AI is already changing insurance claims, but it has not removed human judgment. Software now extracts facts from forms, emails, images and scans; machine-learning models prioritize severity and fraud risk; and generative AI summarizes files and proposes next actions. Straightforward claims can move through largely automated paths, while bodily-injury, disputed-liability, vulnerable-customer and unusual cases should remain with accountable claims professionals.
The practical shift is therefore less “robot adjusters” than a layered operating model: machines handle repetitive evidence work early, route complexity to the right specialist and keep a traceable record of how recommendations were made. That can shorten cycle times and expose fraud sooner, provided insurers control false positives, privacy, cybersecurity and unfair outcomes.
How is AI used in insurance claims processing?
AI is used at each handoff from first notice of loss to settlement. The most reliable deployments combine document extraction, predictive triage, anomaly detection and generative-AI assistance instead of asking one model to make every decision.
First notice of loss and intake
Optical-character-recognition and language models extract names, dates, policy numbers, incident descriptions, invoices and other fields from web forms, emails, PDFs, photographs, scans and handwritten documents. Rules then check whether required evidence is present, whether the policy was active on the loss date and whether the reported event matches the covered peril. A structured claim record can be created before an adjuster opens the file.

Triage and routing
Predictive models classify likely severity, coverage complexity and the next best queue. A low-value windshield loss with complete evidence may enter straight-through processing; a bodily-injury claim, a disputed liability case or a customer showing signs of vulnerability should be routed to a specialist. Triage is a prioritization decision, not a finding that a claimant is dishonest or that coverage is denied.
Assessment and decision support
Generative AI can summarize a long claim file, retrieve relevant policy clauses, flag missing evidence and draft questions for an adjuster. The safer pattern is retrieval from approved policy and claims sources, confidence thresholds and a human sign-off for consequential actions. An automatically generated summary is an aid, not proof of liability.
Settlement, recovery and communication
Once an authorized decision is made, automation can prepare payment instructions, status messages, repair referrals and subrogation leads. Systems can also identify a possible recovery from another insurer or a third party. Every payment rule and exception should remain deterministic and auditable even when a language model prepared the supporting explanation.

It can approve a narrowly defined, low-risk claim when an insurer has authorized straight-through processing, but most consequential claims still require a person who can explain, override and own the outcome. Adoption statistics show why “AI approves everything” is inaccurate.
EIOPA reported in 2024 that 50% of non-life insurers and 24% of life insurers were using AI somewhere in the insurance value chain, including claims management and fraud detection. A 2024 FCA and Bank of England survey found AI in use at 75% of UK financial-services firms; 55% of use cases involved some automated decision-making, but only 2% were fully autonomous. Those figures describe use cases, not a guarantee that a particular claim will be handled without a human.

Insurers generally set an authority matrix. A model may auto-settle a claim below a value threshold when coverage, identity and evidence checks all pass. A human review is normally mandatory for bodily injury, death, vulnerable customers, disputed liability, denials, suspected organized fraud, litigation and any low-confidence or out-of-distribution file. Claimants should be told how to request a human review and how to challenge an adverse decision.
How does AI detect insurance fraud?
Fraud analytics looks for combinations of signals that are difficult to spot manually: repeated addresses or phone numbers, unusual timing, inconsistent injury descriptions, inflated invoices, claim clusters around the same provider and links between people, vehicles, policies or repair shops. A model produces a risk score or network connection for an investigator; it should not be treated as a finding of fraud by itself.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteThe economic incentive is substantial. The Association of British Insurers said insurers identified £1.16 billion in fraudulent UK general-insurance claims and detected more than 98,400 fraud-related claims in 2024. Aviva reported stopping more than 12,700 suspect claims worth £127 million in 2024. These are detected or prevented amounts, not an estimate of all fraud, and Aviva’s figure is a company-reported result.
The National Insurance Crime Bureau’s 2025 annual report describes three complementary uses: an early workers’ compensation model, a questionable-claims network model and a large-language model that helps investigators search and summarize case information. The report says 208,956 questionable claim submissions were reviewed. Such systems surface relationships and prioritize scarce investigative time; they do not eliminate the need for corroborating documents, interviews or due process.
What do real insurance AI deployments show?
Published case studies point to the greatest gains when an insurer narrows the problem, redesigns the surrounding workflow and keeps a human escalation path.

| Deployment | Scope and reported outcome | What the example demonstrates |
|---|---|---|
| Direct Pojišťovna | Windshield claims; handling fell from 15 minutes to about 2 minutes, and 60% were fully automated. | Start with a bounded decision space, validate variable documents and retain human-in-the-loop review. |
| Aviva case study reported by McKinsey | More than 80 models; 23 fewer days to assess complex liability; 30% better routing accuracy; 65% fewer complaints; staff trained for more than 40,000 hours. | Operating-model and workforce changes matter as much as model deployment. Results are case-study figures, not universal benchmarks. |
| Swiss Re ClaimsGenAI | Live since mid-2024; screens more than 40,000 corporate claims annually and provides extracted, validated facts, summaries and recovery or irregularity signals. | Generative AI can assist complex commercial claims while claims experts remain responsible for decisions. |
| Shift Technology customer story published by Microsoft | Cloud tools analyzed more than 2.6 billion policies, claims and supporting documents. | Large-scale document triage is technically possible, but the reported volume does not by itself establish accuracy or savings. |
Direct Pojišťovna used Azure AI Foundry and Document Intelligence to read forms, photos, reports, scans, emails and legal documents. Its product owner, Jakub Lada, reported that per-claim handling fell from 15 minutes to about 2; information-technology head Pavel Lukeš described human-in-the-loop as essential. The transferable lesson is to instrument accuracy on a constrained line before extending reusable components to property, liability or pet claims.

McKinsey’s Aviva case describes a “double helix” in which digital and human paths can switch as new evidence appears. Personal-injury claims default to human interaction. The reported 23-day reduction, 30% routing improvement and 65% complaint reduction should be read as results of that combined redesign, training and governance rather than as a promise from any individual model.
Swiss Re says ClaimsGenAI ingests, classifies and screens unstructured corporate-claims data. Managers receive automatically extracted and validated information, concise summaries and faster access to relevant insights, while claims experts remain in control. This is an assistive pattern for high-complexity work, not unattended settlement.
Why is my insurance claim still taking so long?
AI can remove reading and routing delays, but it cannot create missing evidence, resolve a disputed account of events or make a coverage question legally simple. Delays commonly arise from incomplete forms, medical or repair records that have not arrived, multiple liable parties, catastrophe backlogs, manual legacy-system handoffs and a need to interview witnesses.
Ask the insurer which evidence is outstanding, whether the file has been assigned to a specialist and what decision or payment deadline applies in your jurisdiction. If an automated message appears inconsistent with your documents, request the underlying reason, provide corrections and ask for human escalation. A fraud-risk flag may lengthen review without meaning that the claim has been rejected.

Is AI fair and explainable in insurance decisions?
Fairness is not automatic. A model can reproduce historic disparities through proxy variables such as location, language, repair provider or communication channel, even when protected characteristics are omitted. A fast decision is not acceptable if similarly situated customers receive different outcomes or cannot understand how to appeal.
Controls insurers should require
- Measure errors by segment: track precision, recall, calibration, payment leakage and appeal outcomes by claim type and, where lawful, protected characteristic.
- Set mandatory human-review triggers: include bodily injury, vulnerability, disputed liability, denials, unusual claims and low-confidence outputs.
- Keep an audit trail: log source documents, model and policy versions, evidence or features used, recommendations, overrides and the final decision rationale.
- Protect privacy: limit data access, define retention periods and give customers a usable explanation of how their information affected a decision.
- Control vendors and cyber risk: perform third-party due diligence, enforce least-privilege access, monitor model changes and maintain an incident-response plan.
- Constrain generative AI: ground answers in approved policy sources, require confidence thresholds and prevent a language model from directly issuing an unreviewed payment or denial.
The U.S. Treasury’s Federal Insurance Office said in September 2024 that insurers use AI in claims processing, underwriting, marketing, fraud detection and rating. Treasury roundtables also raised discrimination and privacy protections. OSFI and FCAC identify cybersecurity and third-party dependence as growing concerns. Responsible deployment therefore treats governance as part of the claims product, not as paperwork added later.
How should an insurer introduce claims AI?
A staged rollout limits harm and makes benefits measurable.
- Choose a narrow, material use case. Start with a line such as windshield damage where evidence and permissible outcomes are well defined. Document what the model may recommend and what it may never decide.
- Map the human workflow. Identify every queue, approval, exception and customer communication. Design a switch from digital handling to a named specialist when confidence falls or circumstances change.
- Standardize data and interfaces. Create document and policy-data contracts, connect policy administration, payments, CRM and repair systems, and test legacy-system failure paths. The Lloyd’s Market Association reported data-quality or availability concerns among 49% of respondents and integration problems among 46%.
- Establish a baseline. Record current cycle time, expense, leakage, complaint rate, fraud referrals, appeal outcomes and adjuster workload before automation.
- Validate in production-like conditions. Test precision, recall, calibration, extraction accuracy and subgroup outcomes on representative historical and newly arriving files. Review false positives with claims specialists.
- Launch with limits and monitoring. Use value caps, confidence thresholds and mandatory approvals. Monitor drift, overrides, complaints, payment errors, vendor outages and cybersecurity events.
- Expand only after review. Reuse proven extraction and audit components for another line only when the evidence, regulation and staffing model support it.
What should buyers compare in a claims-AI system?
“AI-powered claims” can mean a document reader, a fraud score, a drafting assistant or a fully automated payment path. Compare the operating capability, not the label.
| Decision area | Questions to ask |
|---|---|
| Line and complexity | Which products, jurisdictions and claim severities are supported? Are bodily-injury and vulnerable-customer paths separated? |
| Automation depth | Is the tool assistive, recommendation-only or straight-through? What value caps and human approvals apply? |
| Evidence handling | Can it extract from PDFs, scans, handwriting, images, emails and legal documents, and show the source for each field? |
| Fraud and networks | Does it score anomalies, connect entities and support investigators without converting a score into an automatic accusation? |
| Integration | Can it exchange data reliably with policy, payments, CRM, repair and subrogation systems? |
| Explainability and control | Are model versions, evidence, overrides, approvals and customer explanations recorded? |
| Measured outcomes | Are cycle-time, cost, accuracy, complaints and leakage figures independently audited or merely vendor-reported case-study results? |
| Deployment obligations | Where is data processed, which regulator applies, and how are residency, retention and incident duties met? |
Will AI replace claims adjusters?
It is more likely to change the job than eliminate it. Machines can read every page, compare patterns and draft routine communications at a scale that people cannot. Adjusters remain needed to investigate ambiguous facts, exercise empathy, negotiate liability, explain decisions, handle appeals and take responsibility when evidence conflicts.

Aviva’s case required more than 40,000 hours of staff training, illustrating that automation shifts skills toward exception handling, model oversight and customer communication. An insurer that removes review capacity while increasing automated referrals can make service worse, even if its extraction model is accurate.
What can policyholders do when AI is involved?
Keep original photographs, receipts, medical records and correspondence, and check every automatically generated summary for factual errors. Ask what information is missing, whether a person reviewed the decision and how to appeal. Correcting a wrong address, date or description early can prevent a valid claim from being routed into the wrong queue.
Do not assume a request for more evidence means a fraud accusation. Conversely, do not ignore a formal denial: use the insurer’s complaint and appeal process, request the decision rationale and observe any regulator or policy deadlines that apply where you live.

Frequently Asked Questions
Does an AI fraud score mean my insurer has accused me of fraud?
No. A score is a prioritization signal based on patterns such as linked claims or inconsistent information. The insurer still needs evidence and should provide a review or appeal route before taking an adverse action.
What information does a claims AI system need to work well?
It needs reliable policy records, consistent claim identifiers, complete documents and clear links to payment, repair and customer systems. Poor data quality and broken legacy integrations can erase the benefit of a sophisticated model.
Can I refuse automated processing of my claim?
The available rights depend on your country, policy and the decision’s legal effect. You can ask the insurer whether a person will review the file, request an explanation and use the formal complaint or appeal process when an automated step affects your coverage or payment.
How can a small insurer adopt claims AI safely?
A small carrier can begin with document extraction or triage for one product, use strict value and confidence limits, and keep specialist review for exceptions. It should contract for audit logs, data residency, security incident duties and access to a human support team before expanding automation.
