Treat its answer as a useful draft, not proof: make your request specific, then verify consequential claims against reliable sources yourself. OpenAI’s guidance describes these risks but does not establish one general error rate; accuracy depends on the question and context.
Why does ChatGPT give wrong answers?
ChatGPT generates responses from patterns learned during training. OpenAI describes those information sources as publicly available internet material, information accessed through third parties, and information provided or generated by users, human trainers, and researchers. Models learn patterns rather than keeping copies of training examples, and the same question can produce different answers on different attempts. Generating a plausible answer is not the same as checking it against a current, authoritative source. OpenAI cautions that ChatGPT can produce incorrect facts and definitions, fabricated quotes, studies, or references, and overconfident answers to ambiguous or complex questions. A polished tone is not evidence that a claim has been verified. OpenAI’s guidance on whether ChatGPT tells the truth describes these limitations.
There is no general error-rate figure established by that guidance. A single percentage would hide important differences between tasks and claims, so assess the specific answer rather than assuming a universal likelihood that it is right or wrong.

How can I reduce wrong or irrelevant answers?
A clearer prompt can make an answer more relevant to your needs, but it cannot guarantee factual accuracy. OpenAI recommends stating the request clearly and specifically, then refining it as needed. OpenAI’s prompt-engineering guidance recommends this iterative approach.
- State the task and context. Explain what you need, who the answer is for, and any relevant background.
- Add constraints that change the answer. Specify the date, jurisdiction, scope, definitions, or other requirements that matter. Ask for a particular format if you need one.
- Surface missing information. If the answer depends on facts you have not supplied, ask ChatGPT to identify what it needs rather than silently filling gaps.
- Refine the prompt based on the miss. If the reply is too broad, vague, or off target, identify the problem and add context or narrow the request.
For example, instead of asking “What are the rules?”, specify which jurisdiction and date you mean and what kind of rules you need. That narrows the scope; you still need to check whether the response is correct.

How should I check a ChatGPT answer?
For consequential information, verify the claim outside the conversation. OpenAI advises checking important information and inspecting external references directly when precision matters. Its accuracy guidance warns that references, quotes, and studies can be fabricated; its advice on verifying ChatGPT answers also points users toward reliable sources.
- Check that each cited source exists. Follow the link or search for the publication yourself; citation-shaped text is not proof that a source is real.
- Confirm the source supports the exact claim. A real page can still be irrelevant, outdated, or weaker than the answer suggests.
- Verify high-impact details with appropriate sources. Check dates, figures, quotations, technical specifications, and legal, medical, or financial claims against relevant primary or qualified sources.
- Inspect calculations and structured results. When using data-analysis tools, review the inputs and method as well as the result, especially if a mistake would matter.
Repeating the question or asking for another version is not independent confirmation: OpenAI notes that responses can vary across queries. The same question may yield different answers, so compare claims with outside evidence rather than relying on consistency between replies.

When should I use Search, Deep Research, or data analysis?
Available tools can make some work easier, but they do not make an answer self-verifying. Access varies, and features can change. OpenAI describes Search as a way to look up current or niche information and return answers with citations; Deep Research can compile cited answers from multiple sources; and data analysis can assist with calculations and structured logic.
