There is no single food proven to restore gut health during antibiotics. First follow the instructions for your specific medicine: some antibiotics should be taken with food, while others need an empty stomach. Check the patient information leaflet or ask a pharmacist if you are unsure. In general, choose a varied diet you can tolerate; probiotic supplements may help some people, but they are not right for everyone.
Start with your antibiotic’s food instructions
Food advice is specific to the medicine, not a general rule for all antibiotics. The NHS guidance on antibiotic interactions says some antibiotics need to be taken with food and others on an empty stomach. Read the leaflet supplied with your prescription and ask a pharmacist or prescriber if it is unclear, or if you have questions about interactions with food or other medicines. The NHS page was last reviewed on 11 November 2022, so use your current medicine leaflet or a pharmacist to confirm instructions.
Do not skip doses, stop early, share antibiotics, or change how you take them because of a food or supplement suggestion. The CDC’s antibiotic-use guidance says to take antibiotics exactly as prescribed and contact a healthcare professional about side effects.
What to eat while you take antibiotics
Unless your medicine’s instructions say otherwise, eating a varied diet that you tolerate is a reasonable way to support your general nutrition during treatment. There is not enough evidence here to say that a particular food rebuilds the gut microbiome or prevents antibiotic-related digestive problems.

- Choose foods you normally tolerate and can comfortably eat.
- If you eat dairy, plain yogurt with live cultures is one optional food containing live microorganisms. It is not established as a substitute for a studied probiotic preparation or as a reliable way to prevent diarrhea.
- If dairy does not agree with you, there is no need to force yourself to eat yogurt. The evidence does not establish that one fermented food is necessary during a course of antibiotics.
Yogurt, fermented foods, and probiotic supplements
Probiotics are found in some foods, including yogurt and other fermented foods, and are also sold as supplements. However, evidence about a particular probiotic preparation cannot automatically be applied to any grocery yogurt or fermented food. Effects vary with the strain, product, patient group, antibiotic, and care setting.
The NIH Office of Dietary Supplements (ODS) professional fact sheet, accessed in 2026, reports that some probiotic strains may reduce antibiotic-associated diarrhea in certain studied groups. It describes evidence for preparations such as Lactobacillus rhamnosus GG (LGG) and Saccharomyces boulardii in particular study contexts; this is not an individualized recommendation for a product, strain, or dose. ODS says the potential benefit appears especially relevant when certain products are started within two days of the first antibiotic dose, but that does not mean every probiotic works for every person.

Evidence reviews suggest a possible benefit, not a guarantee. The National Center for Complementary and Integrative Health (NCCIH) summarizes a review of 17 studies involving 3,631 non-hospitalized participants: people taking probiotics with antibiotics were associated with about half the likelihood of antibiotic-associated diarrhea. NCCIH describes this conclusion as tentative because study quality was moderate. A 2021 BMJ Open Gastroenterology systematic review and meta-analysis of 42 randomized trials in 11,305 adults found a pooled 37% lower risk (RR 0.63; 95% CI 0.54–0.73), with moderate certainty. The authors cautioned that the results may not apply where baseline risk is low; in a sensitivity analysis excluding studies with high or unclear risk of bias, the estimate was no longer statistically significant.
ODS reports that up to 30% of antibiotic users experience antibiotic-associated diarrhea in the literature it reviews; this is not a prediction for every antibiotic or patient. These findings do not identify one best probiotic for everyone. NCCIH says that which products, doses, and patients benefit remains uncertain in several contexts.

Who should check with a clinician before taking probiotics?
Ask a healthcare provider or pharmacist before starting a probiotic supplement if you have a serious underlying condition, a weakened immune system, or take other medicines that could be relevant. NCCIH advises extra caution and close monitoring for people with serious health problems, and reports severe or fatal infections in premature infants given probiotics. Do not give probiotic supplements to a premature infant on the basis of general food advice.

Diarrhea is a recognized antibiotic side effect. The CDC notes that antibiotic use can also lead to C. difficile infection, a more serious problem; an article cannot determine the cause of an individual’s symptoms. Contact a healthcare professional about side effects, and seek timely advice for severe, persistent, or otherwise concerning diarrhea rather than trying to manage it with food alone.
