For most practices, the clearest alternative is to send a prescription to a licensed pharmacy the patient chooses. Mail-order filling may also work when it suits the patient and medication. A doctor’s office may still be able to furnish some medicines directly, but that depends on state and federal rules and the circumstances—not simply on the practice’s preference.
What are the alternatives to selling medicines in the office?
Send the prescription to a patient-selected pharmacy
Ask the patient which pharmacy they prefer, then write or transmit the prescription there. This separates the clinical decision to prescribe from the pharmacy’s dispensing and sale. The American Medical Association (AMA) says physicians should respect patients’ freedom to choose where prescriptions are filled. Check relevant insurance or benefit-network requirements before promising that a particular pharmacy will be covered or that a medicine will cost a certain amount. Consider mail-order fulfillment
Mail order is another way to have a prescription filled, not a guarantee of coverage or suitability. The Food and Drug Administration (FDA) describes mail-order filling when a licensed physician writes the prescription, the exact prescribed drug is dispensed, and labeling requirements are met. That guidance is dated and does not state every current federal or state requirement. For a particular prescription, consider plan coverage, shipping time, medication stability, urgency, and the patient’s preference. FDA guidance on pharmacy compounding and mail-order dispensing.
Use electronic transmission as the delivery method
E-prescribing sends a prescription to a pharmacy; it does not itself authorize a physician’s office to dispense medicine. CMS describes e-prescribing requirements for covered Medicare Part D prescriptions. CMS says NCPDP SCRIPT version 2023011 is scheduled to become the adopted standard for covered Part D transactions on January 1, 2028. Confirm the requirements and any exceptions that apply to the specific prescription and practice. CMS: E-prescribing.

Keep only limited in-office furnishing, if permitted
A practice may be able to furnish a medicine directly in a limited situation, such as an immediate patient need. Whether it can do so—and what registration, storage, labeling, recordkeeping, drug-category, or controlled-substance rules apply—depends on the jurisdiction and workflow. A narrow exception is not a general way around a restriction on routine dispensing.
Why prescribing, transmitting, and dispensing are different
Prescribing is the clinician’s decision and order for a patient. Transmission is how that prescription reaches the chosen pharmacy. Dispensing is the pharmacy’s process of supplying the medicine against the prescription. Practitioner furnishing is the office’s direct supply of medicine to a patient under whatever authority applies. Permission for one of these activities does not automatically establish permission for another.

The AMA’s ethics guidance says physicians should prescribe on medical grounds and patient need, dispense from the office only when doing so primarily benefits the patient, and respect the patient’s pharmacy choice. It generally advises against referring patients to a pharmacy the physician owns or operates. State rules can lead to different answers
These examples illustrate why an office needs to check the law where it practices. They are not interchangeable, and they do not establish a nationwide rule.

Florida: qualifying practitioners may dispense subject to conditions
The 2024 Florida Statutes provide that qualifying practitioners authorized to prescribe may dispense drugs to their patients in the regular course of practice, subject to the statute’s conditions. Those include giving the patient a written prescription and advising that it may be filled either in the practitioner’s office or at any pharmacy. Virginia: office-use supply is not a patient-specific prescription
The Virginia Board of Pharmacy says a permitted pharmacy may provide certain non-compounded prescription drugs to a practitioner authorized to possess them for office use, subject to small-quantity and federal-law conditions. The Board also explains that a prescription marked “For Office Use Only” is not a valid patient-specific prescription. Ohio: furnishing carries labeling and recordkeeping duties
Ohio Revised Code section 4729.291 requires a licensed health professional who personally furnishes drugs to a patient under the cited authority to ensure lawful labeling and packaging and to keep purchase and disposition records. Ohio Revised Code, section 4729.291.

- Patient choice and access: Ask which pharmacy the patient wants to use and whether distance or other access barriers affect the choice.
- Timing and continuity: Consider how quickly the patient needs the medicine and how the workflow supports ongoing therapy.
- Coverage and availability: Check the patient’s plan requirements and whether the medicine is available through the proposed channel.
- Medication suitability: For mail order, consider shipping time and stability as well as the patient’s circumstances.
- Counseling and follow-up: Decide how the patient will get relevant instructions and where to ask questions after filling.
- Compliance workload: Assess the practice’s obligations for authority, registration, storage, labeling, records, and any applicable restrictions.
The available guidance establishes patient-choice and compliance considerations, but does not establish comparative cost, adherence, safety, or access outcomes for these alternatives. Do not describe one as cheaper or clinically superior without evidence specific to the medicine, patient, and plan.
Health-related products are a separate ethics question
AMA guidance treats physician sales of health-related products as a potential financial conflict that can affect patient trust. It says claims should be scientifically supported, financial interests disclosed, and sales generally limited to immediate and pressing patient needs; it also advises against exclusive arrangements that restrict broader patient access. This guidance concerns health-related products and should not be confused with the legal rules for dispensing prescription drugs.
