Intermittent fasting uses recurring shorter fasting periods or restricted eating windows; prolonged fasting means going without food continuously for at least four consecutive days under an international consensus definition. They are different patterns, and evidence about one should not be treated as evidence about the other. For adults with overweight or obesity, intermittent fasting has not been shown to produce meaningfully greater weight loss than regular dietary advice. Evidence on prolonged fasting is much thinner, largely short-term, and often comes from medically supervised protocols.
What is the difference between intermittent fasting and prolonged fasting?
The main difference is how long each continuous fast lasts. The international consensus on fasting terminology defines prolonged fasting as a fast lasting at least four consecutive days. Intermittent fasting refers to recurring shorter fasts or eating-window patterns rather than one continuous multi-day fast.

A protocol’s label does not tell you everything that matters. Whether it allows fluids or calories, how often it is repeated, who follows it, and whether clinicians monitor it can all affect what a study’s results mean.
Does intermittent fasting work better than regular dieting?
Not clearly. A Cochrane review, with evidence current through 5 November 2024, compared intermittent fasting with regular dietary advice for adults living with overweight or obesity. Across 21 studies and 1,430 people in the weight-loss comparison, the review concluded that intermittent fasting may make little to no difference to weight loss compared with regular dietary advice. Certainty varied by outcome.

A separate 2025 BMJ systematic review and network meta-analysis synthesized 99 randomized trials of intermittent-fasting strategies and called for additional high-quality trials lasting longer than 52 weeks. That call matters because short-term changes do not establish how well a pattern works or is maintained over the long term.
These findings do not establish that every approach to eating is equivalent for every person. They do mean the available evidence does not support presenting intermittent fasting as a reliably superior weight-loss strategy for adults with overweight or obesity compared with regular dietary advice.

What do studies say about prolonged fasting?
The evidence base is smaller and less conclusive. A 2023 narrative review of prolonged water fasting identified eight studies that assessed body weight and metabolic risk markers. In the included 5–20-day protocols, the review reported 2–10% weight loss; approximately two-thirds of the weight lost was categorized as lean mass and one-third as fat mass.
Those figures describe short-term results in the reviewed protocols. They do not show that the weight change lasts after eating resumes, establish a durable clinical benefit, or identify an optimal number of fasting days. The review also reported that adverse events in some studies included headaches, insomnia, hunger, and metabolic acidosis. Read the findings in the context of the studies’ protocols and participants, not as a prediction for every person or every type of fast. Read the prolonged water-fasting review.

Is prolonged fasting safe?
The available evidence does not establish prolonged fasting as safe for everyone or support treating an unsupervised multi-day fast as equivalent to a clinical protocol. The studies are limited, protocols vary, and the reported adverse events include metabolic acidosis in some studies.
A retrospective chart review examined 768 visits for medically supervised, water-only fasting at one residential medical facility. Most recorded adverse events were mild, but those observations apply to that specific setting and protocol. They cannot show that unsupervised fasting is safe, or that the findings apply to intermittent fasting or calorie-modified fasts. Read the chart review.

Medical context is particularly important for people with diabetes who take insulin or insulin secretagogues. The American Diabetes Association’s Standards of Care in Diabetes—2026 states: “People with diabetes who are taking insulin and/or secretagogues should be medically monitored during the fasting period.” This guidance is specific to diabetes and those medications; it is not a general endorsement of fasting. If you are considering a multi-day fast, discuss it with a licensed clinician rather than using a supervised study as a do-it-yourself safety guide. This is especially important if you have diabetes and use the medicines named in the ADA guidance.

Do either fasting approach prevent heart attacks or strokes?
The evidence cited here does not establish that intermittent fasting prevents major cardiovascular events. A Cochrane review of intermittent fasting and cardiovascular disease included studies lasting four weeks to six months and judged much of the evidence low or very low certainty. It found no trial data on mortality, cardiovascular mortality, stroke, heart attack, or heart failure.
Weight, blood pressure, or metabolic-marker changes are not the same as evidence that a fasting pattern prevents these outcomes. The lack of trial data means those benefits remain unestablished in this review, not that fasting has been proved to have no effect.

How to compare fasting claims fairly
- Check the duration and repetition. A daily eating window, an alternate-day schedule, and a continuous multi-day fast are different exposures.
- Check what the protocol permits. Water-only fasting, calorie-modified fasting, and restricted eating windows are not equivalent.
- Check who was studied and where. Findings in adults with overweight or obesity, healthy volunteers, people with diabetes, or patients in a supervised facility cannot automatically be transferred across groups or settings.
- Separate short-term measurements from lasting outcomes. A change in weight or a metabolic marker during a study does not establish that it persists after refeeding or improves long-term health.
- Look for safety monitoring and complete outcome reporting. Supervised protocols do not establish the safety of unsupervised fasting, and missing data on major outcomes cannot answer whether they are prevented.
