Check the execution tool’s result, then independently read back the file, setting, record, or application state the command was meant to change. If you cannot verify that state, report the outcome as unknown—not successful.
Why can an AI say a command succeeded when it didn’t?
A tool-using assistant operates through a handoff: the model proposes an action, the application dispatches it to a shell, browser, API, or other tool, that environment returns a result, and the model interprets the result before replying. A mismatch can occur at any point. The command might be malformed, never dispatched, fail during execution, or return an error that the integration drops or mislabels. The model may then describe the task as complete even though the available evidence does not support that claim.
The key distinction is between the assistant’s message and the machine’s result. A tool invocation shows that an action was requested; the tool’s returned status and output show what happened during that attempt. Neither alone necessarily proves that the user-visible change persisted.
How to check whether an AI agent actually ran a command
Follow the action from request to outcome, looking for the first point where the evidence breaks:

- Request: Inspect the command or tool arguments. Do they express the intended action and target?
- Dispatch: Confirm the application sent the request to the expected tool, rather than merely displaying or summarizing a proposed command.
- Execution: Inspect the raw tool result, including exit status, error details, timeout information, and partial output. OpenAI’s shell guidance recommends preserving non-zero exit output and returning timeout outcomes with partial output when available; converting either into a generic success hides important evidence.
- Interpretation: Check whether the assistant received the actual result and its error status. Anthropic’s tool-use guidance describes explicit error signaling and recommends useful error details rather than a vague failure message.
- Effect: Read the target state directly. Check the saved file, changed setting, created record, or updated application view. This independent postcondition check is what establishes whether the requested higher-level change actually happened.
- Report: Compare the final answer with what the evidence proves. If the execution result or state cannot be confirmed, the accurate status is unknown or incomplete.
A zero exit status can indicate that a process finished without reporting an error; it does not, by itself, establish that the intended application-level result persisted. Verify the relevant state separately.
How to troubleshoot a command that appears to succeed
- Reproduce only when safe. Before retrying, consider whether the command could send a duplicate message, create a second record, charge an account, or make an irreversible change.
- Inspect the exact invocation and raw result. Review arguments, output, exit status or error flag, timeout, and any partial output. Do not rely only on the assistant’s summary.
- Find the first failed step. In a trace or execution log, identify the earliest failure and check the command, working directory, authentication, dependencies, and service availability. VS Code’s troubleshooting guidance recommends identifying the exact failed command or tool and first error, then checking prerequisites and recovery options.
- Read back the intended state. Inspect the affected file, setting, record, or application view directly, using a suitable read-only check where possible.
- Choose whether to retry, recover, or stop. After any retry or recovery, check the state again. Starting a new agent session does not undo changes an earlier session may already have made.
- Give a precise status. Separate what is confirmed, what failed, and what remains uncertain. Say “the command was attempted, but I can’t verify the change” when that is what the evidence shows.
What to do when an AI action has several steps
Check each action and its dependencies in order. A later step may depend on a file, setting, or record created earlier, so a failure near the beginning can make the rest invalid. Anthropic’s computer-use guidance says to mark a failed action as an error and later dependent actions as not executed. It also directs implementers to run dependent actions sequentially and return a result for every requested action.

That distinction matters in the final report: “step two failed; steps three and four were not executed” is more useful and accurate than presenting the whole sequence as completed.
When traces help—and what they cannot prove
Tracing helps locate a broken handoff by connecting model activity with tool calls and returned results. OpenAI’s tracing documentation says: “The tracing dashboard shows what your agent did, including each step’s recorded inputs, outputs, duration, and status.” Its documentation describes command execution and web searches as tool spans.

Google Cloud’s observability documentation describes tracing agent reasoning, tool calls, and external interactions to diagnose failed requests, loops, and latency. The Microsoft Agent Framework observability documentation covers telemetry for framework activity. These sources describe coverage of parts of the workflow; they do not establish that any one tracing option is best for every team.
A trace can show that a tool returned success, but that is not necessarily proof that the desired real-world state persisted. For that, add or perform an application-specific read-back check. When evaluating an observability setup, look for whether it links the request, model response, invocation, and result; preserves status, errors, duration, and useful output; fits your framework and telemetry pipeline; and supports checks of the application state that matters.

Protect the data in traces
Detailed traces can contain prompts, responses, tool arguments, and results. Microsoft’s observability documentation identifies these as potentially sensitive data. Limit access, set appropriate retention, and redact sensitive values before enabling or sharing detailed traces.

The official documentation cited here explains tool lifecycles, error handling, troubleshooting, and observability, but it does not establish how often assistants report success when an action did not complete. Treat the mismatch as a practical failure mode to diagnose, not as a quantified rate.
