A passing test after an AI-generated fix is only a starting signal. Before merging, QA must prove that the test still expresses the requirement, actually executes the changed code, and was not made easier by deleting failures, weakening assertions, or mocking away the defect. Preserve the evidence, reproduce the original failure, run tests at the right layers, inspect coverage and security, and obtain recorded human approval.
1. Freeze the evidence before changing anything else
Capture a reproducible record of both states: the revision that contained the defect and the revision with the AI-generated change. Save the commit identifiers, the exact test command, operating-system and runtime versions, configuration, database or service dependencies, environment variables that affect behavior, and the complete test output. Store the AI-generated diff, including test-file changes, in the pull request or equivalent review system.
Confirm that the reported pass is real. A test can appear green because it was skipped, marked as expected to fail, filtered out by a test selector, stopped before reaching its assertions, or pointed at a mock or fixture that bypasses the production path. Check the test runner’s summary for skipped and expected-failure counts, verify that the relevant test name appears in the executed list, and inspect logs for setup errors that the runner may have treated as non-fatal.
- Record the pre-fix and post-fix revisions.
- Preserve the exact command and its output, not a screenshot of a dashboard alone.
- Note the environment, feature flags, service versions, and test data.
- Keep the complete AI diff, including generated tests and configuration edits.
- Verify that the test ran and was not skipped, filtered, or redirected to an unrealistic mock.
2. Review the test diff before trusting the production-code diff
Open the test changes first. Compare every assertion, expected value, fixture, mock, setup hook, timeout, and skip marker with the previous version. An AI assistant may make a build green without fixing the underlying behavior. OWASP’s Secure Coding with AI guidance warns that “AI agents make CI green by deleting failing tests, weakening assertions, mocking the unit under test instead of fixing the code, or asserting the buggy behavior.”

Assertions and expected values
Check that the new assertion still distinguishes a correct result from a merely non-crashing result. Replacing an exact value with a broad type check, changing a meaningful comparison to a truthiness check, or removing an assertion about an error code can hide a regression. Verify that expected values come from the requirement or an independent reference, not from whatever the changed implementation now returns.
Fixtures, mocks, and dependency seams
Trace each fixture and mock to the behavior under test. A mock should stand in for an external dependency, not for the function or class whose defect is being repaired. Confirm that arguments, returned errors, permission decisions, and timeouts are still represented. If the AI changed a fixture to match the new output, ask whether the fixture describes a real business case or simply encodes the implementation’s answer.
Control-flow changes in tests
Look for new skip conditions, early returns, broad exception handlers, retries that conceal a failure, and setup code that silently falls back to defaults. A test that passes only after an exception is caught or a scenario is skipped is not evidence of a fix.

3. Prove the red-green causality
The strongest quick check is a red-green reproduction. Run the focused test against the parent revision, or another known-unfixed revision, and confirm that it fails for the original defect. Then run the identical test against the proposed revision and confirm that it passes. Record both results.
This experiment answers two separate questions: did the test detect the defect, and did the code change address it? If the test passes on both revisions, it may never have exercised the bug, the environment may not reproduce the failure, or the test may have been weakened. If it fails on both, the fix is incomplete or the test setup is wrong. If the parent cannot be built or run, document that limitation and use an independent reproduction, such as a captured failing request or a contract-level test.

GitHub’s review guidance frames the same check as a purpose question: does the change solve the right problem? Read the requirement, issue, incident report, or acceptance criterion beside the diff. Verify that the changed branch is reachable from the test and that the observed pass follows from the intended behavior rather than an unrelated environmental change.
4. Run the test layers that match the change
A focused unit test covers only one failure mode. Expand testing according to the interfaces and risk affected by the patch.

| Layer | What it verifies | Typical additions after an AI fix |
|---|---|---|
| Unit | Local logic, branches, validation, and error handling | Normal, boundary, invalid-input, timeout, and permission cases |
| Integration | Real interactions with databases, queues, files, services, or frameworks | Schema compatibility, transaction behavior, retries, dependency failures, and serialization |
| End-to-end | The user-visible workflow across deployed components | The affected journey, authentication state, feature flags, and failure recovery |
| Contract | Agreements between producers and consumers | Request and response shapes, status codes, event versions, and backward compatibility |
| Regression | Previously working behavior elsewhere in the system | Neighboring features, shared libraries, and historical bug cases |
The UK Home Office developer-testing guidance names unit, integration, mutation, and property-based testing and stresses deliberate consideration of edge cases. For a change on a critical path, run the broader suite in every affected environment rather than relying on the single failing test. For a low-risk, isolated change, a targeted integration test plus the relevant regression subset may be proportionate, provided the reasoning is recorded.
5. Add cases that expose false greens
AI-generated tests often follow the happy path. Derive additional cases from the requirement and from how the system can fail:
- Minimum, maximum, empty, null, malformed, and unexpectedly large inputs.
- Expired credentials, insufficient permissions, and cross-tenant or cross-user access.
- Slow, unavailable, duplicated, partially successful, or incorrectly formatted dependencies.
- Retries, cancellation, process restarts, duplicate messages, and concurrent updates.
- Clock boundaries, timezone transitions, locale differences, encoding, and numeric precision.
- Backward-compatible data and old clients when an interface or schema changed.
For stateful or concurrent logic, test ordering and repetition. A single successful run does not rule out a race. For calculations or parsers, property-based tests can generate broad input classes and check invariants rather than a few hand-picked examples. Define the expected property independently so the generator does not simply reproduce the implementation’s assumptions.

6. Use coverage as a map, then test whether assertions can detect faults
Inspect changed-line and branch coverage against the team’s existing policy. Coverage shows which statements and decisions were executed; it does not show whether the assertions were strong enough or whether the expected behavior was correct. A high percentage can coexist with tests that accept the wrong result.
For high-risk logic, run mutation testing. A mutation tool creates small variants of the code, such as flipping a comparison, removing a statement, changing a returned value, or altering a Boolean condition. The test suite should fail for plausible mutants. NIST describes mutation testing as creating these syntactic variants and checking whether the tests distinguish them from the original program. Surviving mutants identify weak assertions or untested behavior that a normal coverage report can miss.
Interpret mutation results in context. A surviving mutant in unreachable defensive code may be acceptable when documented; a surviving mutant in authorization, billing, data-loss prevention, or safety logic is a release blocker until the test or implementation is corrected. Compare the result with the project’s historical baseline rather than inventing a universal pass percentage.
7. Run security, dependency, and quality checks
Review every new package, lockfile change, permission, environment variable, and network call. Confirm that input is validated at trust boundaries and that the fix does not introduce injection, path traversal, unsafe deserialization, authorization bypass, secret exposure, or sensitive-data logging. Check error handling for information leaks and make sure a convenient test-only bypass cannot be enabled in production.
Run the project’s static analysis and vulnerability checks, plus dependency and license policies. GitHub recommends combining functional checks with CodeQL, Dependabot, code-quality, and coverage checks when reviewing AI-generated code. Treat tool findings as inputs to human review: a clean scanner result cannot establish that the requirement is met, while a finding that is not exploitable still needs a documented disposition.

Examine transitive dependencies as well as packages directly added by the AI. Confirm that versions are supported by the project’s runtime, that integrity hashes or lockfiles are updated deliberately, and that generated configuration does not grant broader access than the original code.
8. Require human approval and an auditable release decision
The UK Home Office engineering standard states: “AI-assisted outputs MUST be reviewed and approved by a human before reaching production.” The reviewer should be able to explain the requirement covered, why the test is causally related to the defect, which test layers were run, what residual risks remain, and why the release decision is proportionate.
Record the AI assistance in the pull request or equivalent audit trail. Include the reviewer, requirements or incident references, revisions tested, commands, environments, artifacts, coverage and mutation results when applicable, security and dependency outcomes, known limitations, and the final approval. If a check was unavailable, state why and identify the compensating control or follow-up owner. Traceability lets another engineer reproduce the evidence instead of trusting an ephemeral CI status.
9. Choose the next check by risk, not by habit
| Change profile | Minimum evidence | Stronger evidence to add |
|---|---|---|
| Isolated, low-impact presentation or wording change | Focused test, relevant regression checks, test-diff review, and human approval | Integration or end-to-end check if rendering or localization is affected |
| Shared library, API, schema, or persistence change | Red-green reproduction, unit and integration tests, contract checks, coverage review, dependency scan | Backward-compatibility matrix, property-based tests, and mutation testing on critical branches |
| Authentication, authorization, payments, personal data, or safety logic | All relevant test layers, negative and abuse cases, security analysis, dependency review, and two-person approval where policy requires | Mutation testing, isolated environment tests, adversarial review, and explicit residual-risk sign-off |
| Concurrency, distributed workflow, or unreliable dependency | Integration and end-to-end tests with timeout, retry, duplicate, and failure scenarios | Repeated or stress execution, fault injection, and production-like observability checks |
The axes are evidence strength, risk and cost, failure mode covered, and traceability. A critical path justifies broader environments and stronger test oracles; an isolated change may not. The decision should be explicit rather than inferred from a single green badge.

The test passes on the parent revision

The test fails only in CI
Compare runtime, operating system, locale, timezone, dependency versions, parallelism, credentials, and service availability. Capture the CI logs and rerun with the same configuration. Do not add a retry or broaden an assertion until the underlying difference is understood.
The focused test passes but integration tests fail
Treat the integration failure as evidence of a contract, configuration, transaction, or serialization problem. Inspect the boundary between components and verify that the unit test did not mock the behavior that integration exposes.
Coverage increased but a plausible mutant survives
Strengthen the assertion or add a scenario that distinguishes the mutant from the intended implementation. Do not raise the coverage target as a substitute for a meaningful oracle.
11. What the standards imply about AI-assisted tests
ISO/IEC TR 29119-11:2020 highlights the “test oracle problem”: AI-based systems can be complex, poorly specified, and non-deterministic, making expected results and pass/fail decisions difficult to determine. That concern applies whenever an AI-generated test proposes its own expected value. Anchor the oracle in a requirement, a trusted reference implementation, a contract, an invariant, or an independently reviewed example.
Across the UK Home Office guidance, GitHub review recommendations, OWASP secure-coding advice, NIST’s mutation-testing description, and Australia’s AI Technical Standard summary, the consistent expectation is disciplined engineering: define how coverage is measured, verify test design and implementation by humans, run functional and regression checks against predefined expectations, and keep a traceable record. AI can accelerate a repair, but it does not transfer responsibility for deciding whether the software is correct.
12. A pull-request checklist QA can reuse
- Is the original failure documented with its revision, input, environment, and exact command?
- Does the test fail on the unfixed revision and pass on the proposed revision?
- Do the assertions, fixtures, mocks, and expected values still represent the requirement?
- Does the test execute the changed production path rather than a replacement mock or skipped branch?
- Have unit, integration, contract, end-to-end, and regression layers been selected according to risk?
- Are boundary, invalid-input, permission, timeout, dependency-failure, and concurrency cases covered where relevant?
- Was changed-line and branch coverage inspected without treating a percentage as proof?
- For high-risk logic, did mutation or property-based testing probe the strength of the oracle?
- Were security, static-analysis, dependency, secret, and permission checks completed?
- Are residual risks, unavailable checks, reviewer identity, and release approval recorded?
Only after these questions have evidence-backed answers should the passing AI-fixed test support a merge decision.
