If you’re choosing a fiber supplement for chronic idiopathic constipation (CIC), psyllium has the clearest evidence among the supplements evaluated by a joint 2023 American Gastroenterological Association and American College of Gastroenterology guideline. That is not proof that psyllium is best for every person or purpose: the recommendation is conditional and based on low-certainty evidence. Inulin, wheat bran, and methylcellulose have different properties, and the guideline does not settle which fiber is best for goals such as cholesterol management, diarrhea, IBS, or blood sugar.
How psyllium differs from other fibers
Fiber supplements are not interchangeable. Psyllium and inulin are soluble fibers; wheat bran is insoluble. Soluble fiber attracts water and forms a gel during digestion, while insoluble fiber adds bulk to stool and may help food move more quickly through the digestive tract. The 2023 joint AGA-ACG guideline considered bran, inulin, psyllium, and methylcellulose for adults with CIC. Its finding applies to that condition and population—not to every use of fiber.
What the evidence says for chronic constipation
Among the supplements evaluated, the guideline panel found the clearest evidence for psyllium. It concluded that psyllium appeared effective, while evidence for bran and inulin was very limited and uncertain; the panel found no studies of methylcellulose. In the guideline’s analysis, psyllium was associated with an estimated mean increase of 2.32 spontaneous bowel movements per week, based on a meta-analysis of three studies. A pooled estimate from two studies found a relative risk of 1.86 for global relief. The guideline rates the certainty of evidence for these estimates as low; they are study results, not a promised personal outcome. The guideline details the evidence and its limitations.

Choose based on your goal, not the word “fiber”
If your goal is relief from diagnosed chronic idiopathic constipation, psyllium is the best-supported option among the four supplements the guideline evaluated, but low certainty means the evidence is limited. The guideline is not a comparative verdict for cholesterol, diarrhea, IBS, blood sugar, or general wellness; the cited evidence does not establish which supplement is best for those goals.
Diet also matters. The guideline says clinicians should assess dietary fiber intake; supplementation may be a first-line option, particularly for people whose intake is low, and adequate hydration should be encouraged. MedlinePlus notes that a balanced diet may already provide enough fiber, and that supplements or artificially fortified foods may not offer the same benefits as naturally fiber-rich foods. MedlinePlus lists fruits, vegetables, whole grains, legumes, nuts, and seeds as fiber-rich foods.

Follow the exact product’s directions and take it with liquid
Psyllium is sold in forms including powder, granules, capsules, liquid, and wafers. Products and servings can differ, so use the directions on your own package or prescription rather than assuming one format or serving equals another. MedlinePlus says powder and granules should be mixed with 8 ounces (240 mL) of liquid immediately before use, and that at least 8 ounces (240 mL) of liquid should be taken with psyllium. Taking it with too little liquid can cause choking. Read MedlinePlus instructions and warnings for psyllium; a specific label record in the NIH Dietary Supplement Label Database also advises new users to start with one serving daily and increase gradually—follow the label for the product you have.

Increase fiber gradually and consider tolerance
Gas, bloating, and abdominal discomfort can occur when fiber intake rises. MedlinePlus advises increasing fiber gradually and getting enough fluids. The AGA-ACG guideline also notes that flatulence is common with fiber supplementation. MedlinePlus provides dietary fiber guidance.
Check medicine timing and ask about medical risks
Tell a doctor or pharmacist about medicines and supplements you take. MedlinePlus says not to take digoxin, salicylates such as aspirin, or nitrofurantoin within three hours of psyllium. Speak with a clinician before use if you have difficulty swallowing, intestinal blockage, rectal bleeding, or certain medical conditions.
