When cell adhesion goes wrong, the effects depend on which cells or tissue are affected. Problems attaching skin layers can cause fragile skin and blisters; autoimmune disruption of connections between skin cells can cause blisters and sores; and impaired adhesion of immune cells can make it harder to fight infections. “Cell adhesion disorder” is not one diagnosis, and a symptom alone cannot identify its cause.
What cell adhesion does—and why symptoms differ
Cell adhesion is the process by which cells bind to one another or to surrounding structures. It helps tissues stay intact and allows certain cells, including white blood cells, to move where they are needed. The effects of a disruption depend on the adhesion mechanism and the tissue involved: a structural problem in skin does not produce the same pattern as a problem with immune-cell movement.
Three conditions illustrate these differences: epidermolysis bullosa (EB), pemphigus, and leukocyte adhesion deficiency type 1 (LAD-I). They have distinct causes and are not different names for a single disease.
How different adhesion problems affect the body

Skin fragility from inherited changes: epidermolysis bullosa
EB is a group of rare conditions in which skin blisters or tears easily. Most forms are inherited and involve gene mutations that alter proteins helping skin layers bind and remain strong. Symptoms commonly start at birth or in infancy, though severity varies by type and subtype. Friction or minor trauma can trigger blisters; some forms mainly affect areas such as the hands and feet, while more severe forms can involve broader areas or mucosal surfaces. The National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) says, “The primary symptom of epidermolysis bullosa is fragile skin that leads to blistering and tearing.” NIAMS’s EB overview was last reviewed in September 2023.

Why EB is not one molecular defect
Different EB types involve different structural proteins and levels of the skin. In epidermolysis bullosa simplex, mutations in KRT5 or KRT14 can disrupt keratin networks in the epidermis. Junctional EB can involve changes affecting laminin 332 or type XVII collagen, proteins involved in attaching the epidermis to underlying layers. Dystrophic EB involves type VII collagen. These examples help explain why symptoms and severity differ across EB forms. MedlinePlus Genetics describes these genetic mechanisms; its EB overview notes that NIAMS has identified more than 30 subtypes.
MedlinePlus Genetics estimates that junctional EB affects approximately 3 people per million per year in the United States. That figure is specific to junctional EB and the United States; it is not a worldwide rate or an estimate for all EB types. The captured page does not state a publication year for the estimate.

EB care may include managing pain and treating wounds caused by blisters and tears. Wound dressings may be part of individualized wound care, but they do not correct the underlying genetic cause; a clinician can advise on suitable care. NIAMS notes that there is no cure for EB and that research into possible treatments continues.
Autoimmune disruption of skin-cell connections: pemphigus
Pemphigus is an acquired autoimmune disease, not an inherited EB subtype. In pemphigus, antibodies target desmogleins—proteins that help bind skin cells to one another—and less commonly other skin proteins. When those connections are disrupted, skin becomes fragile and fluid can collect between cell layers, forming blisters. Blisters may break and leave crusty sores; some forms affect mucosal surfaces as well as skin.

Symptoms depend on the form
- Pemphigus vulgaris: often begins with blisters in the mouth and may also affect the skin.
- Pemphigus foliaceus: affects the skin.
NIAMS says genetic and environmental factors may contribute. Some medicines or, rarely, a tumor may trigger pemphigus-like disease; these possibilities do not establish the cause of an individual case. See NIAMS’s pemphigus overview, last reviewed in August 2024.
Impaired immune-cell movement: LAD-I
In LAD-I, inherited mutations in ITGB2 impair β2 integrins, proteins that help white blood cells (leukocytes) attach to the lining of blood vessels and move through the vessel wall toward infection or injury. When this trafficking process is impaired, the body may struggle to get leukocytes to affected tissues.

Reported signs include serious bacterial or fungal infections, delayed wound healing, delayed separation of the umbilical-cord stump, inflammation or infection at the stump, severe gum and periodontal disease, and little or no pus at infection sites. These are clues associated with LAD-I, not general symptoms of every cell-adhesion problem. MedlinePlus Genetics estimates that LAD-I occurs in 1 per million people worldwide and notes at least 300 reported cases in scientific literature. Its page does not state a publication year for those figures.

Blistering, skin sores, recurring infections, or slow-healing wounds can have many causes. None of these symptoms alone establishes EB, pemphigus, LAD-I, or another specific condition. A clinician can assess the full pattern, timing, family history, and other findings and decide whether further evaluation is needed. Seek medical care for persistent or worsening symptoms, and prompt care for severe or rapidly progressing infection or skin damage.
