Am I at risk of plague? For most people who have no relevant travel, flea, or animal exposure, plague is not an everyday risk. It still occurs in specific animal and flea settings, however, and an infected flea bite or contact with an infected animal can transmit it. A rare lung form can spread through infectious respiratory droplets. If you have a plausible exposure and compatible symptoms, seek urgent medical care.

What plague is—and where it occurs

Plague is an infection caused by the bacterium Yersinia pestis. It persists in animal and flea cycles, so it is not only a historical disease. Risk is geographically uneven: the U.S. Centers for Disease Control and Prevention (CDC) says plague occurs naturally in parts of the western United States and in certain regions of Africa and Asia. Being in the West does not mean every person or place has the same risk; local risk depends in part on disease activity among animals.

The World Health Organization (WHO) reports that most cases reported during 2019–2025 occurred in the Democratic Republic of the Congo and Madagascar. That describes the distribution of reported cases over that period, not an annual count or a measure of risk for every country.

How do people get plague?

The usual route is a bite from a flea infected with Y. pestis. People can also become infected through direct contact with an infected animal, including while handling it. The specific exposure matters more than a general fear of the disease: a person without relevant flea, animal, or travel exposure is not facing the same circumstances as someone who has encountered infected wildlife in an affected area.

Can plague spread between people?

Pneumonic plague affects the lungs and can spread from an infected person through infectious respiratory droplets. This is a real but specific exception; it is not a reason to treat plague as something that readily spreads through ordinary casual contact. CDC says person-to-person spread has not been documented in the United States since 1924. CDC also notes rare pneumonic cases associated with sick cats, so contact with a sick animal can warrant particular attention.

What forms of plague are there?

The forms differ in where the infection affects the body and how it matters for transmission. Bubonic plague involves lymph nodes; septicemic plague affects the bloodstream; pneumonic plague affects the lungs. The respiratory-droplet route applies to pneumonic plague, not to plague in general.

Plague can become severe, especially in septicemic and pneumonic forms. WHO says untreated bubonic plague has a case-fatality ratio of 30%–60%; it also warns that untreated septicemic or pneumonic plague can be fatal. Those statements concern untreated disease and should not be read as outcomes for people who receive prompt medical care.

What should you do after a possible exposure?

If you have a plausible flea or animal exposure and develop symptoms that could fit plague, seek urgent medical assessment. Tell the clinician when and where the exposure happened, whether you had a flea bite or animal contact, and any relevant travel history. Do not wait to see whether symptoms pass or try to treat the infection yourself.

CDC advises clinicians to begin treatment when plague is suspected rather than wait for confirmatory test results. Antibiotics can treat plague, but the choice of treatment and how it is given are clinical decisions based on factors such as severity. The important action for a reader is prompt professional assessment, not obtaining or taking antibiotics without medical direction.

How to reduce exposure in areas where plague occurs

  • Make the area less attractive to rodents. Reduce rodent habitat around homes and recreational areas.
  • Avoid direct contact with potentially infected animals. Wear gloves if handling or skinning an animal that may be infected. Ask local health officials how to dispose of a dead animal rather than handling it bare-handed.
  • Reduce flea bites. Use an appropriate repellent when activities could expose you to fleas. CDC identifies DEET products for skin and clothing and permethrin products for clothing; follow each product’s label.
  • Protect pets from fleas. In areas where plague occurs, use veterinary flea-control products. Pets that roam freely can encounter infected animals or fleas.

CDC says a plague vaccine is no longer available in the United States.
