Decaf is not caffeine-free: the U.S. Food and Drug Administration (FDA) says an 8-fluid-ounce cup typically contains 2–15 mg. The amount varies by product, serving size, and preparation, so count caffeine from all sources—not just coffee.
Does decaf coffee have caffeine?
Yes. The FDA says decaffeinated coffee typically contains 2–15 mg of caffeine per 8-fluid-ounce cup. That is a typical range, not a guaranteed amount for every coffee. Larger servings and different products or preparation methods can contribute different amounts.
Decaf therefore reduces caffeine exposure compared with regular coffee, but it does not reduce it to zero. FDA guidance does not establish that a particular decaffeination method makes coffee safer during pregnancy.
How much caffeine can you have during pregnancy?
ACOG recommends keeping total caffeine intake below 200 mg a day. Its patient guidance, published and last reviewed in October 2020, says research suggests moderate consumption below that amount does not cause miscarriage or preterm birth. ACOG describes 200 mg as the amount in one 12-ounce cup of coffee, but coffee caffeine content varies; this is a rough reference, not a reliable conversion for every cup.

The limit applies to caffeine from all sources combined. ACOG specifically names tea, chocolate, energy drinks, and soft drinks as sources to include along with coffee. Some medicines may also contain caffeine, so check the label or ask a pharmacist or clinician if you are unsure.
Does decaf count toward the 200 mg limit?
Yes. Since decaf still contains caffeine, include it in your daily total. A typical 8-ounce decaf coffee contributes a small fraction of the 200 mg limit, but the exact amount depends on the product and serving. Do not assume a large mug or café drink matches the FDA’s 8-ounce example.

For a specific product, check its package or ask the café, manufacturer, or seller about the serving’s caffeine amount. The FDA notes that caffeine content and container sizes vary and directs consumers with product questions to the manufacturer or seller.
What the pregnancy guidance does—and does not—say
ACOG’s guidance is not a promise that caffeine poses zero risk at every dose. Its 2010 Committee Opinion said moderate caffeine consumption below 200 mg per day did not appear to be a major contributing factor in miscarriage or preterm birth, while the relationship between caffeine and fetal growth restriction remained undetermined. It also said no final conclusion could be made about high caffeine intake and miscarriage.

In that 2010 opinion, ACOG reported equivocal findings for intrauterine growth restriction: compared with intake below 100 mg per day, reported odds ratios were 1.2 for 100–199 mg per day, 1.5 for 200–299 mg per day, and 1.4 for more than 300 mg per day. These figures do not show that caffeine caused growth restriction; ACOG characterized the evidence as uncertain. The opinion is older, so it is useful for understanding the limits of the evidence rather than as a newer recommendation.
Caffeine can also affect how you feel. ACOG notes that it may interfere with sleep and contribute to nausea or light-headedness. If decaf worsens symptoms or you have a particular concern, discuss your intake with your clinician.

Practical ways to keep track
- Use the actual serving size, not the word “cup,” when estimating intake. FDA’s decaf range is for 8 fluid ounces.
- Include all coffee, tea, chocolate, energy drinks, soft drinks, and any relevant medicines in your daily total.
- For café drinks or products without a stated caffeine amount, ask the seller or manufacturer rather than assuming the amount is the same as the FDA’s typical decaf example.
- If you have a clinician-directed caffeine limit, significant caffeine sensitivity, a high-risk pregnancy, or a medically specific concern, follow your clinician’s individualized advice.
