If a blood pressure reading is higher than usual, sit quietly and repeat it using proper technique, then record both results. One high reading alone is not an immediate cause for alarm. But if a repeat reading is above 180 systolic and/or 120 diastolic, contact a health professional immediately; call 911 if that reading comes with emergency symptoms such as chest pain or difficulty speaking.

First, check the number and how you feel

Blood pressure has two numbers: systolic, the upper number, and diastolic, the lower number. The American Heart Association (AHA) treats a reading above 180 systolic and/or 120 diastolic as severely high. The next step depends on whether a correctly repeated reading remains in that range and whether you have concerning symptoms.

A higher-than-usual reading below 180 systolic and 120 diastolic, with no emergency symptoms Sit quietly, take another reading correctly, and record both. If readings remain elevated or you are concerned, discuss them with a health professional.
Above 180 systolic and/or 120 diastolic, without emergency symptoms Wait at least one minute and measure again. If the repeat is still above either threshold, contact a health professional immediately.
Above 180 systolic and/or 120 diastolic with chest pain, shortness of breath, back pain, numbness, weakness, vision changes, or difficulty speaking Call 911. Do not wait to see whether the pressure falls on its own.

These US-oriented emergency instructions follow the AHA’s guidance on blood pressure readings. If you are outside the United States, use your local emergency number.

How to repeat a home reading accurately

Position and recent activity can affect a measurement. For the most useful repeat, use an automatic upper-arm cuff monitor and follow these steps:

  1. For 30 minutes beforehand, avoid smoking, caffeine, alcohol, and exercise when possible. Empty your bladder before measuring.
  2. Sit quietly for at least five minutes. Keep your back supported, feet flat on the floor, and legs uncrossed.
  3. Place the cuff on bare skin, using the size specified for your arm. Support your arm so the cuff is at heart level.
  4. Stay quiet and still during the measurement. Take two readings about one minute apart.
  5. Write down both readings and the time. If you are tracking blood pressure at home, follow your clinician’s instructions about how often to measure and bring the record to appointments.

The CDC’s measurement instructions explain how crossed legs, an unsupported arm below chest height, and nervousness can affect results. Ask a health professional to check that your monitor is accurate and its cuff fits properly; the AHA also recommends an automatic upper-arm cuff monitor.

What one high reading can—and cannot—tell you

A single measurement is a snapshot, not a diagnosis. Blood pressure can shift with recent activity, caffeine, alcohol, tobacco, nervousness, and measurement position. The AHA says one high reading is not an immediate cause for alarm, while MedlinePlus explains that clinicians usually diagnose hypertension using two or more high readings on different days, together with a person’s history and symptoms.

High blood pressure often has no warning signs, so feeling well does not rule it out. If readings continue to be higher than usual, share your log with a clinician; home monitoring can support care but does not replace regular visits.

Keep prescribed medication unchanged unless your clinician advises otherwise

Continue taking blood pressure medicine as prescribed. Do not stop it or change the dose based on a home reading without checking with the prescribing professional. A blood pressure number alone cannot establish the cause of a change or determine the right treatment.
