If a Diablo IV World Boss gives you no gear, or you cannot use the Heavenly Forge, first separate the two systems: Essence of Hellfire rewards Lair Boss Keys for surviving World Boss encounters; Sanctification happens at the Heavenly Forge and requires a Heavenly Sigil plus an eligible item. The Hellfire key reward is not a promise of a gear drop.
First, identify which reward or feature is missing
The wording “Hellfire World Boss loot” can point to two different Diablo IV mechanics. Blizzard’s 2.5.0 PTR overview describes Essence of Hellfire as a Divine Gift that awards Lair Boss Keys for surviving World Boss encounters, with the key reward doubled while the gift is Purified. Blizzard separately describes Heavenly Sigils as the items used to travel to the Heavenly Forge for Sanctification. Those descriptions do not say World Boss gear is forged at the Heavenly Forge.
- No expected reward after a World Boss: check whether you expected Lair Boss Keys from Essence of Hellfire or a gear item. The cited Hellfire description supports the key reward, not guaranteed equipment.
- Cannot reach or use the Heavenly Forge: check for a Heavenly Sigil and whether your item is eligible for Sanctification.
If Essence of Hellfire did not award Lair Boss Keys
Blizzard’s 2.5.0 PTR overview says Essence of Hellfire awards Lair Boss Keys for surviving World Boss encounters. If that is the reward you expected, check the Divine Gift and reward UI after the encounter and confirm that you survived it. The overview says the reward is doubled while Essence of Hellfire is Purified; it does not establish that every World Boss awards gear or that a particular item drop is guaranteed. Because this is a PTR description, check the live in-game UI and current patch notes for the behavior on your build.
Blizzard has announced Azmodan as a permanent World Boss who can appear among the bosses invading Sanctuary. Confirm the encounter and reward details in game rather than assuming every boss encounter has identical rewards. See Blizzard’s Season of Divine Intervention announcement.

If you cannot use the Heavenly Forge or Sanctify an item
Check for a Heavenly Sigil
Heavenly Sigils are used to travel to the Heavenly Forge. Blizzard’s Season of Divine Intervention announcement says they are earned in Torment difficulties. If you do not have a Sigil, you may not be able to access the Forge even if you have an item you want to Sanctify. See the season announcement for the system description.
Check the item’s current eligibility
Blizzard’s patch 2.5 notes specify that an item must have item power 750 or higher in Torment difficulties to be Sanctified. This is a version-specific rule, so inspect the item’s current in-game tooltip and check the patch notes for your live build before treating it as universal. The Diablo IV patch notes also document changes and fixes to these systems; they do not establish one universal bug or fix for every Forge problem.

Use this troubleshooting order
- Confirm the game and encounter. The Hellfire and Heavenly Forge clues point to Diablo IV. Blizzard also has a Support page about loot from older World of Warcraft expansion bosses, but its guidance is for WoW and should not be applied to Diablo IV. If you meant WoW, use the relevant game-specific support guidance instead.
- Name the reward you expected. For Essence of Hellfire, look for Lair Boss Keys and confirm you survived the World Boss encounter. Do not interpret that key reward as guaranteed gear.
- For Sanctification, verify access and eligibility. Check whether you have a Heavenly Sigil, then read the item’s current Sanctification tooltip and compare it with the live patch notes.
- Check the live Divine Gift and reward UI, then current patch notes. Blizzard’s published notes describe system changes, but the cited sources do not confirm a current, universal bug matching “loot won’t drop or forge.”
Do not apply World of Warcraft loot rules to Diablo IV
Blizzard Support’s article about older World of Warcraft expansion bosses says item loot is not guaranteed and that most such lockouts are weekly. Those statements concern WoW, not Diablo IV’s Essence of Hellfire or Heavenly Forge. See the Blizzard Support article only if you are troubleshooting that WoW encounter.
