Neither is universally better. The best choice is the place you can walk safely and regularly. Both outdoor and treadmill walking count toward aerobic activity; research finds some differences in how people walk at the two settings, but it does not establish that one produces better long-term health outcomes for everyone.
What research says about treadmill and outdoor walking
The strongest direct comparison is a 2024 systematic review and meta-analysis of 55 crossover studies involving 1,005 healthy adults aged 18–65. The studies compared motorized treadmill and overground walking at similar speeds. On treadmills, participants had higher relative oxygen consumption and cadence on average, along with shorter stride and step lengths. These are measured differences, not proof that one setting is healthier overall. The review assessed short-term physiological, perceptual, and biomechanical outcomes, not long-term health outcomes across all populations. Vickery-Howe et al., 2024.
The review did not find a clear overall energy-expenditure advantage for either surface. Results varied across studies, so these findings cannot be translated into a reliable calorie percentage or a prediction for an individual walk. Treadmill construction—including surface stiffness and motor power—may also influence results.
Some biomechanical measures, including certain push-off forces and knee and ankle moments, were lower on treadmills in the studies reviewed. The authors note that this may make treadmill walking useful initially in some rehabilitation contexts, but it does not establish that treadmills prevent injuries or feel better on everyone’s joints.

For indoor options beyond home or gym treadmills, a CDC scoping review of mall-walking programs describes potential access benefits, including ways to address weather, safety, and accessibility barriers. It also judges the evidence about program effectiveness to be limited and often weak; it is not a head-to-head comparison of treadmills and outdoor walking. Farren et al., 2015.
Do you burn more calories walking outside?
There is no dependable universal rule that outdoor walking burns more calories than treadmill walking—or the reverse. The 2024 review found no clear overall energy-expenditure difference, and results varied. Speed, terrain, treadmill design, and the person walking all matter. A laboratory comparison at a matched speed does not predict the calorie cost of every real-world walk.

Older individual studies have reported different results: a 2008 study in healthy older adults found treadmill metabolic requirements about 23% higher than overground walking, while a 1970 Canadian report based on two people walking at 3.5 mph found outdoor energy cost 15% higher without a load and 23% higher with a 28-pound load. Their different participants and methods make these figures poor grounds for a general calorie claim. 2008 study; 1970 report catalogue record.
How much walking should you aim for?
For adults, the World Health Organization recommends 150–300 minutes of moderate-intensity aerobic activity per week, 75–150 minutes of vigorous activity, or an equivalent combination. The recommendation concerns total activity, not a preferred walking location; physical activity across different domains can provide benefits. WHO, 2020 guidelines.

Brisk walking is generally safe for most people, according to the CDC. A single moderate-to-vigorous activity session can provide immediate benefits, including reduced anxiety, and regular activity has longer-term benefits. These general findings do not show an outdoor-specific mental-health advantage over treadmill walking. CDC adult activity benefits.

- Choose outdoors if you have a suitable route and prefer changing scenery or terrain.
- Choose a treadmill or indoor route if weather, schedule, or outdoor access makes it easier to walk regularly.
- Mix the two if switching settings helps you stay consistent. The evidence here does not show that a mixed routine is medically superior.
- Start at a comfortable pace and build consistency gradually; you do not need a particular treadmill incline or setting to make walking worthwhile.
If you have a chronic condition, pain, balance limitations, or mobility concerns, the CDC’s general statement that brisk walking is safe for most people is not individualized medical clearance. Use guidance suited to your circumstances. The direct-comparison meta-analysis focused on healthy adults aged 18–65, so its findings do not settle what is best for every clinical population or older adult.
