A short-lived change in your poo can follow a change in diet, routine, hydration, travel or medication. But an unfamiliar change that persists or returns, blood in your stool, unexplained weight loss or pain should be discussed with a clinician. Black or dark-red stool and bloody diarrhea need urgent advice under NHS guidance; bleeding that will not stop, a lot of blood or large clots need emergency care.

Use your usual bowel pattern as the baseline: frequency, stool consistency and how difficult it is to pass all matter. The right level of care depends on what has changed, how long it lasts, other symptoms and your personal circumstances. The service names below are specific to the country or health system identified.

What counts as a change in bowel habits?

A change is a noticeable difference from your own usual pattern. It may mean going more or less often, having softer or looser stools, or becoming constipated. People do not all have the same normal frequency, so a single number cannot describe everyone’s baseline.

For reference, the US National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) describes diarrhea as loose, watery stools three or more times a day, or more often than is normal for you. That is a symptom description, not a threshold for when to seek care. NIDDK lists fewer than three bowel movements a week, hard or lumpy stools, painful or difficult passage, and feeling that you have not fully emptied your bowels among constipation symptoms. These descriptions help explain symptoms; they do not diagnose their cause.

When should you arrange a routine medical appointment?

Arrange a GP appointment in the UK for an unfamiliar change that persists, or for bowel symptoms that concern you. NHS guidance says to seek a check for symptoms such as a persistent change in bowel habit, blood in your poo, tummy pain, bloating, a lump, unexplained weight loss, or fatigue and shortness of breath that may be related to anemia. These symptoms are common and do not by themselves mean cancer, but they deserve assessment. See the NHS guidance on bowel cancer symptoms.

In Northern Ireland, nidirect advises speaking with a GP if symptoms persist and returning if they persist or recur after treatment stops, regardless of their severity or your age. A short-lived change may have an everyday explanation, but persistent or recurring symptoms should not be dismissed. When do stool or bowel symptoms need urgent advice?

Blood or dark stool

Under NHS guidance in the UK, black or dark-red stool, or bloody diarrhea, calls for an urgent GP appointment or NHS 111 advice. The correct service may differ outside the UK, so contact your local urgent-care service. Do not assume that a symptom is harmless because it happens only once.

Diarrhea with concerning symptoms

NIDDK advises contacting a doctor right away for diarrhea with severe abdominal or rectal pain, frequent vomiting, red blood or pus in stool, black tarry stool, altered mental state, or signs of dehydration. For adults, it also advises prompt contact for diarrhea lasting more than two days, a high fever, or six or more loose stools in a day. These thresholds are US federal patient guidance; use the relevant local service where you live.

Dehydration can develop with diarrhea. NIDDK lists extreme thirst or dry mouth, urinating less, tiredness, dizziness or lightheadedness, dark urine, skin that does not quickly flatten after being pinched, and sunken eyes or cheeks as warning signs. Contact a clinician promptly if these occur.

Children and people at higher risk

For children, NIDDK advises prompt medical contact for diarrhea lasting more than one day, frequent loose stools, inability or refusal to drink, or fever in an infant or high fever in an older child. An infant under 12 months, a child born prematurely, or a child with other medical conditions may need particular care. Warning signs of dehydration in infants and young children include no wet diaper for three or more hours, absent tears, a sunken soft spot, or unusual drowsiness. Caregivers should seek pediatric advice promptly if these signs appear.

Pregnant people, adults over 65, people taking antibiotics and people with weakened immune systems are at higher risk of diarrhea complications, according to NIDDK. They should stay in contact with a doctor when diarrhea occurs.

When is bleeding or constipation an emergency?

NHS guidance says to call 999 or go to A&E in the UK for rectal bleeding that does not stop, a lot of blood, or large blood clots. Elsewhere, use your local emergency number or emergency department.

NIDDK says constipation accompanied by inability to pass gas, vomiting, constant abdominal pain or fever needs immediate medical review. If symptoms are severe or worsening quickly, seek emergency help through your local system rather than waiting for a routine appointment.

What can cause a temporary change?

Many short-term changes have common explanations, but symptoms alone cannot establish a cause. Constipation can be associated with changes in diet or routine, insufficient fluids, travel, pregnancy, aging, ignoring the urge to go, or medication changes. Medicines and supplements that NIDDK lists as possible contributors include iron, opioid pain medicines, some antidepressants, diuretics, calcium-channel blockers, some anticonvulsants, Parkinson’s medicines, anticholinergics, and antacids containing aluminum or calcium.

Acute diarrhea commonly results from viral gastroenteritis, food poisoning or medication side effects; antibiotics and magnesium-containing antacids are examples of medicines that may be involved. Persistent diarrhea can be associated with infections, food allergies or intolerances, digestive conditions, surgery or long-term medicines. Do not stop a prescribed medicine on your own; speak with the prescriber about a possible side effect.

What might a medical assessment involve?

A clinician may ask when the change began, what your stools are like, how often you go, what other symptoms you have, and about your general health, medical history and family history. An examination may include checking your abdomen and, in some cases, a rectal examination. The clinician may request a stool sample or blood test, or refer you for further tests.

A FIT (faecal immunochemical test) checks stool for blood. NICE’s UK suspected-cancer guidance recommends quantitative FIT to help guide referral for adults with a change in bowel habit and certain other presentations. This is a clinician-directed pathway, not a reason to self-order an at-home kit or a substitute for discussing symptoms. Needing a test or referral does not itself mean you have cancer. How to prepare for a conversation with a clinician

Before an appointment, note when the change started, whether it is improving or recurring, how often you are passing stool, and any changes in consistency or difficulty. Record accompanying symptoms such as pain, fever, blood, vomiting, weight loss or signs of dehydration. Bring a list of medicines and supplements, including recent changes. This information can help a clinician assess what is happening; it should not delay urgent or emergency care when warning signs are present.
