Our verdict on OpenCouch

OpenCouch is an open-source chat companion for day-to-day emotional support, self-reflection, and practical coping. It is aimed at teams building a locally deployed mental-health chatbot rather than people seeking a finished consumer service. The project combines a Next.js web chat interface, a text terminal interface, and a FastAPI REST and WebSocket backend. It can run deterministic local sessions without external API keys, while real model operation requires an OpenAI API key.

Its strongest differentiator is the combination of persistent memory and structured support flows. OpenCouch stores semantic, episodic, and procedural memory across sessions, with Postgres-backed persistence for threads, memory, sessions, crises, and feedback. Safety classification and routing run on every turn, while specialist crisis-agent handoff includes durable audit logging. Thirteen state-tracked guided exercises cover grounding, breathing, thought-work, and values reflection. These features give teams a defined foundation for multi-turn wellness interactions instead of a basic open-ended chat interface.

The platform also fits teams that want control over deployment and backend integration. It supports web, API, and self-hosted access, with integrations for the OpenAI Agents SDK, OpenAI Realtime, PostgreSQL, and OpenTelemetry. OpenAI Realtime voice backend modules extend the project beyond text workflows. However, its pre-beta status and local/self-hosted orientation make it a better fit for development, experimentation, and internally managed deployments than for organizations seeking a hosted product with an established support offering. Teams needing therapy, diagnosis, or emergency response should choose a service designed for those roles instead.
