A missed call, an outdated number, or a message no one can explain is not just a failed contact: it is evidence that a notification path did not meet its objective. In the after-action report, identify where the process broke down, what that meant for the exercise, and who will correct it and arrange a retest.
What a call-down drill is meant to test
A notification drill tests more than whether someone placed a call. It can reveal whether assigned personnel know how to start the process, whether contact details are current, whether recipients understand the message, and whether the plan works as assigned.
For the Risk Management Program facility categories covered by its guidance, the U.S. Environmental Protection Agency (EPA) describes notification exercises as tests of whether personnel understand how to initiate the facility notification system, emergency contact information is current, and critical information is communicated clearly. EPA frames these requirements around specified facility categories and cites 40 CFR 68.96(a), 68.95(a)(1)(i), and 68.90(b)(3); applicability depends on the facility and its circumstances. More generally, FEMA describes exercises as a way to test and validate plans, procedures, and capabilities and identify gaps. Its exercise resources and Exercise Evaluation Guides offer ways to organize evaluation and collect information for after-action reporting. The right objective and evaluation criteria come from the plan and program you are exercising.

Record the failure precisely
Write the finding so a reader can tell what the plan expected, what the exercise observed, and how the difference affected the objective. Keep an unsuccessful attempt distinct from a completed notification: leaving a voicemail, reaching the wrong person, or failing to obtain the planned acknowledgment does not establish that the intended recipient was notified.
For each call or attempted contact, capture:
- The contact and role as listed in the plan.
- The name and role of the person who actually received the call, if anyone.
- The time of each attempt and any receipt or acknowledgment.
- Whether the message was understood and acknowledged using the plan’s method.
- The channel used, including any handoff or backup attempt.
- The obstacle observed and its effect on the exercise objective.
- The correction needed, the person responsible for it, a due date, and the planned retest.
EPA’s guidance specifically calls for documenting the contacts called, the recipient’s name, call time, relevant exercise notes, and required corrections. It also advises correcting copies of the emergency plan after the exercise. The assigned owner, due date, and retest fields above are practical ways to make follow-up trackable, not fields EPA identifies as mandatory. EPA notification exercise guidance
When calling external contacts for an exercise, make the drill status unmistakable. EPA gives this example for both the start and end of each call: “THIS IS AN EXERCISE. THIS IS AN EXERCISE. THIS IS AN EXERCISE.” Follow the procedures in your own plan to prevent confusion with an actual emergency. EPA notification exercise guidance

Turn the observation into a corrective action
A useful finding states the capability the exercise was meant to demonstrate, the observed result, and the gap between them. For example, if a backup contact receives the call but cannot identify the next step, record that the handoff did not achieve the planned notification objective. Do not infer a broader failure than the exercise evidence supports.
Choose a correction that addresses the observed cause. Depending on the plan and the finding, options may include:
- Verify the source of contact information and update the affected entry.
- Clarify who is authorized to initiate the call-down and designate backup coverage where the plan needs it.
- Revise an unclear sequence or specify what counts as acknowledgment.
- Test a secondary channel if the primary method failed or was unavailable.
- Update and distribute corrected copies of the plan or contact list.
Assign each action to a named role or person, set a due date, and specify how the affected path will be retested. FEMA’s drill-review material recommends an immediate post-exercise evaluation, discussion of what worked and what needs improvement, and an after-action report with improvement steps. Close the action only after the correction has been made and the relevant path has been retested. A successful retest is evidence that the identified issue was addressed; it does not guarantee that every future notification will succeed.
Choose a test that matches the communication path
For a communications drill in FEMA’s radiological emergency preparedness program, guidance calls for checking that communications hardware functions and that receiving organizations understand likely emergency messages. It recommends varying exercise times to include different work shifts. This is program-specific guidance, not a universal legal standard or frequency; use the criteria and schedule that apply to your own program. FEMA radiological emergency preparedness guidance
For other organizations, decide what the exercise needs to prove. If the objective is only contact-list accuracy, a controlled call sequence may be sufficient. If it includes message comprehension, acknowledgment, shift coverage, or hardware performance, design the exercise to observe those elements too. FEMA’s Alert and Warning Exercise Starter Kit includes sample materials for alert-and-warning procedures, including notice and no-notice scenarios. Use notice conditions appropriate to the plan, participants, and risk of confusion.

Use backup tools without confusing the responsibility
Manual calling, automated notifications, radios, and public alerting systems solve different operational problems. When selecting or evaluating a method, check who can initiate and authorize a message, how contact details are maintained, what acknowledgment is required, which audiences and channels it reaches, how failures escalate, and what evidence the drill can capture.
A backup radio may be useful when it fits the organization’s procedures and coverage. FEMA’s business emergency-planning guide recounts a drill that exposed a failed radio system and the absence of a backup radio, illustrating why alternate communications can matter. A radio does not correct stale contacts, unclear responsibility, or an untested escalation path. FEMA business emergency-planning guide

Keep strengths as well as gaps in the report
An after-action report should preserve what worked alongside what needs improvement. A recipient may have acknowledged promptly even though a later handoff failed; a secondary channel may have reached the right role despite an outdated primary number. Record such observations against the exercise objectives rather than reducing the report to a list of failures.
