Needle exchange programs—often called syringe services programs (SSPs)—help prevent HIV by making sterile syringes and other injection supplies easier to obtain and used equipment safer to dispose of. Using a new sterile syringe and equipment for every injection reduces the chance of contact with another person’s blood. SSPs can also connect people with HIV testing, vaccination, medical care, and substance-use treatment.
How can sharing injection equipment spread HIV?
HIV can be transmitted when blood containing the virus remains on a syringe or other injection equipment and that equipment is reused by someone else. A sterile syringe and sterile equipment for each injection interrupt this route by reducing the chance of exposure to contaminated blood. The CDC identifies one-time use of sterile needles and syringes as the safest and most effective way to limit HIV transmission during drug injection (CDC guidance on HIV risk among people who inject drugs).
What does a syringe services program do?
An SSP is a community-based prevention program. Sterile supplies and safe disposal are central services, but the exact services and ways to access them differ by location. A program may operate at a fixed site, through mobile services or outreach, or through other local arrangements.
- Provides sterile supplies: Access to new syringes and injection equipment makes it more practical to use sterile supplies consistently.
- Accepts used equipment: Safe disposal helps reduce discarded syringes in the community and the risk of accidental needle sticks.
- Connects people with care: SSPs may offer or link participants to infectious-disease testing and vaccination, HIV care, and substance-use-disorder treatment.
CDC describes SSPs as a way to provide these services and link people to care (CDC: About Syringe Services Programs). Needs-based distribution—providing supplies according to participant need—is identified as a best practice in a CDC-authored review; overly restrictive access can make consistent use harder (Broz et al., review of SSP implementation and effectiveness).

Do needle exchange programs work?
Evidence supports a meaningful prevention benefit, but the figures summarize different kinds of evidence and are not promises of an individual outcome.

Program design and reach matter. The CDC-hosted review emphasizes broad, needs-based access and links to medications for substance use disorder and antiretroviral therapy as ways to strengthen a program’s impact. Peer outreach and telehealth can also extend access (Broz et al.).

What do surveillance figures say about access and sharing?
CDC’s national HIV risk, prevention, and testing surveillance page reports that 19% of the population covered by that surveillance reported receptive syringe sharing, and 37% reported receptive sharing of injection equipment. These figures describe that surveillance population; they are not universal estimates for every community. The same page reports that 57% received sterile syringes from SSPs, with reported city-level figures ranging from 3% to 85%—a wide variation that underscores the importance of local access (CDC surveillance: HIV risk, prevention, and testing behaviors among people who inject drugs).
Are SSPs safe, and do they increase drug use or crime?
CDC states that comprehensive SSPs are safe, effective, and cost-saving, and that they do not increase illegal drug use or crime (CDC: Infectious diseases in persons who inject drugs). Program availability, policies, and legal rules vary by location, so local public-health guidance is the best source for current access information.

What should you do to reduce your own risk?
- Use new, sterile equipment every time. Do not share syringes or other injection equipment. CDC advises using new, clean syringes and equipment for each injection (CDC: HIV prevention and injection drug use).
- Find local supplies and disposal options. Contact a local SSP or public-health department; some pharmacies also sell needles without a prescription. What is available depends on local rules and services.
- Do not rely on bleach as a substitute. CDC says disinfecting a shared syringe with bleach is less protective than using a new sterile syringe (CDC guidance).
- Act promptly after a possible recent exposure. If you may have been exposed to HIV within the past 72 hours, contact a healthcare provider right away and ask whether post-exposure prophylaxis (PEP) is appropriate (CDC guidance).
SSPs are one part of HIV prevention and care: access to sterile equipment reduces a specific route of transmission, while testing, treatment, and other prevention services address additional needs.
