AI-based MRI body-composition results can estimate amounts of fat and muscle in the scanned region, but they are not automatically a diagnosis or a personalized forecast. Ask what the system measured, how it was validated, and how the result fits your health history before drawing conclusions.
What did the AI measure on my MRI?
Some automated MRI systems estimate several distinct tissue compartments: subcutaneous adipose tissue (fat beneath the skin), visceral adipose tissue (fat around internal organs), skeletal muscle, skeletal-muscle fat fraction, and intramuscular adipose tissue. The measures depend on the model and the anatomy included in the scan; a whole-body estimate is not interchangeable with a measurement from one region or a single slice.
A 2026 study in Radiology used a fully automated deep-learning framework to estimate these five measures from UK Biobank and German National Cohort data collected between April 2014 and May 2022. Its reference curves were adjusted for age, sex, and height. The study calculated measures for 66,608 participants. Read the study in Radiology.
Ask your clinician:
- Which tissue compartments did this report include?
- Which MRI region or regions were analyzed, and was the result a whole-body volume, a regional measurement, or a single-slice estimate?
- Is the reported number an absolute value, percentile, or z-score?
What does the result mean, and is it a diagnosis?
A result may be a measurement or a risk marker, not a diagnosis. The 2026 study found associations between certain categories and later health outcomes. Compared with participants in the middle z-score category, those in the high visceral adipose tissue category had a hazard ratio of up to 2.26 for incident diabetes; the high intramuscular adipose tissue category had a hazard ratio of 1.54 for incident major adverse cardiovascular events; and the low skeletal muscle category had a hazard ratio of 1.44 for all-cause mortality.

These are adjusted estimates of relative hazard in a study population. They are not your absolute probability of developing a condition, and they do not establish that a measurement caused an outcome. Your clinician can explain whether the finding is clinically meaningful for you alongside your medical history, examination, symptoms, and other tests.
A separate prospective analysis of 11,348 German National Cohort participants without known pre-existing conditions found that higher paraspinal intermuscular adipose tissue was associated with higher odds of several cardiometabolic risk factors. That association does not show that the tissue measurement causes those risk factors. The 2026 Radiology report summarizes the broader study findings.

Ask directly: “Is this an established diagnosis for me, or a measurement that may indicate risk and needs context?”
Does the reference population fit my situation?
A percentile or z-score is meaningful only in relation to the reference group and adjustments used to calculate it. The 2026 reference curves were normalized for age, sex, and height using UK Biobank and German National Cohort participants. The authors and the Radiological Society of North America identify validation in clinical populations and disease-specific reference values as next steps. A reference may therefore not be established for every illness, patient group, or clinical setting.

Ask:
- Which population was used as the reference, and what adjustments were applied?
- Has this tool or reference been validated for people with my condition or in this clinical setting?
- How much weight should this result carry alongside the information already used to guide my care?
Was the AI analysis technically reliable?
Segmentation—the process of identifying tissue on an image—has been evaluated in different MRI cohorts, but performance depends on the particular model and data tested. In one 2020 adipose-tissue segmentation study, the evaluated model reported a Dice overlap of 0.94, sensitivity of 96.6%, specificity of 95.1%, precision of 92.1%, and accuracy of 98.4%. Those figures describe that model’s study evaluation; they are not a guarantee for every system, scan, or patient. See the 2020 segmentation study.
Ask whether the AI output or tissue segmentation was reviewed by a radiologist or clinician, whether the analyzed images were adequate, and whether the specific system was validated for this purpose. If the report is unclear, ask the clinician who can review both the scan and the result.

Can I compare this result with another scan or method?
Not necessarily. A comparison between quantitative MRI and DXA published in 2018 reported correlations of 0.99 for whole-body adipose tissue and 0.97 for lean tissue, but agreement was lower for visceral adipose tissue, with a coefficient of variation greater than 20%. Correlation does not mean two methods give interchangeable values, particularly when definitions or regions differ. See the 2018 comparison.
Before comparing results over time, ask whether the metric definition, anatomical region, scan protocol, segmentation method, and reference population match. Also ask whether repeating the analysis would affect a clinical decision; do not assume a repeat scan is needed based on a population association.

- If I have another scan, can its result be compared meaningfully with this one?
