Kizeo Forms is a mobile data-collection and field-operations platform for organizations with teams working on site. Administrators build custom forms through a web interface, then deploy them to Android and iOS smartphones and tablets. Field users can collect data offline, including photos, electronic signatures, and geolocation, with automatic synchronization when connectivity returns. The platform is suited to construction, maintenance, manufacturing, QHSE, logistics, energy, agriculture, HR, and other operational teams that need structured field workflows rather than simple online surveys.
The published plans show two billing options for each tier. Smart costs $18 per user per month with annual billing or $25 with monthly billing, and includes custom forms, AI-generated forms, unlimited forms and data, offline access, standard automations, integrations, and API access.
