If you use hearing aids, a compatible TV streamer may send the sound to them. When the difficulty persists across programs or affects everyday conversation, consider a hearing evaluation rather than assuming the TV is the only problem.
Why TV dialogue can be hard to follow
TV sound mixes speech with music and effects, while room noise and the distance between the listener and speakers can make voices harder to pick out. Difficulty understanding speech against background sound can also be a sign worth discussing with a hearing professional; it does not by itself diagnose hearing loss.
The National Institute on Aging lists needing to raise the TV volume until others complain and having trouble understanding speech in noisy places among possible signs of hearing loss. Its infographic, last updated June 5, 2025, says about one-third of older adults have hearing loss; that is a statistic about older adults generally, not the share of viewers who struggle with TV dialogue. Try these no-cost changes first
- Turn on captions. Check the TV’s accessibility or caption settings and, if you are watching through a streaming service, check the app’s playback settings too. Captions can show spoken words and relevant sounds, but availability and controls vary. Look for options to adjust size, appearance, or position if the text is hard to read or covers important content. The American Speech-Language-Hearing Association recommends: “Turn on captioning when watching TV or talking over video platforms such as Zoom.” ASHA communication guidance and the Hearing Loss Association of America’s captioning guide explain caption access and formats.
- Try the TV or app’s speech controls. Open the audio settings on the device playing the program and look for wording such as “dialogue,” “voice,” “clear speech,” or “speech enhancement.” Labels and availability differ by manufacturer, model, and service, so use the menus or manual for your equipment rather than relying on a universal path. Some streaming services may also provide a dialogue mode for supported content.
- Reduce competing sound. Turn down or pause other audio, close a door if outside noise is distracting, and move closer to the TV if practical. ASHA advises reducing background noise in communication settings; less competing sound may make speech easier to distinguish.
Smart TVs and some soundbars offer adjustable dialogue-related features, but an effect that helps one person may alter music or sound effects in a way another person dislikes. Compare the options before buying equipment

When a TV streamer may make sense for hearing aids
A TV audio streamer can route television sound directly to hearing aids or through an intermediary, which may let a listener hear at a different level from other people in the room. The required connection depends on the TV and hearing devices. Check guidance for the exact models or ask the hearing-care professional who supports the aids before purchasing. Evidence does not show that streaming always improves understanding. A 2024 study abstract in the American Journal of Audiology reported improved performance in noise when hearing-aid microphones were muted, but no benefit from TV streaming in quiet. The study included 20 experienced hearing-aid users, so those results should not be treated as a promise for every listener or setup. American Journal of Audiology, 2024

- Confirm that the streamer works with both your TV and hearing aids.
- Follow the manufacturers’ pairing and connection guidance, then start at a comfortable listening level.
- If you are unsure how to balance streamed audio with the hearing aids’ microphones, ask your hearing-care professional to help adjust the setup.
When to consider a hearing evaluation
If dialogue remains difficult across different programs despite trying captions and settings, or if you also struggle to follow ordinary conversation—particularly when there is background noise—consider a hearing evaluation. TV volume alone is not enough to determine whether you have hearing loss. NIA’s list of possible signs can help you decide whether to seek advice.
Over-the-counter hearing aids are intended for adults 18 and older who perceive mild-to-moderate hearing loss, according to HLAA. They may not be appropriate when other symptoms or medical concerns are present, so do not choose one solely because you turn up the TV. What to do if captions are not working
If captions are absent, inaccurate, delayed, incomplete, or cover important content, first check whether the issue is in the TV settings or the streaming service. You can report a caption problem to the distributor or the FCC. HLAA explains how to file a complaint in its TV captions complaint guidance.
