The Federal Aviation Administration (FAA) required an investigation, then authorized Falcon 9 flights to resume on July 25 after determining that the anomaly posed no public-safety issue. The investigation could continue after that decision.
What happened on the July 11 Falcon 9 flight?
The Starlink Group 9-3 mission encountered a failure in Falcon 9’s upper stage while in space. The FAA’s incident record identifies an upper-stage failure; it was not the first-stage booster exploding or crashing. The agency reported no injuries to members of the public and no public property damage. FAA incident statement
Futurism reported that the Starlink satellites had been released but did not reach their intended orbit and were expected to re-enter Earth’s atmosphere and fully demise. Its account also described visible ice buildup before the failure. That observation is not confirmation of the failure’s root cause. Futurism’s July 15, 2024 report
Why were Falcon 9 rockets grounded?
The FAA required SpaceX to investigate the mishap. It said it would participate in every step and that the agency must approve SpaceX’s final report, including its corrective actions. The stated purpose was to establish the root cause, improve public safety, and identify measures to prevent a recurrence. FAA incident statement

At the time of Futurism’s July 15 report, Falcon 9 flights were grounded pending the investigation. That describes the situation then—not a continuing fleet-wide ban.
When did Falcon 9 flights resume?
The FAA approved a return to flight on July 25, 2024. SpaceX had requested a public-safety determination on July 15; after its review, the FAA concluded that no public-safety issues were involved in the Starlink Group 9-3 anomaly. The investigation remained open, and the approval did not remove other applicable launch-licensing requirements. FAA return-to-flight statement

Can a rocket return to flight before an investigation is finished?
Yes. FAA guidance describes two routes: the agency can accept a final mishap report and corrective actions before flights resume, or it can make a separate return-to-flight determination while the investigation continues. A return decision is about whether the agency finds the relevant systems, processes, or procedures safe for public operations; it does not necessarily mean every technical question has been resolved. FAA mishap guidance
In deciding whether an earlier return is appropriate, the FAA considers the nature and consequences of the mishap, any debris, the performance of safety-critical systems, and whether the safety analyses supporting the license remain valid.

Was ice the cause of the failure?
The available FAA statements confirm the upper-stage failure and the later safety determination, but do not establish a final technical cause. Futurism’s report of ice buildup is an observation, not a confirmed finding that icing caused the anomaly. It would therefore be inaccurate to present ice as the proven cause.
