AI image generators struggle with hands because they must turn a complicated, moving three-dimensional structure into a coherent two-dimensional image. Fingers can bend, cross, disappear behind one another, or grip objects; a small mistake in shape or visibility can make the whole hand look wrong. The result may have too many or too few fingers, odd proportions, or an implausible pose—but that does not mean every generated hand is flawed.
What tends to go wrong
Hand errors are broader than simply miscounting fingers. Researchers have documented malformed shapes and proportions, missing or extra digits, awkward configurations, and fingers oriented in physically implausible ways. A 2024 study evaluated 240 photorealistic images generated by DALL-E 3, Stable Diffusion XL, and Stable Cascade from 10 prompts, with eight images per prompt. Its error categories included proportion, extra-part, orientation, configuration, and missing-part errors. The sample illustrates possible failure types; it does not establish a universal error rate for those systems or for image generators generally. The study, published in PLOS ONE, includes an example of a hand with six fingers.
Why hands are unusually difficult to depict
A hand has many possible shapes and poses
A hand is a three-dimensional structure with many joints and possible articulations. The generator has to infer its shape and pose from image patterns, then depict them in a flat image. The challenge grows when fingers are partly hidden, overlap, or cross: the image must still make clear which finger belongs where and which one is in front. HandRefiner identifies hand structure and finger occlusion as challenges for generation and refinement.
Objects and gestures add relationships to get right
A hand holding a camera or knife is not just a hand plus an object: the fingers must wrap around it in a plausible way, and the object must appear in the right position relative to the palm. Gestures and interlaced hands add further pose and overlap constraints. In a BBC Science Focus interview published February 4, 2023, University College London computer scientist and author Professor Peter Bentley described this problem of context. He recalled asking DALL-E for interlaced hands and getting an image that looked like “two wrists and a ball of fingers.” That is an interview example, not a controlled comparison of prompts.

Recognizing a hand is not the same as modeling its anatomy
Bentley described image generators as lacking an understanding of a hand’s three-dimensional geometry. As he put it, “They’ve got the hang of the general idea of a hand. It has a palm, fingers and nails but none of these models actually understand what the full thing is.” This helps explain why an image can look hand-like at a glance but fail when examined for anatomy, pose, or how the fingers interact with something.
What research is doing about it
Researchers are exploring approaches that provide more structure than a text prompt alone. The methods differ in when they intervene and what they try to control:

These are research approaches, not evidence that a particular consumer generator currently offers the same controls. The cited work does not provide a fair, current comparison of consumer products or show that changing a prompt reliably fixes hand anatomy. When assessing an actual tool, distinguish pose controls built into generation from editing an existing image; also check whether its claimed hand improvements have been evaluated specifically on anatomy.
When generated hands need expert review
For illustration or creative work, a malformed hand may be a quality problem. For medical or educational imagery, it can mislead. A study of hand-surgery patient-education images found limitations in anatomical realism and fabricated anatomy. That study concerns patient-education use; it should not be taken as an assessment of every creative use of generated images. Medical visuals should be reviewed by qualified professionals before they are used to teach patients or explain anatomy.

What the evidence does—and does not—show
The cited studies document recurring kinds of hand errors and investigate ways to guide or repair generated images. They do not establish a single failure rate for AI image generators, show that all current models behave alike, or rank the latest consumer tools. Performance can vary by system and image, so a familiar example of extra fingers is not a complete explanation of the problem: shape, pose, visibility, and interaction matter too.
