A workable AI governance program uses the NIST AI Risk Management Framework’s four functions—Govern, Map, Measure and Manage—as its operating spine. Inventory every AI system, assign accountable owners, classify legal and societal risks, set privacy and safety controls, test performance and harms, document decisions, and keep monitoring after deployment. Use ISO/IEC 42001 when you need a formal AI management system, and map the EU AI Act separately wherever that law applies; neither voluntary framework replaces legal advice.
What an AI governance checklist must accomplish
AI governance is the set of decision rights, controls and evidence that determine how an organization selects, builds, buys, deploys, monitors and retires AI. It is broader than a model-approval form. A chatbot, fraud model, recruiting screener and generative-AI assistant can create different privacy, safety, discrimination, security, consumer-protection and employment risks, even when they use similar technology.
The NIST AI RMF 1.0, released on January 26, 2023, provides a voluntary, technology-neutral structure. Its Core groups outcomes and actions into four functions:

| Function | Question it answers | Typical evidence |
|---|---|---|
| Govern | Who sets policy, accepts risk and provides resources? | Policy, roles, risk appetite, training records, review minutes |
| Map | What is the system, who can be affected and what could go wrong? | Inventory entry, intended-use statement, impact assessment, legal register |
| Measure | How will the organization test trustworthiness and detect deterioration? | Validation results, fairness and privacy tests, security assessments, monitoring metrics |
| Manage | What will the organization do about identified risk? | Mitigation plan, approval or rejection, incident records, corrective actions, retirement decision |
NIST describes the framework as helping developers, users and evaluators manage AI risks that could affect individuals, organizations, society or the environment. Treat privacy, safety, security, transparency, explainability, accountability and fairness as connected socio-technical properties: a technically accurate model can still be unsafe if people cannot challenge its output or if operators are overloaded.

1. Set scope and build an authoritative AI inventory
Start with a single register for internally developed models, embedded vendor features, APIs, automated decision rules, fine-tuned models, pilots and systems used by contractors. Include systems that employees can access through a corporate account; “shadow AI” is a governance gap, not an exception.
Required inventory fields
- System name, version, model family and deployment environment.
- Business purpose, intended users, prohibited uses and whether the system generates advice, content, rankings or autonomous actions.
- Business owner with authority to stop use, and technical owner responsible for operation.
- Developer, vendor, data processors and connected services.
- Training, fine-tuning, retrieval and input data sources, including provenance and licensing.
- People or groups affected, the decisions influenced and the potential severity of harm.
- Deployment geography, sector and lifecycle stage: proposal, development, testing, production, restricted pilot or retirement.
- Interfaces, downstream systems, human-review points and dependencies.
- Current risk tier, approval status, last assessment and next review date.
NIST Govern 1.6 calls for mechanisms to inventory AI systems and resource them according to organizational risk priorities. Make the register change-controlled: a new model version, data source, geography, purpose or vendor should create a review event rather than silently overwrite history.
2. Assign accountability and escalation rights
Accountability must be a named responsibility, not a generic statement that “the team” owns AI. One person may hold several roles in a small organization, but approval and independent challenge should remain distinct for high-impact systems.

|---|---|
| Business owner | Defines the use case, benefits, affected users and resources; can pause or withdraw the system. |
| Technical owner | Controls architecture, model versions, access, deployment, rollback and operational performance. |
| Privacy lead | Reviews lawful purpose, minimization, rights handling, sensitive data, retention and privacy testing. |
| Security lead | Assesses threats, access controls, secrets, supply-chain exposure, abuse resistance and incident response. |
| Legal or compliance reviewer | Maintains the requirements register and interprets applicable privacy, employment, consumer, sector and AI rules. |
| Risk approver | Accepts residual risk within delegated limits or escalates it to the appropriate committee or executive. |
| Incident lead | Coordinates triage, containment, notification, investigation, remediation and lessons learned. |
| Independent escalation route | Lets employees, affected people and reviewers challenge a decision without reporting solely to the delivery team. |
Document reporting lines, decision deadlines, required competence and training. A human reviewer who lacks authority, time or expertise is not meaningful oversight.
3. Map legal, regulatory and societal obligations
Maintain a living requirements register for every deployment geography and affected population. Record the obligation, the system or process it covers, the control that addresses it, the evidence owner and the date it was last checked. Include privacy and data-protection law, sector rules, employment and anti-discrimination requirements, consumer-protection duties, product or workplace safety rules, intellectual-property and confidentiality constraints, records obligations and applicable AI legislation.
NIST Govern 1.1 requires legal and regulatory requirements involving AI to be understood, managed and documented. The register should capture uncertainty as well: if counsel has not resolved whether a use falls inside a legal definition, mark the issue, assign an owner and set a decision date. Do not treat a general-purpose model as automatically exempt because its vendor markets it as a productivity tool.
Assess impacts beyond legal compliance
- Which people could lose access, income, safety, privacy or reputation because of an error?
- Could an output be used downstream for a more consequential decision than the original design intended?
- Could language, disability, cultural context or unequal access produce disparate outcomes?
- What environmental, labor or community effects follow from operation at scale?
- What remedy, appeal or human contact is available to an affected person?
4. Classify risk and set explicit tolerance
Create categories that lead to different controls. A simple four-tier model can be adapted to the organization’s risk appetite:
Recommended Free Tools
| Tier | Illustrative use | Minimum governance |
|---|---|---|
| Prohibited | Uses barred by law, policy or unacceptable harm, such as deliberately deceptive or unsafe operation. | Do not develop or deploy; document the rejection and block access. |
| High impact | Systems that influence employment, eligibility, essential services, safety, significant finances or similarly consequential outcomes. | Formal impact assessment, independent review, qualified human decision-maker, stronger testing, incident plan and executive risk approval. |
| Moderate | Customer support, internal knowledge or recommendations where errors can cause material but recoverable harm. | Documented purpose, privacy and security review, testing, user disclosure, monitoring and named owner. |
| Low | Low-consequence drafting, summarization or experimentation with no sensitive data and no automated decision. | Acceptable-use rules, access controls, basic logging and a route to report problems. |
Define prohibited uses, approval thresholds, mandatory human review, residual-risk acceptance and conditions that force reapproval. Risk should be proportional to potential harm, exposure, reversibility and uncertainty, not merely to model size.

5. Establish privacy and data-governance controls
For each data element, record where it came from, why it is needed, the lawful or authorized purpose, who can access it, how long it is retained and how it will be deleted. Minimize collection and exposure; do not place confidential, personal or regulated data into a model or vendor service without an approved purpose and contract.
Privacy control questions
- Can the organization explain the provenance, quality and permitted use of training, retrieval and evaluation data?
- Are sensitive attributes, identifiers and confidential business information segmented, masked or excluded where possible?
- Are access, export, logging and administrator privileges limited and reviewed?
- Can a person exercise applicable access, correction, objection, deletion or other rights?
- Are retention schedules applied to prompts, outputs, embeddings, backups and logs?
- Does the service provider use submitted data for training or other purposes, and can that use be disabled contractually?
- Have outputs been tested for memorization, personal-data leakage, prompt-injection exfiltration and reconstruction of confidential content?
Keep test cases, findings, remediation and retest results. Privacy is a named trustworthy-AI characteristic in the NIST framework, so it belongs in model evaluation and operations rather than only in a one-time legal review.
6. Design for safety, security and resilience
Write misuse cases before launch: harmful instructions, fraud, harassment, unsafe advice, unauthorized actions, data exfiltration, model manipulation and failures caused by bad or shifted inputs. For each case, specify prevention, detection, response and recovery.
Crashes, No Sound, or Screen Glitches?

A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11- Use secure development practices, dependency review, secrets management, least-privilege access and environment separation.
- Test adversarial prompts, malicious files, indirect prompt injection, data poisoning, model extraction and denial-of-service scenarios when relevant.
- Define confidence limits, refusal behavior, rate limits, content controls and safe fallback paths.
- Require resilience, backup, rollback and a tested way to disable automated actions.
- Record safety assumptions, known limitations and conditions under which the system must not be used.
NIST calls for a safety-first mindset, impact documentation, testing, incident identification and information sharing. A control that exists only in a policy but has never been exercised is an unverified claim; run tabletop and technical exercises at a frequency appropriate to the risk tier.
7. Provide transparency, explainability and contestability
Tell users when they are interacting with or being evaluated by AI, unless a documented exception applies. Explain the system’s purpose, data limitations, uncertainty, human role and how to obtain help. Preserve the input context, model version, output, reviewer action and decision rationale needed to reconstruct a consequential event.
Match explanation to impact
- Low impact: label generated content and provide a way to report an incorrect or unsafe response.
- Moderate impact: disclose relevant limitations, show the basis or sources where feasible and provide human support.
- High impact: explain the material factors considered, identify the accountable decision-maker, provide a meaningful contest route and prohibit rubber-stamping by reviewers.
Explainability does not mean exposing proprietary source code or promising that a complex model has a single simple cause for every output. It means giving people information that lets them understand, challenge and correct an outcome proportionate to its consequences.
8. Specify real human oversight
Write the exact point at which a qualified person must review, override, pause or stop an AI output or action. Define the reviewer’s competence, authority, independence, workload and response time. Do not label a process “human in the loop” if the person sees only a recommendation, cannot inspect relevant information or is measured mainly on speed.
Require reviewers to record acceptance, modification, rejection and escalation reasons. Analyze overrides for recurring failure patterns, disparate effects and signs that the model or workflow needs redesign. For safety-critical or high-impact use, provide a default-safe state when the reviewer is unavailable or the model is outside its validated conditions.

9. Test and measure before and after deployment
Define pass/fail criteria before testing so a favorable result cannot be retrofitted after the fact. The measurement plan should cover validity, reliability, safety, security, privacy, fairness, robustness, explainability and operational performance as applicable to the use case.
| Measurement area | Examples of evidence |
|---|---|
| Validity and reliability | Performance against representative, versioned test data; error rates by important segment; repeatability and calibration. |
| Fairness | Outcome and error comparisons for relevant groups, with documented rationale for selected metrics and thresholds. |
| Robustness and safety | Boundary tests, adversarial tests, unsafe-output rates, fail-safe behavior and recovery results. |
| Privacy | Leakage tests, access reviews, retention checks and rights-request results. |
| Security | Threat-model findings, penetration or red-team results, vulnerability remediation and access logs. |
| Operations | Latency, availability, cost controls, drift indicators, complaint volumes and override rates. |
Control the test data and code, identify who performed and independently reviewed the tests, preserve results and retest after material changes. Set recurring measurement intervals by risk tier; continuous monitoring is appropriate for systems whose inputs, models or impacts change quickly.
10. Monitor incidents, complaints and model change
Monitoring begins at launch and includes more than accuracy. Track performance, harmful outputs, complaints, privacy and security events, drift, access anomalies, supplier changes, override rates and unexpected downstream use.

- Detect and triage: capture the system, version, time, input context, affected people and immediate risk; assign a severity level.
- Contain: pause automation, restrict access, roll back a model or disable a feature when continued operation could increase harm.
- Notify: follow internal escalation and any legal, contractual or regulator notification deadlines.
- Investigate: preserve evidence, identify technical and process causes, and check for similar events elsewhere.
- Correct: fix data, prompts, code, controls, training or user communication; obtain approval before reopening.
- Learn: record root cause, affected scope, remediation owner, due date and lessons for other systems.
Define complaint intake and response channels for employees, customers and affected individuals. Trend complaints and overrides rather than closing each ticket in isolation.
11. Control vendors and the AI supply chain
Vendor due diligence should cover the service’s model and data practices, security controls, privacy commitments, subcontractors, geographic processing, intellectual-property and rights risks, evaluation evidence, incident history and business continuity. Put operational requirements in the contract.
- Specify permitted data use, retention, deletion, isolation and breach notification.
- Require documentation of model versions, material changes, limitations and test results.
- Obtain change notices, audit or assurance rights and a usable incident contact.
- Define service levels, access controls, support for rights requests and cooperation with investigations.
- Maintain a contingency plan: export needed records, switch providers, fall back to manual work or shut down safely.
Reassess a supplier when its model, hosting location, subprocessors, terms, training practice or intended use changes. A contract cannot transfer the organization’s accountability for decisions made with the service.
12. Keep documentation that proves controls work
For each system, maintain a traceable evidence package:

13. Plan safe decommissioning
Retirement is a lifecycle stage, not an afterthought. Define triggers such as unacceptable residual risk, repeated control failure, legal change, unsupported dependencies, supplier withdrawal, drift beyond validated limits or a safer replacement.
- Approve the withdrawal and identify affected users, decisions and integrations.
- Stop new processing and disable credentials, endpoints, scheduled jobs and automated actions.
- Roll back or replace dependent workflows and provide a manual or alternative path.
- Notify users or affected people when continued reliance could cause confusion or harm.
- Retain or delete prompts, outputs, models, logs and personal data according to legal and policy requirements.
- Record the shutdown, residual obligations, lessons learned and owner for any follow-up.
NIST Govern 1.7 explicitly addresses safe phasing out of AI systems. Test shutdown and rollback before an emergency requires them.
14. Set review cadence and improve the program
Assign a review frequency to each risk tier and trigger an out-of-cycle review after a material model, data, purpose, geography, supplier or interface change; a significant incident or complaint trend; a legal change; or evidence that assumptions no longer hold. Include external stakeholder feedback in design and operation.
| Review activity | Suggested trigger | Decision output |
|---|---|---|
| Operational check | Routine interval set by risk tier | Continue, tune monitoring or open corrective action |
| Change review | New model, data, vendor, purpose or geography | Re-test, reapprove, restrict or reject the change |
| Incident review | Severity threshold or repeated complaints | Containment, root cause, remediation and notification decision |
| Governance review | Periodic management or board cycle | Risk appetite, resources, policy and portfolio decisions |
Choosing NIST AI RMF, ISO/IEC 42001 and the EU AI Act
These instruments serve different purposes. Use them together when appropriate rather than treating them as interchangeable checklists.
| Dimension | NIST AI RMF 1.0 | ISO/IEC 42001 | EU AI Act |
|---|---|---|---|
| Legal status | Voluntary framework. | International management-system standard; organizations may pursue conformity or certification through applicable arrangements. | Jurisdiction-specific European Union law with binding obligations and enforcement institutions. |
| Geography | Usable in any geography. | Usable internationally. | Applies according to the Act’s scope and roles, including certain activity connected to the EU; obtain legal advice for classification. |
| Operating model | Govern, Map, Measure and Manage outcomes and actions. | Formal management-system structure, documented processes and continual improvement. | Risk categories, prohibited practices and obligations that vary by system role and category. |
| Lifecycle coverage | Designed for the full AI lifecycle and adaptation to context. | Organization-wide management-system lifecycle. | Requirements attach to defined practices, providers, deployers and risk categories. |
| Human oversight | Outcome to define oversight proportionate to risk. | Requires documented organizational controls and competence; exact measures depend on the system and implementation. | Contains specific obligations for applicable high-risk and other covered systems; map the legal duty to the responsible role. |
| Documentation and evidence | Flexible evidence selected for the use case. | More formal management-system records and audit evidence. | Legally required documentation and technical or operational evidence for applicable categories. |
| Auditability | Self-assessment or external assessment is possible but not mandated by the framework. | Designed to support conformity assessment or certification where pursued. | Supervision and enforcement follow the Act and relevant authorities. |
| Integration | Can extend existing privacy, security, quality and enterprise-risk systems. | Integrates through common management-system controls and continual-improvement practices. | Must be mapped to existing legal, privacy, security, quality and product processes. |
| Implementation effort | Flexible; effort scales with risk and organizational maturity. | Typically higher process and evidence discipline because it is a management-system standard. | Effort depends on whether a system is covered and which obligations apply; legal classification comes first. |
| Review cadence | Set by risk, change and incident conditions. | Management-system review and continual-improvement cycles. | Deadlines and ongoing duties arise from the applicable legal requirements. |
The NIST AI RMF Playbook can suggest actions and references, but NIST states that it is neither a checklist nor a sequence every organization must follow. Use the Core to tailor controls to context. A NIST-published crosswalk can support alignment between NIST and ISO/IEC 42001; it does not make one framework a substitute for the other. For the EU AI Act, first determine whether the organization is a provider, deployer or another covered actor and which risk category applies, then map each legal duty to an owner, control and evidence item.
A practical implementation sequence
Organizations starting from zero can build the program in increments without waiting for a perfect policy.
- Mandate and risk appetite: approve prohibited uses, decision rights, escalation and resources.
- Discover: populate the inventory through procurement, engineering, security, privacy and employee-use channels.
- Tier: classify systems by potential harm, exposure, reversibility and uncertainty.
- Baseline controls: require purpose, owner, data review, security access, user disclosure, testing and monitoring for every tier.
- Deepen high-impact controls: add independent review, human authority, stronger evidence, contest routes and executive risk acceptance.
- Operationalize: connect monitoring, incident response, supplier management, change control and decommissioning to existing workflows.
- Assure: sample evidence, test controls, review metrics and close corrective actions on a defined schedule.
Measure governance itself: inventory coverage, percentage of systems with current owners and assessments, overdue reviews, test pass rates, unresolved high-severity findings, mean incident response time, complaint resolution and successful rollback exercises. These indicators show whether policies operate in practice without pretending that one score captures AI risk.
Common failure modes and fixes
“We have a policy, so we are covered.”
A policy without an inventory, owner, test result or monitoring record cannot demonstrate control. Link each requirement to an evidence artifact and review it.

“The vendor handles compliance.”
Vendor assurances do not define your purpose, affected groups, human decisions or local legal duties. Perform due diligence, contract controls and ongoing change review.
“Human review” is a checkbox
If reviewers lack authority, context, time or independence, automation bias remains. Set competence requirements, workload limits, override logging and a stop mechanism.
Testing only average accuracy
Average performance can hide subgroup errors, privacy leakage, unsafe edge cases and drift. Test the harms that matter for the actual use and keep results versioned.
Ignoring low-code and embedded AI
Features inside ordinary software can process sensitive data or influence decisions. Include procurement catalogs, expense claims, support tools and employee experiments in discovery.
No retirement plan
Unmaintained models continue to receive data and drive decisions. Define shutdown triggers, deletion, replacement, user notice and rollback before launch.
What “proof” looks like to an auditor, regulator or affected person
A credible record connects the entire chain: the organization knew the purpose and affected groups; assigned an accountable owner; identified applicable duties; evaluated risks; selected and tested controls; obtained an approval within delegated authority; monitored real operation; responded to incidents and complaints; and changed or retired the system when assumptions failed. Keep enough context to reproduce a decision, while applying access and retention controls to the evidence itself.
This approach turns the checklist into an operating system for responsible AI rather than a one-time compliance exercise. NIST supplies the flexible structure, ISO/IEC 42001 can formalize management-system governance, and the EU AI Act supplies binding duties where its scope reaches the deployment.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsFrequently Asked Questions
Is the NIST AI RMF mandatory?
NIST AI RMF 1.0 is voluntary. It is a flexible control and evidence structure; organizations still must meet applicable laws and contracts.
Can ISO/IEC 42001 replace an EU AI Act assessment?
No. ISO/IEC 42001 can organize an AI management system and support conformity work, but it does not determine whether a system falls into an EU AI Act category or satisfy every legal obligation.
How often should an AI system be reassessed?
Set the interval by risk tier and reassess sooner after a material model, data, purpose, geography or supplier change, a significant incident, a complaint trend or a legal change.
What is the most important evidence to retain?
Keep the inventory and owner, intended use, risk and legal assessment, data records, test results, approvals, monitoring logs, incidents, complaints, change decisions and retirement record.
