Correlation does not prove causation. A correlation describes how two variables move together; causation means a change in one helps produce a change in the other. An observed relationship may reflect cause and effect, but it can also result from chance, a third factor, biased selection, faulty measurement, or other errors. To judge the claim, look beyond the pattern to timing, study design, uncertainty, and plausible alternative explanations.

What correlation and causation mean

Correlation describes a relationship

Correlation, or statistical association, describes whether variables vary together and how strong or large that relationship appears. In epidemiology, measures such as risk ratios and odds ratios quantify associations; which measure is appropriate depends on the study design. For example, the CDC identifies the odds ratio as the preferred association measure for case-control data. An association is descriptive: it does not, by itself, explain why the variables are related.

Causation is an explanatory claim

A causal claim says that changing an exposure produces a change in an outcome. An observed association can be interpreted as an effect only if the exposure is causally related to the outcome. That condition cannot be inferred from the association alone. The CDC’s Field Epidemiology Manual describes chance, bias, confounding, investigator error, and a true association as possible explanations for an observed relationship.

Why two variables can move together without one causing the other

A third factor may account for the pattern

Confounding occurs when a third factor distorts the apparent relationship between an exposure and an outcome. In a CDC example, manufacturing workers appear to have higher mortality, but their older average age could explain at least part of the difference. Age is a potential confounder when it is related to the outcome independently of the exposure and related to the exposure without being a consequence of it.

Chance, selection, and measurement can mislead

  • Chance: A pattern may arise from random variation in the data.
  • Selection bias: The way people enter or remain in a study can make the compared groups unrepresentative or systematically different.
  • Information bias and measurement error: Exposure or outcome data may be collected inaccurately or differently across groups.
  • Missing data or analysis choices: Incomplete information or errors in study conduct and analysis can distort the reported relationship.

These possibilities are why a relationship should be checked against how participants were selected, how variables were measured, and how the analysis was conducted—not just whether a statistical test produced a significant result.

How to assess a reported statistical relationship

  1. Identify what was measured. Find out how the exposure and outcome were defined, who was included, and how the association was expressed. Interpret a risk ratio, odds ratio, or other measure in light of the study design.
  2. Check the order of events. The proposed cause must precede the outcome. If the outcome came first, the proposed causal direction is untenable. Precedence is necessary for causation, but it does not prove it.
  3. Look for differences between the groups. Ask whether age or another factor could be related to both exposure and outcome. Consider whether the study measured and handled that factor well; adjustment cannot guarantee that all confounding is removed.
  4. Inspect selection and measurement. Consider who was recruited, who was excluded or lost to follow-up, how exposure and outcome were recorded, and whether missing data or measurement error could alter the comparison.
  5. Read the estimate with its uncertainty. Consider the effect size and confidence interval, not only a p-value or a “statistically significant” label. A confidence interval gives a range of values consistent with the data under the interval procedure. A small p-value addresses chance under the statistical test; it does not rule out confounding, bias, or design and analysis errors.
  6. Compare evidence across studies. Check whether the relationship appears in relevant populations and settings, and whether it is plausible given subject-matter knowledge. A dose-response pattern may add evidence, but neither consistency, plausibility, nor dose-response alone proves causality.

What study design can—and cannot—show

Observational studies document exposures as they occur; experiments assign an intervention or exposure. The CDC describes randomized controlled trials as the reference standard in epidemiology, but random assignment is not possible or ethical for many questions. Even a well-designed experiment must be assessed for how it was conducted and analyzed.

Evidence question Observational study Experiment
Who determines exposure? Researchers document exposure as it occurs. Researchers assign an intervention or exposure.
How is confounding addressed? Design, measurement, stratification, and adjustment can help; residual confounding may remain. Random assignment can balance factors on average, but conduct, adherence, loss to follow-up, measurement, and analysis still matter.
Does exposure precede outcome? Depends on sampling and follow-up; a cross-sectional association may not establish sequence. The study can be designed so assignment precedes measured outcomes.
Can it study ethically or practically difficult exposures? It can study exposures researchers cannot ethically or practically assign. Assignment may be infeasible or unethical for many exposures.
What conclusion does it support? An association is observed; a causal interpretation needs assumptions and supporting evidence. A well-designed and conducted experiment can provide stronger causal evidence, but does not automatically settle every question.

The distinction is about the strength and kind of evidence, not a simple rule that every experiment establishes causation or every observational finding is uninformative. The CDC’s Field Study Design chapter explains the role of experiments and observational studies in epidemiology.

What a scatter plot can tell you

A scatter plot can make the direction and strength of a relationship easier to see and help reveal outliers. It cannot show, by itself, whether one variable caused the other. As the CDC’s COVE guidance puts it: “Remember that scatter plots do not prove causation.” Treat a visible pattern as a reason to investigate, not as a causal verdict.

Statistical significance is not practical importance

A statistically significant result does not necessarily mean the association is large or consequential. Large studies can detect weak associations as statistically significant; small studies can fail to detect associations that may matter. Evaluate the estimated effect and its uncertainty in context rather than treating significance as a measure of importance or proof of cause and effect.

A practical standard for causal claims

Before accepting a claim that one thing causes another, ask whether the exposure came first; whether the groups differed in relevant ways; whether selection, measurement, missing data, or analysis could have distorted the result; and whether the size and uncertainty of the effect make it meaningful. Then consider whether the relationship is consistent and plausible in relevant evidence. These are checks that strengthen or weaken a causal interpretation, not a mechanical test that guarantees one.
