Use a prescribed epinephrine auto-injector as soon as you suspect anaphylaxis or a serious allergic reaction is beginning. Do not wait for symptoms to worsen or for hives to appear: a life-threatening reaction can occur without skin symptoms. Follow the person’s written action plan and the instructions for their specific device, then get emergency medical guidance.

How to recognize a severe allergic reaction

Anaphylaxis is a serious allergic reaction that can develop quickly and affect the skin, airway, breathing, circulation, stomach or intestines, and nervous system. Signs can include:

  • Difficulty breathing, wheezing, a hoarse voice, or a feeling that the throat is closing
  • Swelling of the tongue, face, or mouth; widespread hives; or severe itching
  • A weak pulse, dizziness, fainting, or loss of consciousness
  • Vomiting, diarrhea, or abdominal pain
  • Confusion, seizures, or unusual anxiety

These examples are not a checklist that must be completed before acting. A severe reaction may happen without hives or other skin signs. The American College of Allergy, Asthma & Immunology (ACAAI) says that 10–20 percent of life-threatening severe allergic reactions have no skin symptoms; the page does not specify a year for that estimate. ACAAI’s epinephrine auto-injector guidance advises people with a history of severe reactions to use epinephrine as soon as they suspect allergen exposure or feel a reaction starting.

When to use the auto-injector

Use the person’s prescribed device promptly when anaphylaxis is suspected or a serious allergic reaction is beginning. Do not wait to see if symptoms become more severe. The FDA patient information for EpiPen and EpiPen Jr says to use the device “right away” for an allergic emergency. MedlinePlus similarly advises injecting as soon as a serious allergic reaction is suspected. Follow the individual’s written action plan if available; when in doubt about a suspected severe reaction, do not delay the prescribed rescue treatment while seeking certainty.

What to do after using it

  1. Use the device as its instructions direct. MedlinePlus describes the general injection site as the middle of the outer thigh and says an injection can be given through clothing in an emergency. Device technique differs, so the product leaflet and the person’s training take precedence.
  2. Get emergency medical guidance. FDA patient information says to get emergency help for further treatment if needed, and MedlinePlus advises immediate emergency medical treatment after injection. Follow the action plan and local emergency guidance.
  3. Watch for persistent, worsening, or returning symptoms. Symptoms can continue or recur. FDA labeling says a second prescribed device may be used if symptoms do not improve, worsen, or return; it recommends immediate access to two devices. More than two doses should be given only by a healthcare provider.
  4. Rest quietly while waiting for care. MedlinePlus recommends resting quietly while waiting for emergency medical treatment.

There is a limited nuance about automatically calling emergency medical services (EMS): the American Academy of Allergy, Asthma & Immunology’s summary of the 2023 practice parameter says EMS may not be required if the response is prompt, complete, and durable and additional auto-injectors are available. That exception is not a reason to stay home when symptoms are severe, fail to resolve promptly and completely or nearly completely, return, or worsen. The person’s written plan and local medical guidance should govern the decision.

Prepare the person and caregivers

Ask a doctor or pharmacist to demonstrate the specific device for the person who may need it and for caregivers. A medication-free epinephrine auto-injector trainer can help people practice, but it contains no medication and cannot treat a reaction. Make sure caregivers know where the actual prescribed devices are kept and how to use them. FDA EpiPen labeling recommends carrying two devices and replacing a used one so that two are available again.

Dosage and weight labeling are product-specific. For example, the 2026 U.S. FDA label lists EpiPen 0.3 mg for people weighing 66 lb (30 kg) or more and EpiPen Jr 0.15 mg for approximately 33–66 lb (15–30 kg). Do not apply those thresholds to another product; use the prescribed device and its labeling.
