Verizon’s 2026 reports add a financial-impact view to the breach patterns in its Data Breach Investigations Report (DBIR). Together, they answer more of the question of how breaches happen and what insured organizations lose—but they do not establish the full economic cost of cybercrime.
What does Verizon mean by the “breach report puzzle”?
The DBIR examines how breaches happen and who is behind them. Its companion, the 2026 Breach Impact Study, adds a financial dimension based on cyber-insurance claims. The DBIR team describes that as filling in some missing pieces, not completing the picture: the study’s introduction jokes, “With that finally solved, I suppose we can all pack up and go home, right?” before adding, “Unfortunately, it is not quite that simple, but we believe this report can help us find some of the missing impact pieces of this puzzle.”
The distinction matters: the DBIR’s breach findings and the impact study’s claim-loss figures come from different kinds of evidence. One describes reported breach patterns; the other describes losses recorded in a curated set of insurance claims.
What did Verizon’s 2026 DBIR find?
Known vulnerabilities became the leading reported entry point
Verizon says exploitation of vulnerabilities accounted for 31% of breaches and, for the first time in the DBIR’s 19-year history, surpassed stolen credentials as the leading breach entry point. Verizon also says AI is accelerating exploitation of known vulnerabilities and shrinking defenders’ patching window from months to hours. That is Verizon’s stated interpretation of the pressure on patching; it is not a claim that every vulnerability is exploited at that speed.

Other findings show exposure beyond software flaws

These findings point to distinct security concerns: keeping software patched, recognizing social engineering on mobile devices, understanding how employees use AI tools, and managing dependence on third parties. They are not interchangeable measures of risk.
How much do breaches cost, according to the impact study?
Verizon Business and CyberAcuView analyzed 69,683 cyber-insurance claims for U.S. incidents from January 1, 2019, through October 31, 2025. Of those claims, 38,181 had recorded losses paid to policyholders. Among the reviewed paid-out claims, the study reports these thresholds:

These are threshold figures, not a single price tag for a typical breach or a forecast for any particular organization. Verizon chose medians rather than averages because very large outliers can distort averages. The figures describe the distribution of impact among the paid claims reviewed, not every incident in the United States.
Are these numbers the total cost of a breach?
No. The study defines its figures as insurable loss and says they are a potential floor, not a ceiling, for the true economic impact. It does not estimate uninsured losses, reputational damage, or costs that never become insurance claims. Recorded payments can also fall short of an organization’s total impact because of deductibles, coverage limits, sublimits, or incomplete insurance towers.

The timing of claims is another limitation. Claims can remain open for years; 60% of 2025 claims were still open when the study was prepared. Verizon therefore omits 2025 from year-over-year comparisons. The study’s incident window runs through October 31, 2025, but its reported 2025 claims were not yet mature enough for that comparison.

- Prioritize vulnerability management. Verizon’s leading-entry-point finding and its warning about faster exploitation make timely patching a central concern.
- Include mobile channels in awareness efforts. The reported success-rate comparison suggests that email-only phishing guidance may miss an important form of social engineering.
- Understand actual AI use. Frequent employee use increased in the DBIR’s findings, but the figure alone does not say whether that use was approved or unsafe. Organizations need their own visibility to answer those questions.
- Review third-party exposure. The third-party finding is a reminder that security risk can extend beyond an organization’s own systems; the report figure does not, on its own, prescribe a particular vendor-control program.
- Treat claim thresholds as lower-bound context. Insured organizations can use them to understand the range of recorded claim impacts, but should not treat them as a complete estimate of the business cost of an incident.
