For mild adult acne, start with one proven over-the-counter active—usually 2.5% benzoyl peroxide, adapalene, or salicylic acid—and use it consistently. See a dermatologist when acne is painful, scarring, widespread, hormonally patterned, or not improving with a well-used OTC plan. Prescription combinations, hormonal medicines, isotretinoin, and selected procedures are matched to the acne type and the risks of treatment; there is no single best option for every adult.
How to choose an adult-acne treatment
Adult acne usually falls into overlapping patterns: clogged pores (whiteheads and blackheads), inflamed pimples, deeper nodules, or breakouts that follow a hormonal pattern on the lower face, chest, or back. Severity, scarring risk, previous treatment, pregnancy considerations, other medicines, and how quickly you need control all affect the choice.
The 2024 American Academy of Dermatology (AAD) guideline contains 18 evidence-based recommendations and five good-practice statements. It strongly supports topical retinoids, benzoyl peroxide, topical antibiotics, oral doxycycline and combinations that use different mechanisms. The guideline also emphasizes individualized treatment rather than a universal routine.
| Situation | Common starting direction | Supervision and cautions |
|---|---|---|
| Mostly whiteheads and blackheads | Adapalene or salicylic acid; benzoyl peroxide if inflamed spots are also present | OTC access is common; irritation is the main early problem |
| Mild inflamed pimples | 2.5% benzoyl peroxide, adapalene, or both used in a tolerable schedule | Start gradually and protect irritated skin |
| Many inflamed lesions, chest or back acne, or persistent flares | Prescription combination therapy and, when appropriate, a limited course of oral doxycycline | Clinician assessment; antibiotics should be paired with benzoyl peroxide |
| Lower-face or cyclical adult acne | Consider hormonal options such as spironolactone or a combined oral contraceptive when clinically appropriate | Prescription screening and monitoring are required |
| Nodules, scarring, major distress, or failure of standard therapy | Dermatology referral; isotretinoin may be considered | Medical supervision and significant safety monitoring |
Cost and access vary by country, insurance, formulation and clinician availability. OTC products are usually the easiest to obtain; prescriptions require an evaluation; injections and extraction are office procedures. The evidence reviewed by the AAD does not establish a broadly applicable treatment-success percentage for these options.

Over-the-counter options
Benzoyl peroxide
Benzoyl peroxide reduces acne-causing bacteria and is particularly useful for mild inflamed pimples. Products may contain concentrations up to 10%, but the AAD advises starting at 2.5% to reduce dryness, irritation and burning. A stronger concentration is not automatically a better choice. Apply a thin layer to acne-prone skin according to the product directions rather than repeatedly spot-treating with excessive amounts.
Benzoyl peroxide can be used alone for mild inflammatory acne or paired with a retinoid. It is also important when an antibiotic is prescribed: the AAD recommends using topical and oral antibiotics simultaneously with benzoyl peroxide to reduce the risk of antibiotic resistance. Benzoyl peroxide can bleach fabric, so use white towels and allow the product to dry before dressing.
Adapalene
Adapalene is an OTC topical retinoid that helps prevent clogged pores and reduces inflammation. It is a strong guideline-supported option for comedonal and inflammatory acne. Use it consistently as directed, applying a thin layer to the acne-prone area rather than only to visible spots. If peeling or burning develops, introduce it less often, use a gentle cleanser and moisturizer, and increase frequency only as your skin tolerates it.
Do not stack several irritating actives on the first night. A simple routine makes it easier to identify the cause of a reaction and to stay consistent. Discuss adapalene and all retinoid use with a clinician if you are pregnant, planning pregnancy or breastfeeding, because treatment selection may need to change.

Salicylic acid
Salicylic acid unclogs pores and exfoliates. The AAD says it works best for whiteheads and blackheads. It can be useful as a cleanser or leave-on product when comedonal acne is the main issue, but overuse can dry or irritate skin. If you also use adapalene or benzoyl peroxide, add salicylic acid cautiously rather than starting every active at once.
Azelaic acid
Azelaic acid can unclog pores, address acne-related bacteria and inflammation, and fade dark spots left after acne. It is a conditional guideline option and may be available without a prescription or by prescription depending on the formulation and country. Its ability to address both active acne and post-acne discoloration can make it useful when lingering marks are a major concern.
Avoid unproven household substitutes
Toothpaste is not an acne treatment. Julie Harper, MD, FAAD, advises keeping toothpaste for teeth and relying on clinically proven ingredients such as adapalene, benzoyl peroxide and salicylic acid. Toothpaste can irritate or burn skin without treating the processes that cause acne.

Retinoids and combination therapy
Prescription retinoids are used when OTC adapalene is insufficient, not tolerated, or when a clinician wants a different formulation or strength. Combining a retinoid with benzoyl peroxide attacks clogged pores, inflammation and acne-related bacteria through different mechanisms. Combination products can simplify a routine, while separate products allow more control over frequency and irritation.

Topical antibiotics
Topical antibiotics can reduce inflammatory lesions, but they should not be used as an indefinite solo treatment. The AAD recommends pairing topical antibiotics with benzoyl peroxide to help prevent resistance. Your clinician may also combine them with a retinoid so that maintenance does not depend on prolonged antibiotic exposure.
Clascoterone
Clascoterone is a topical antiandrogen approved for acne in people aged 12 and older. AAD treatment information describes twice-daily application. It may be considered when androgen-related signaling appears relevant, particularly if standard topical combinations have not provided enough control. A prescriber should confirm whether it fits your skin, other medicines and treatment goals.
When oral antibiotics are appropriate
Oral doxycycline and related antibiotics are options for more inflammatory acne, including acne that is extensive or difficult to reach with topical treatment. They are not a cure for the underlying tendency to form acne, so guidelines advise limiting systemic antibiotic exposure to the shortest clinically appropriate course and combining the antibiotic with topical therapy, including benzoyl peroxide.
- Ask how the oral medicine will be combined with a topical retinoid or benzoyl peroxide.
- Clarify the planned review date and what will replace the antibiotic for maintenance.
- Report significant side effects or new medical problems promptly rather than stopping and restarting on your own.
- Tell the prescriber about pregnancy, plans for pregnancy, allergies and all medicines or supplements.
If acne returns after an antibiotic course, that usually signals a need to adjust the maintenance plan or reconsider the diagnosis, not to repeat antibiotics indefinitely.

Hormonal treatment for adult acne
Spironolactone
Spironolactone can help women with stubborn hormonal acne on the face, chest or back. It is a prescription medicine and is most useful when the clinical pattern supports hormonal treatment. Your clinician will decide whether it is appropriate, review medical history and other medicines, and determine what monitoring is needed.
Combined oral contraceptives
Combined oral contraceptives are another conditional hormonal option for selected patients. Suitability depends on factors such as health history, contraceptive needs, risks and preferences. They are not an automatic first choice for every adult with acne; discuss alternatives if hormonal contraception is not desired or is medically unsuitable.
Hormonal therapy generally requires patience and a maintenance strategy. Tell the clinician if you are pregnant, trying to conceive or breastfeeding, because those circumstances change which medicines can be considered.

When isotretinoin is considered
Isotretinoin is a prescription retinoid reserved for severe acne, acne that is scarring, acne causing substantial psychosocial burden, or acne that has failed standard oral or topical therapy. The AAD guideline states that patients with severe acne or those who have failed standard oral or topical treatment should be offered isotretinoin consideration.

Isotretinoin requires a dermatologist’s assessment, medical supervision and significant safety monitoring. Before starting, discuss previous treatments, mental and physical health, all medicines, pregnancy potential and the monitoring schedule. Do not obtain or share isotretinoin outside the prescribing and monitoring system. A dermatologist determines candidacy, dosing, follow-up and when treatment should stop.
Procedures for selected lesions
Intralesional corticosteroid injections
An intralesional corticosteroid injection places medicine directly into a large, painful inflammatory lesion. It is an adjunct, not a replacement for an acne-control plan, and can provide faster relief than waiting for the lesion to settle naturally. A trained clinician must select the lesion and dose because inappropriate injection can cause complications such as a depression or pigment change.
Acne extraction
During acne extraction, a dermatologist physically removes selected breakouts, usually clogged lesions that are suitable for the technique. Extraction does not prevent new acne and should not be attempted aggressively at home, where squeezing can worsen inflammation and scarring.

The updated AAD guideline found insufficient evidence to recommend chemical peels, laser and light-based devices, microneedling, dietary changes, vitamins or plant-based products routinely for acne. It recommends against adding pneumatic broadband light to adapalene 0.3% gel. These findings do not mean every device or procedure is useless for every person; they mean the evidence is not strong enough for routine guideline recommendations. Ask what specific acne type the procedure targets, what evidence supports it, what adverse effects are possible and how it fits with proven medical therapy.
A practical step-up plan
- Classify the problem. Note whether you mainly have blackheads and whiteheads, inflamed pimples, deep nodules, scarring, dark marks or a cyclical lower-face pattern.
- Start one tolerable OTC active. For inflamed mild acne, begin with 2.5% benzoyl peroxide; for clogged pores, consider adapalene or salicylic acid. Azelaic acid is an option when acne and dark marks both matter.
- Build consistency before adding complexity. Use the product as directed, introduce irritating products gradually, moisturize and avoid picking. Changing several products at once makes irritation and benefit impossible to interpret.
- Arrange a dermatology visit for red flags. Seek care for painful nodules, scarring, widespread chest or back acne, significant distress, suspected hormonal acne, or acne that persists despite consistent OTC use.
- Use prescriptions strategically. A clinician may combine a retinoid, benzoyl peroxide, azelaic acid, clascoterone or a topical antibiotic; oral doxycycline may be added for inflammatory disease, with antibiotic exposure limited.
- Escalate when the burden justifies it. Consider hormonal treatment for an appropriate hormonal pattern, an injection for a large painful lesion, or isotretinoin for severe, scarring, highly burdensome or treatment-resistant acne.
Common failure modes and fixes
- Burning and peeling: reduce application frequency, simplify the routine and moisturize; ask a clinician if symptoms are severe or persistent.
- No improvement because use is inconsistent: attach treatment to a daily habit and use a thin, even layer as directed.
- Antibiotic monotherapy: ask whether benzoyl peroxide and a non-antibiotic maintenance treatment should be added.
- Picking or home extraction: stop manipulating lesions and request professional extraction or an intralesional injection when appropriate.
- Chasing procedures before establishing medical care: ask whether the proposed peel, laser, light treatment or microneedling has guideline support for your acne type.
- Ignoring pregnancy or medication context: disclose pregnancy, pregnancy plans, breastfeeding, allergies and every medicine before starting or changing treatment.
How the options differ at a glance
| Option | Best fit | Speed and burden | Access |
|---|---|---|---|
| Benzoyl peroxide 2.5% | Mild inflamed pimples; antibiotic companion | Gradual control; dryness, burning and fabric bleaching possible | OTC |
| Adapalene | Clogged pores and inflammatory acne | Gradual; irritation is common while adapting | OTC in many markets |
| Salicylic acid | Whiteheads and blackheads | Exfoliating; overuse can irritate | OTC |
| Azelaic acid | Acne with post-acne dark spots | Gradual; formulation determines access | OTC or prescription, depending on country |
| Topical antibiotic plus benzoyl peroxide | Inflammatory acne requiring prescription treatment | Limited antibiotic exposure; resistance prevention requires the combination | Prescription |
| Oral doxycycline | More extensive inflammatory acne | Systemic side-effect burden; shortest appropriate course | Prescription |
| Spironolactone or combined oral contraceptive | Selected hormonally influenced acne | Requires screening, follow-up and individual risk assessment | Prescription |
| Isotretinoin | Severe, scarring, highly burdensome or treatment-resistant acne | Most intensive monitoring and safety requirements | Dermatologist prescription |
| Injection or extraction | One or a few suitable lesions | Potentially faster relief; does not prevent new acne | In-office procedure |
When to seek medical help promptly
Arrange professional care if you develop deep painful nodules, scars, rapidly worsening acne, extensive trunk involvement, or distress that affects daily life. Also seek advice before starting prescription treatment, combining multiple strong actives, or using any acne medicine during pregnancy or while trying to conceive. A dermatologist can confirm that the eruption is acne, choose a proportionate treatment and set a maintenance plan that avoids unnecessary antibiotic exposure.
