There is no proven universal pre-run meal, hydration volume or toilet-spacing rule for IBS; use your own symptom patterns to guide choices and seek professional advice before making restrictive diet changes.
What the evidence says about running and IBS
Running does not affect everyone with IBS in the same way. In a 2023 study, runners with IBS reported more gastrointestinal symptom severity than control runners both at rest and during running. Within the IBS group, however, the median total symptom-severity score was 150 at rest and 118 during or after running. The authors reported a statistically significant reduction and concluded that moderate-intensity running could benefit symptoms, but the findings do not mean running will relieve symptoms for an individual. The American College of Gastroenterology’s 2018 monograph also suggests exercise for overall IBS symptom improvement, while rating the recommendation weak and the supporting evidence very low quality. Treat exercise as a potentially helpful part of a routine, not an IBS treatment guarantee. See the ACG monograph.
How to plan meals before a run
Keep a steady routine rather than experimenting on run day
NICE advises people with IBS to eat regular meals, take time while eating, avoid skipping meals and avoid long gaps between meals. This is general IBS guidance, not a prescribed pre-run fueling schedule. NICE does not establish a universal number of minutes or hours to leave between eating and running, and the sources here do not establish one ideal pre-run meal. See NICE recommendations for IBS.

Instead of copying a runner’s meal plan, keep brief notes on what and when you ate, the run’s intensity and duration, and any symptoms. Over time, this may help you notice personal patterns without assuming that one food or timing rule applies to everyone. Avoid making a last-minute change to a familiar routine before a run you cannot easily shorten.
Do not borrow another runner’s food exclusions
A 2021 study of runners with IBS or inflammatory bowel disease (IBD) found that some participants reported avoiding milk products (53%), legumes (37%) and meat (31%) before races, while also selecting grains containing gluten (40%) and high-FODMAP fruits (38%). These figures describe that study’s combined IBS/IBD sample, not all runners with IBS, and they do not establish a universal list of trigger foods. Get help before trying restrictive diets
If symptoms persist despite general lifestyle and dietary advice, NICE says further dietary management—including avoiding individual foods or following a low-FODMAP diet—should be advised by a health professional with expertise in dietary management. The American Gastroenterological Association (AGA) likewise supports individualized dietary advice and registered dietitian nutritionist involvement when collaboration or implementation support is needed. How to plan fluids without over- or underdoing it
NICE recommends at least 8 cups of fluid per day—approximately 2,000 ml in its quality-standard material—as general IBS advice, especially water or non-caffeinated drinks. This is a daily IBS guideline, not a target for a run or an exercise-fueling prescription. See NICE’s general IBS guidance.

For prolonged exercise, the sports nutrition position statement describes fluid replacement as an individualized choice: a planned approach or drinking according to thirst may suit different people and conditions. The sources do not establish one volume that is right for every runner. Consider the run’s length, weather, your own response and whether water will be available; both too little and too much fluid can be a concern. See the sports nutrition position statement.
- Carry water: Consider this if access to refills is limited and you prefer to bring your own supply. A belt with a soft flask is one optional way to carry it, not an IBS remedy.
- Use refill points: Check whether water is available along your route, and account for whether facilities will be open.
- Choose a shorter loop: A route that passes your starting point can make it easier to adjust or stop if conditions or symptoms change.
How to choose a route when urgency is a concern
There is no validated IBS-specific route-planning or toilet-spacing protocol in the sources reviewed. Route choices are practical accommodations, not clinical rules. Choose what fits your own needs and the places you run.

- Start with a familiar loop or out-and-back where you know how to return or cut the run short.
- Where available, check for public toilets and water access before setting out.
- If symptoms begin, use a route that offers a straightforward way to shorten or end the run rather than pressing on because the planned distance is fixed.
These options can make a run feel more manageable, but they cannot predict or prevent symptoms.

A registered dietitian with relevant IBS or sports-nutrition experience can help tailor changes to your symptoms and running routine. Do not assume new, severe or concerning symptoms are IBS or try to manage them only by changing a meal or route; seek medical assessment.
