MRI and CT can both measure body composition and show how fat is distributed, including visceral fat around the organs and subcutaneous fat beneath the skin. Neither is a universal accuracy winner. The right choice depends on the tissue being measured, the scan and analysis protocol, radiation considerations, and whether suitable images already exist.

What MRI and CT can measure

Both methods produce images that can be analyzed to distinguish tissue compartments and assess fat distribution—information that body weight and BMI alone do not provide. The American Diabetes Association’s 2026 Standards of Care state that CT and MRI can accurately assess body composition, including distinguishing visceral from subcutaneous adipose tissue. Their use for this purpose has largely remained in research because of practical and cost barriers.

These scans do not automatically yield a single, interchangeable measure called “body fat.” An analysis may quantify a particular tissue, such as adipose tissue or skeletal muscle, at a specific anatomical location. That result is different from a whole-body estimate of fat mass, lean mass, or fat-free mass. A useful comparison begins by naming the tissue or compartment the clinician or researcher wants to measure.

How to choose between MRI and CT

Decision MRI CT
Radiation Does not use ionizing radiation. Uses ionizing radiation; an extra CT should not be obtained solely for general body-composition curiosity without a clinical rationale.
What it can assess Can characterize body composition and fat compartments. Can characterize body composition and fat compartments, with tissue-level image analysis.
Access and practicality Cost and limited availability can be barriers. Practicality depends on the clinical setting and reason for the scan; cost can also be a barrier.
Using existing images Use for analysis depends on whether suitable images and a suitable protocol are available. An existing clinical scan may sometimes be repurposed for body-composition analysis if its coverage and image characteristics suit the question.
Protocol needs The acquisition and analysis protocol must fit the measurement question. Landmark or slice selection, contrast conditions, segmentation, and image quality can affect results.

The Obesity Medicine Association’s 2022 clinical practice statement also identifies CT radiation and the expense and practical constraints associated with imaging. In general, MRI’s lack of ionizing radiation is an important distinction, not proof that it is always the more appropriate or more accurate test. CT’s potential advantage is that a scan already obtained for a clinical reason may be usable for an additional analysis, avoiding another scan—provided the images are fit for that purpose.

When MRI makes sense

  • Avoiding ionizing radiation matters. MRI does not use ionizing radiation, which can make it preferable when the clinical or research question and available resources support MRI.
  • The appropriate MRI images and protocol are available. A scan must still capture and support analysis of the tissue or compartment of interest; the modality name alone does not guarantee a useful measurement.
  • The goal is a specified tissue or fat compartment. State what is being measured and how, rather than treating an MRI result as a universal body-fat score.

Access and cost may make MRI impractical for some clinical or research settings. The available evidence does not support choosing MRI on the assumption that it beats CT across all body-composition measurements.

When CT makes sense

  • A clinically indicated CT already exists. Opportunistic analysis may extract body-composition information without ordering an additional scan, if image coverage, quality, and protocol are suitable.
  • The analysis follows a defined CT protocol. The selected anatomical level, contrast conditions, segmentation approach, and quality controls should be appropriate to the target measure.
  • The clinical or research question justifies CT imaging. Because CT uses ionizing radiation, do not recommend an additional scan solely to measure body composition without a clinical rationale.

An existing CT is not automatically usable. Incomplete coverage, motion, fluid overload, leads, metal artifacts, and other image-quality issues can compromise analysis. Contrast-enhanced images may be useful for segmentation in suitable circumstances, but contrast can alter radiodensity and measured area; unenhanced scans are preferred when radiodensity is the outcome of interest.

What makes a CT measurement comparable

CT body-composition workflows commonly transfer DICOM images to specialized software, where an analyst selects images or anatomical landmarks and segments the target tissues. Segmentation may be manual, semiautomatic, or automated. The international expert working group’s 2026 methodological guide emphasizes reporting enough detail for others to understand how a measurement was produced.

  • Record the anatomical location and image-selection method.
  • Document contrast use and phase, and whether radiodensity or cross-sectional area is being analyzed.
  • Name the segmentation software and version, whether analysis was manual or automated, and the relevant tissue thresholds.
  • Describe the evaluator and quality-control procedures, and how incomplete fields of view were handled.
  • For longitudinal comparisons, keep equipment and acquisition protocols consistent where possible. If equipment or protocols differ, cross-calibration may be needed; multiple slices may help when detecting small changes is critical.

These factors matter because measurements from different scanners, protocols, anatomical levels, or time points may not be directly comparable. A number without its measurement location and method can be difficult to interpret or reproduce.

Why there is no single accuracy winner

Accuracy depends on the tissue or compartment, image acquisition, anatomical selection, and analysis method. The sources available here do not provide a comprehensive direct MRI-versus-CT agreement estimate across fat compartments and muscle measurements, so a general claim that one modality is “more accurate” is not established.

One often-cited quantitative MRI comparison does not answer that head-to-head question: a 2018 study summary covering 4,753 UK Biobank imaging participants compared quantitative MRI with DXA. It reported correlations of 0.99 for fat tissue and 0.97 for lean tissue, with coefficients of variation of 4.5% and 4.6%, respectively; agreement was lower for visceral adipose tissue, with a coefficient of variation greater than 20%. Those are MRI-versus-DXA findings, not MRI-versus-CT results. The study summary is available at PubMed.

Similarly, the CT guide describes reliability and validity estimates for particular tissues, anatomical sites, and protocols. Those findings should not be collapsed into one general CT accuracy figure or compared with MRI results measured under different conditions.

When another method may be more practical

If the question is a clinical body-composition assessment rather than detailed imaging of a specific tissue or region, DXA or bioelectrical impedance may be considered. The ADA’s 2026 Standards describe these as clinical options but note their limitations. The 2022 ABRASSO position paper describes DXA as generally more accessible and affordable than CT or MRI, while noting that CT and MRI can provide more detailed assessment of specific tissues and smaller regions. These methods are not interchangeable: compare them by the intended outcome and the limitations that matter for that use.
