If a breach exposed only your name and address, save the notice, verify what was taken, and watch for signs of misuse. That exposure alone does not prove identity theft or automatically mean you need a credit freeze or paid monitoring. Your next steps depend on whether the notice also lists a password, financial details, or a Social Security number.
1. Check exactly what the breach exposed
Keep a copy of the breach notice. Note which information was involved, when the incident happened if a date is provided, and what the organization says it has done. A notice that lists only a name and address calls for a different response than one that includes a Social Security number, payment details, bank-account information, or login credentials.
If the notice is unexpected or you are unsure it is genuine, contact the organization using its known website or contact details—not links or phone numbers in a message you have not verified. The Federal Trade Commission’s breach checklist explains why steps should match the type of information exposed; the checklist is a PDF dated September 28, 2015: FTC data breach checklist.
2. Take action if other information was exposed
Password or online login
Change the password for the affected service and for any other account where you reused it. Use a distinct password for each account. Check the affected account for unfamiliar activity, especially if it stores payment information.

Payment-card details
Contact the bank or card issuer using the number on your card or its official website. Ask whether to cancel the affected card and get a replacement. Review transactions and update automatic payments if the card number changes.
Bank-account details
Contact your bank promptly and ask what protections it recommends. The FTC’s breach checklist advises asking the bank to close the affected account and open a new one, reviewing transactions, and updating automatic payments.

Social Security number
Check your credit reports, consider a credit freeze, and take steps to guard against tax identity theft. A freeze can make it harder for someone to open new credit in your name, but you may need to lift it when applying for credit or another service that checks your report. The FTC’s checklist discusses these steps specifically for Social Security number exposure, not as a default response to a name-and-address-only breach.
3. Watch for signs that someone is misusing your information
A breach notice is not proof that anyone has used your information. Pay attention to unfamiliar accounts or charges, unexplained withdrawals, or debt-collector calls about debts you do not recognize. If you see a warning sign, contact the affected company’s fraud department, ask it to close or freeze compromised accounts, change relevant passwords, and keep a record of dates, contacts, and case numbers.

For suspected identity theft in the United States, use the FTC’s IdentityTheft.gov recovery process. The FTC says its form can create an Identity Theft Report and a personalized recovery plan. Follow the steps for your situation and save the report and related correspondence.
4. Decide whether a fraud alert or credit freeze fits
These are free U.S. credit-report protections. They can help with new-credit fraud, but neither removes your name or address from the breached organization’s records, and neither prevents every kind of misuse. A name-and-address exposure alone does not make either one mandatory.

If a Social Security number or other sensitive financial information was exposed—or you have reason to suspect new-credit fraud—review your reports for accounts or inquiries you do not recognize. The FTC identifies AnnualCreditReport.com as the route to request free reports from the nationwide bureaus; its current recovery guidance says consumers can check weekly for free.

6. If you live outside the United States
The steps above describe U.S. FTC guidance and U.S. credit-report tools. Elsewhere, contact your country’s privacy regulator, identity-theft reporting body, bank or card issuer as appropriate, and local credit-reporting agencies. Names, rights, and procedures vary by country.
