Photos can make facial differences look more noticeable because each image captures one angle, distance, lighting setup and instant. A close selfie, slight head turn or unfamiliar left-right orientation can change what stands out; one photo cannot establish that your face is unusually asymmetrical. For a more consistent comparison, use greater camera distance, eye-level framing, even light and a relaxed, straight-on expression. Sudden facial weakness or drooping is different: seek urgent medical assessment.
Why a photo can make your face look different
A camera turns a three-dimensional, moving face into a flat image. That means the picture emphasizes only what was visible from that camera position at that moment—not a definitive version of your face.
Distance and perspective change proportions
When the camera is close, features nearer to it can look larger relative to features farther away. A slight turn can also bring one side of the face closer to the lens, making it appear more prominent. DermNet cautions that wide-angle settings can create geometric distortion in clinical images, including making the nose look too large. A 2023 study of smartphone and full-frame photography likewise found facial distortion varied with focal length; its abstract reports 12%–19% more vertical midface stretching at tested 24, 35 and 50 mm focal lengths than in its 60-inch full-frame reference setup. Pose, expression and lighting emphasize different details
A tilted or turned head changes which contours are visible. A still frame can also catch a brief, uneven expression that would not be noticeable while you are talking. Strong light from one side creates sharper shadows and highlights than soft, even light, changing how facial contours appear. Why your face may look different from the mirror
A mirror shows a reversed view of your face; a typical photo shows the opposite left-right orientation. Because faces are not perfectly symmetrical, reversal shifts which side’s features appear where. The unfamiliar arrangement can make small differences stand out—not because the photo proves that one version is more accurate or more flawed. Neither a mirror nor one photograph is a complete stand-in for how others encounter you. A mirror is familiar but reversed; a photo preserves one orientation but freezes one view; people generally see you at ordinary conversational distance, in three dimensions and while you move. A small 2014 study of people with peripheral facial palsy found that patients with left-sided palsy selected mirror images as most attractive in 90% of cases in that subgroup. How to take a more consistent comparison photo
- Increase camera distance. Move farther away than a close arm’s-length selfie and use ordinary optical framing or crop afterward where possible. Distance changes perspective; no particular phone lens guarantees a perfectly true likeness. The 2023 study’s comparison setup and DermNet’s guidance discuss image distortion.
- Keep the camera near eye level and face it squarely. Use the same head position each time, with a relaxed, neutral expression. Small position changes can alter facial appearance, which is why clinical photography uses standardized capture. Clinical photography guidance.
- Choose soft, even light. Face a window or use another broad, gentle light source rather than a strong light from one side. The AAD recommends natural window light for skin photographs. AAD photo guidance.
- Compare like with like. If you want to track an appearance over time, take multiple images under similar conditions rather than judging from one flattering or unflattering frame. For documenting a skin concern for a dermatologist, the AAD recommends photographs from different perspectives and focusing on the area of concern. Keep medical images secure. AAD photo guidance.
When facial asymmetry needs medical attention
Longstanding differences in appearance are not the same as a new loss of facial movement. Sudden weakness or drooping on one side—including a drooping eyebrow or mouth—needs urgent medical assessment. The cause cannot be determined from a selfie, and you should not try to distinguish Bell’s palsy from stroke or another cause by comparing photos. If a change is persistent, worsening, painful or accompanied by altered facial movement, seek a clinician’s assessment rather than relying on repeated photo comparisons. Clinical images are standardized because capture conditions can change what is visible; a casual photo is not a diagnosis.
