Air-conduction open-ear earbuds send sound through the air toward the ear canal and eardrum. Bone-conduction headphones use a transducer against the head to send vibrations through the skull toward the inner ear. Both can leave the ear canal open, but they are different technologies.
How each technology sends sound
Air-conduction open-ear earbuds
A small speaker directs sound through the air toward the ear canal, where it travels to the eardrum. The device sits near the ear rather than sealing the canal with an inserted tip; designs vary. Bose describes its open-ear headphones and air-conduction technology in these terms.
Bone-conduction headphones
A contact transducer rests against the cheek or nearby skull and vibrates. Those vibrations travel through bone toward the inner ear, stimulating the cochlea without relying on the usual route through the ear canal and eardrum. Shokz describes the pathway through the temporal bone in its FAQ; a U.S. Army Research Laboratory report also discusses air- and bone-conducted sound.
In short, “open-ear” does not tell you the sound pathway. Some open-ear products use air conduction; others use bone conduction.

Which type should you choose?
Choose air-conduction open-ear earbuds if sound character is the priority
This design may suit you if you want an open fit but prefer conventional speaker-like sound. Bose Ultra Open Earbuds are one example of this approach, not a recommendation for every listener.

Consider bone conduction if head contact suits your fit or activity
Bone-conduction headphones may make sense if you want the canal unobstructed and are comfortable with a transducer resting against your head. Shokz OpenRun Pro 2 is one cited example.
Check the exact product, not just the category name
Before buying either style, check the manufacturer’s current product information for fit, contact position, controls, battery, water-resistance rating, and suitability for your intended activity. Shokz notes that water protection varies by product and points buyers to each model’s IP rating; specifications and features are model-specific.

Open-ear does not mean risk-free
An unsealed ear canal can leave ambient sound accessible, but neither design guarantees awareness: audio may distract you or mask a cue. Do not treat either category as hearing protection or as a guarantee of safe running, cycling, or road awareness. The Army report discusses open canals in particular communication settings; it does not establish consumer safety outcomes.
