Parosmia is when a real odor smells distorted; phantosmia is when you perceive an odor without a corresponding odor source. The distinction is whether there is an identifiable smell being misperceived. Either experience may be unpleasant, and neither term by itself explains the cause.
How phantosmia and parosmia differ

Why a smell may change or seem to appear from nowhere
Smell changes can occur in connection with a range of conditions and exposures. The National Institute on Deafness and Other Communication Disorders (NIDCD) lists aging, sinus and other upper respiratory infections, smoking, nasal growths, head injury, hormonal disturbances, dental problems, chemical exposures, medications, head and neck radiation, and nervous system conditions among possible causes of smell disorders generally. That broad list does not mean every item causes both parosmia and phantosmia equally. [NIDCD]
For phantosmia specifically, Mayo Clinic lists head injury, upper respiratory infection, aging, temporal lobe seizures, inflamed sinuses, brain tumors, certain medicines, Parkinson’s disease, and COVID-19 as possible associations. These are possibilities, not a way to diagnose the cause from the smell symptom alone. [Mayo Clinic]

Parosmia and phantosmia may also occur after head trauma. Recovery estimates reported for post-traumatic smell dysfunction apply to that specific context; they should not be used to predict recovery for everyone with either symptom. [American Family Physician, September 2023]
Why an apparent phantom odor should be evaluated
Not noticing a smell source does not, on its own, prove that the odor is phantom. A source in the nose or sinuses can sometimes account for a complaint that seems to be an odor without a source. A clinician can assess this possibility before determining whether the symptom fits phantosmia. [ORL terminology review, 2023]

Phantosmia is a clinical smell symptom; the name does not mean the experience is imaginary or establish a psychological cause. It is one description of what someone perceives, not an explanation of why it is happening.
How clinicians assess smell changes
Evaluation starts with the person’s history and an examination. NIDCD says a clinician may examine the ears, nose, and throat; ask about health history, including injuries and chemical exposure; and use a supervised smell test. Such tests can measure odor-detection thresholds or a person’s ability to identify odors. The goal is to characterize smell function and look for a possible underlying cause. [NIDCD]

When appropriate, an ear, nose, and throat (ENT) specialist may use nasal endoscopy or consider CT or MRI to investigate a possible nasal or sinus source. Imaging is selected according to the clinical situation; it is not automatically needed for everyone with phantosmia. [ORL terminology review, 2023]
Treatment depends on the cause
There is no single treatment established for everyone with phantosmia or parosmia. Care depends on what evaluation finds: for example, a structural nasal problem such as polyps may need treatment directed at the obstruction. If you suspect a medicine is affecting your sense of smell, discuss it with the clinician who prescribed it rather than stopping it on your own. When to talk with a healthcare professional
Talk with a doctor about a new or concerning change in smell. NIDCD recommends medical advice for smell changes and notes that an otolaryngologist may help identify a cause and treatment options. Mayo Clinic also advises professional evaluation for phantosmia so possible serious causes can be considered or ruled out. If a smell change begins suddenly alongside other acute neurological or serious symptoms, seek prompt medical care. The smell symptom by itself does not determine whether the situation is an emergency.
