Cerebrospinal fluid (CSF) analysis remains useful in neurological diagnosis, and US neurology training guidance explicitly includes supervised LP performance. That supports maintaining the skill in training; it does not prove that an “empowerment” program improves outcomes or reduces delays or referrals.
Why should neurologists learn bedside lumbar puncture?
LP provides a route to collect CSF for neurological diagnosis, including evaluation of central nervous system infection and neurodegenerative disease. When indicated, it can also measure CSF pressure. The 2017 neurological consensus guideline describes LP as important despite advances in neuroimaging. These uses make the procedure relevant to neurology practice and education.
Training guidance makes the educational case more specific. The ACGME Neurology Supplemental Guide places LP within procedural knowledge, including technique, indications, contraindications, and complications. Its milestone language says: “Performs lumbar puncture using appropriate technique with faculty member at bedside.” The emphasis is supervised performance, not an expectation that every clinician perform every LP independently.
What does bedside LP competence involve?
Competence extends beyond needle placement. A safe training pathway should build the ability to make and carry out the whole clinical decision, with the exact curriculum and credentialing requirements set locally.

- Assess the indication and the patient’s condition, including potential raised intracranial pressure or mass effect, bleeding risk and relevant medication, infection at the puncture site, and instability.
- Explain the procedure, obtain consent, and identify when imaging, deferral, or specialist input is appropriate.
- Prepare sterile equipment, position the patient, identify landmarks, and perform the procedure under appropriate supervision.
- Collect and label CSF correctly, recognize complications, and escalate when attempts are unsuccessful or risk is elevated.
Simulation can provide practice before supervised patient procedures, but the guidance cited here does not compare simulation-first training with other approaches or establish which curriculum produces better outcomes.
When should an LP be delayed or preceded by imaging?
Patient selection is a core part of the procedure. The 2017 neurological consensus guidance recommends clinical assessment and brain imaging when mass effect, abnormal intracranial pressure, or tonsillar herniation is suspected, including specified concerning clinical circumstances. This is not a universal instruction to image every patient before LP; the decision depends on clinical suspicion and risk factors.

NICE guidance for bacterial meningitis recommends LP before antibiotics unless the procedure is unsafe or would cause clinically significant delay. It advises stabilizing an unprotected airway, respiratory compromise, shock, uncontrolled seizures, or bleeding risk before LP. NICE also lists signs that can indicate raised intracranial pressure and directs clinicians to its imaging recommendation.
WHO guidance identifies cerebral herniation in the setting of raised intracranial pressure as the most serious, rare complication. It lists bleeding disorders, local infection, and hemodynamic or respiratory compromise as reasons to address the problem before proceeding. The practical lesson for trainees is to recognize risk and seek senior guidance rather than treat bedside access as a reason to bypass clinical assessment.

How should clinicians handle antithrombotic medication?
Antithrombotic decisions are time-sensitive and individualized. The UK Association of British Neurologists’ August 2026 guideline update says clinicians should balance bleeding risk, thrombosis risk, and the potential harm of delaying LP. It identifies urgent indications including strong suspicion of bacterial meningitis after initial immediate antibiotics, viral encephalitis, and CNS vasculitis. Drug-specific timing and laboratory thresholds should follow current local guidance and specialist advice; no single threshold should be applied to every patient.
Which procedural safeguards matter?
The neurological consensus guideline recommends atraumatic needles and sterile practice, including sterile gloves and thorough skin disinfection. It describes entry below the spinal cord, with L3–L4 or L4–L5 as typical levels. These are principles for supervised clinical practice, not a substitute for hands-on training or local protocol.

Attempts matter for patient comfort. In evidence cited by the 2017 consensus guideline, the odds of post-LP back pain were higher after 2–4 attempts (odds ratio 2.1; 95% CI 1.7–2.7) and after 5 or more attempts (odds ratio 5.4; 95% CI 2.9–10.2), compared with the study’s reference group. These are population-level associations, not predictions of an individual patient’s risk. A trainee should seek help when difficulty or repeated attempts make escalation appropriate.

It does not establish that a specific bedside “empowerment” initiative improves patient outcomes, shortens waiting times, or reduces radiology referrals. The defensible goal is therefore supervised, safety-conscious competence: clinicians who can identify an appropriate patient, perform the procedure within their training and local rules, and recognize when to defer or escalate.
