programs typically teach attention to present-moment experience, noticing thoughts without automatically acting on them, and responding to difficult feelings with less reactivity. A trained facilitator and regular sessions matter; downloading an app or meditating occasionally is not the same intervention evaluated in clinical guidance.
Mindfulness can be uncomfortable for people with trauma symptoms, panic, dissociation, or severe agitation. Tell the facilitator about these concerns and ask how practices can be shortened, adapted, or replaced. Meditation should complement, not delay, assessment when risk or functional decline is significant.
Interpersonal psychotherapy (IPT)
Interpersonal psychotherapy is another psychological option when CBT or BA are not acceptable or suitable. IPT concentrates on current interpersonal problems that can maintain or trigger depression, such as grief, role transitions, conflict, or persistent isolation. Sessions help you identify patterns, communicate needs, and build support while addressing depressive symptoms.
IPT may fit someone who sees a clear connection between mood and relationship changes or who prefers a therapy centred on current life circumstances rather than thought records or activity schedules. Discuss the therapist’s training, session format, and how progress will be monitored through shared decision-making.
Structured exercise as a treatment option
Regular physical activity supports general wellbeing, but NICE also describes a specific depression treatment: a practitioner-led group program using moderate-intensity aerobic activity, usually more than once a week for about 10 weeks. This is more structured than a general instruction to “exercise more.”

Planning a safe, realistic program
- Choose an activity you can repeat. Walking, cycling, swimming, or another aerobic activity can work if intensity is moderate and the format is accessible.
- Start from current capacity. A clinician or exercise professional can adapt duration, pace, rest periods, and supervision for pain, disability, cardiovascular disease, or deconditioning.
- Address emotional barriers. Anxiety about groups, body shame, low confidence, and fear of being judged can be discussed before attendance. A smaller group, quieter time, or supported introduction may help.
- Plan for logistics. Consider travel, changing facilities, weather, childcare, equipment, and what you will do when energy is low.
- Track more than mood. Record attendance, energy, sleep, social contact, and functioning. Mood may change gradually even when a single session does not feel rewarding.
Do not treat exercise as a test of willpower or as a substitute for medical care. Chest pain, fainting, severe breathlessness, or another acute symptom requires medical attention rather than pushing through a session.
Lifestyle measures that support recovery
NICE advises regular physical activity and a healthy lifestyle as wellbeing measures alongside treatment. These habits can make therapy easier to use, but they are not a replacement for assessment or a promise that depression will resolve through discipline.
- Sleep routine: keep wake time reasonably consistent, reduce long daytime naps, and discuss persistent insomnia or possible sleep apnoea with a clinician.
- Food and hydration: aim for regular meals and enough fluids. Depression can disrupt appetite, so practical options may be more useful than an idealised diet.
- Alcohol and drugs: reducing use can improve sleep, mood, and safety; seek support if stopping suddenly could cause withdrawal.
- Connection: schedule low-pressure contact with a trusted person, support group, or community service instead of waiting until you feel sociable.
- Daily structure: use a short list of essential tasks, rest, and one meaningful activity. Overloading the day can reinforce a sense of failure.
These steps work best when they are specific and small enough to repeat. A therapist or clinician can help convert broad intentions into a plan that accounts for symptoms and circumstances.
Light therapy, yoga, and supplements: what the evidence supports
Light therapy deserves a careful distinction. NICE says evidence is uncertain for light therapy in winter-pattern depression. The American Psychiatric Association guideline lists bright-light therapy and yoga as possible options in selected circumstances, but that does not establish them as universal substitutes for psychological treatment.
Supplements are not automatically safer or equivalent to established care. The APA reports insufficient evidence for several complementary treatments, including omega-3 used alone and S-adenosyl-L-methionine (SAMe) used alone. Products can vary in dose and purity, interact with medicines, affect bleeding or blood pressure, and be inappropriate in pregnancy or bipolar disorder. Discuss any supplement with a clinician or pharmacist, including the exact product and dose, and do not use it to postpone assessment.

How to monitor whether a plan is working
Agree in advance what improvement would look like. Useful measures include mood, interest, sleep, appetite, concentration, work or study attendance, self-care, social contact, and ability to complete planned activities. A simple weekly record is often enough; a clinician may also use a standard symptom questionnaire.
Follow-up timing
If you decline treatment or symptoms are improving, NICE recommends providing information, a way to seek help, and reassessment normally within two to four weeks. The interval is not a guarantee that waiting is safe in every situation: contact the service sooner if symptoms intensify, functioning drops, side effects appear, or suicidal thoughts emerge.
When to change course
- Some improvement: continue the agreed plan, reinforce the activities or skills that helped, and set the next review.
- No meaningful change: check attendance, barriers, diagnosis, medical factors, and therapeutic fit; consider switching from guided self-help to CBT, BA, IPT, or another suitable option.
- Worsening symptoms or increased risk: contact the clinician urgently for reassessment and a higher level of support.
- Possible bipolar symptoms, psychosis, or severe impairment: seek specialist assessment rather than treating the episode as uncomplicated mild depression.
If you already take an antidepressant
Do not stop, skip, or taper a prescribed antidepressant on your own. Sudden changes can cause withdrawal symptoms or a return of depression, and the safest taper depends on the medicine, dose, duration, other conditions, and previous responses. Tell the prescriber that you want to consider psychological treatment, guided self-help, exercise, or another option. Together you can decide whether to continue medication, add therapy, or taper gradually with monitoring.
A practical decision path
- Book an assessment. Confirm the diagnosis and review safety, bipolar symptoms, substances, medical issues, pregnancy considerations, and current medicines.
- State your preference. Explain whether you want no medication, a time-limited trial of a non-drug option, or a combined approach.
- Match the format. Choose guided self-help for a lower-intensity supported plan; CBT or BA for structured skills work; IPT for relationship-focused concerns; mindfulness for a facilitated meditation format; or structured exercise when moderate aerobic activity is feasible and acceptable.
- Write down the plan. Include session dates, between-session tasks, adaptations for health or access, the contact point for concerns, and the review date.
- Review at two to four weeks when appropriate. Use your agreed measures, report deterioration promptly, and change treatment if the response or fit is inadequate.
The best alternative is the one you can access, understand, and continue safely with professional review. Choosing a non-drug treatment does not mean managing depression alone, and choosing medication later is not a failure of the original plan.
When to seek urgent help
Seek urgent help immediately if you may act on thoughts of suicide, cannot keep yourself safe, are hearing or seeing things others do not, are unable to care for basic needs, or have rapidly escalating agitation or sleeplessness. Contact local emergency services or a crisis service, tell a trusted person, and do not stay alone while waiting for help. If risk is not immediate but thoughts of self-harm are increasing, contact your clinician promptly rather than waiting for the scheduled review.
