Contact Form V05 is a free, downloadable HTML contact-page template from Colorlib. It pairs a photo panel and contact details with a four-field form and social-media links. It supplies the page’s front end, not an email-delivery service: to receive submissions, you must connect the form to a backend or form service.
What Contact Form V05 includes
Colorlib describes V05 as a split-layout contact page. One half is a photo panel; the other combines a four-field form with contact information such as an address, phone number and email address. A “follow us” row adds social links.
The description does not name the four form fields. Check the live preview or downloaded files before assuming which fields are included or changing them to match your needs. Colorlib offers both a live preview and a download.
What you need to make the form send email
The visible form does not deliver messages by itself. Colorlib’s guidance for its templates is that “The templates cover the front end only.” To receive submissions, connect the form to a server-side backend, a serverless function or a form-handling service. Colorlib names Formspree, Basin and Web3Forms as examples; those examples are not an endorsement, and their terms and pricing are not established here.

- Choose how submissions will be handled. Use a backend you control, a serverless function, or a form service that fits your requirements.
- Connect the form. Configure its submission destination and any required fields or provider-specific settings. The exact setup depends on the handler you choose; the V05 description does not specify a particular endpoint or configuration.
- Test a submission. Send a test message and confirm it reaches the intended inbox or system. A page that renders correctly is not proof that delivery is configured.
Does V05 require Bootstrap or jQuery?
No. Colorlib describes the download as HTML and CSS with a few lines of vanilla JavaScript. It says V05 ships without jQuery or Bootstrap files and can be used on its own or within a Bootstrap 4, 5 or 6 project. Bootstrap is therefore an optional compatibility context, not a requirement.
Can you use it for a commercial project?
Colorlib states that its templates may be used for personal and commercial projects under the CC BY 3.0 license, provided the Colorlib credit link is retained. Keep that attribution when using the template under those stated terms; do not treat the credit condition as optional.

V05 suits a site that wants contact details and social links presented alongside a conventional contact form, with imagery as part of the layout. Before choosing it, confirm that the photo-led split design and the form fields in the actual download fit your page. If you need messages delivered, include the work of connecting and testing a submission handler in your implementation plan.
