A PHP email form can fail at several points: the browser may not reach the handler, the PHP code may error, the server may lack a mail transport, or a message accepted for delivery may be filtered or rejected later. Trace the submission through those layers in order; a successful form submit or a true result from mail() does not by itself prove the message reached an inbox.
1. Confirm the form reaches the PHP handler
First establish whether the request arrives at the code that sends the email. Check that the form’s method and action match the handler, and that each submitted field’s name matches what the PHP code reads. A mismatched name, incorrect path, or unexpected submission method can prevent the expected values from reaching the mail code.
- Open the handler URL or submit the form and confirm the handler actually runs.
- If JavaScript or AJAX submits the form, inspect the browser’s Network panel for the request and response; also check the server response and logs.
- Review PHP and web-server logs for routing errors, parse errors, exceptions, missing includes, and permission problems.
Do not show detailed errors to visitors on a production site. Use private server logs for diagnosis, and remove credentials or personal data before sharing code or logs.
2. Verify the PHP runtime and active configuration
A script can behave differently under the web server than it does from a terminal. The web server may use a different PHP version, configuration file, environment, or permissions. Check the runtime and configuration that serve the form, rather than assuming command-line PHP reflects them.

Resolve any PHP warning or error before investigating delivery. A missing include, invalid syntax, or exception can stop execution before the email function is called.
3. Check whether PHP has a working mail transport
PHP’s mail() function depends on an available mail transport. The PHP manual says mail functions require access to sendmail or a compatible wrapper; see PHP’s mail requirements.

On Unix-like hosting
Inspect the web server’s PHP sendmail_path setting and verify that the configured executable or wrapper exists and can be used by the web-server account. Hosting providers may manage this transport or restrict direct local mail, so confirm the expected setup with the provider.
On Windows
For PHP’s direct SMTP configuration on Windows, the documented settings include SMTP, smtp_port, and sendmail_from. Check the active values in the web server’s PHP configuration. PHP documents these platform-specific settings in its mail configuration reference.

Check the result and warning
Inspect the return value from mail() and safely log any warning or error. A false result points to failure at the PHP/transport handoff, but the exact warning and server configuration are needed to identify why.
A true result means the message was accepted for delivery, not that it arrived. As the PHP mail manual explains, acceptance does not guarantee that mail reaches its intended destination. If PHP reports success, investigate the configured mail server’s logs, any bounce or rejection notice, spam filtering, and the destination address.

4. Build safe, valid message headers and handle addresses correctly
The PHP manual says a message needs a From header, provided explicitly or through configuration. If it is missing, PHP may issue a warning. Keep From set to an address controlled by your site. If you need to reply to the visitor, validate their address and place it in Reply-To rather than using untrusted form text as the sender.
Additional headers should be separated with CRLF line endings. Sanitize any external data used to construct headers to prevent header injection; do not concatenate arbitrary submitted text into a header.

For an email field, PHP’s FILTER_VALIDATE_EMAIL checks whether the string matches supported email syntax. That validates form, not mailbox existence. PHP distinguishes validation from sanitization: sanitizing may change input, but does not establish that the changed value is valid. The PHP validation example and filter documentation explain these distinctions.
5. Decide whether to use local mail or authenticated SMTP
The right transport depends on what your host supports and what evidence the failure produces. A local sendmail-compatible transport may be appropriate where the host configures and supports it. If no usable local transport is available, use a provider’s SMTP service with its required host, port, encryption mode, credentials, and approved sender settings. Obtain those exact values from the provider; generic settings are not reliable.
PHPMailer is a PHP library with an integrated SMTP client, authentication support, contact-form examples, and Composer installation guidance. It can make SMTP configuration and errors more explicit, but it cannot fix a request that never reaches PHP or incorrect provider settings. Keep SMTP credentials private and do not expose verbose SMTP diagnostics to visitors.

What to collect if the cause is still unclear
A specific diagnosis requires the form’s method and action, the relevant PHP handler with secrets removed, the exact warning or error, hosting operating system and provider, PHP version and active mail configuration, and whether the code uses mail() or a library. If mail() returns true but no email arrives, also collect delivery evidence from the mail transport or provider; acceptance alone does not identify where the message went afterward.
