Mitochondrial DNA (mtDNA) is a small genome inside mitochondria and is usually inherited from the mother; nuclear DNA is the much larger genome in the cell nucleus and follows the inheritance pattern of each gene. Mitochondrial disorders can result from variants in either genome. The distinction also matters for testing: a clinical test for suspected disease is not the same as a consumer mtDNA ancestry test, which traces only one maternal line.
Where mitochondrial and nuclear DNA are found
Most of a cell’s DNA is in its nucleus, where it is organized into chromosomes. A smaller amount is inside mitochondria, the structures that help cells produce energy. The U.S. National Library of Medicine describes the distinction simply: “Most DNA is located in the cell nucleus (where it is called nuclear DNA), but a small amount of DNA can also be found in the mitochondria (where it is called mitochondrial DNA or mtDNA).” MedlinePlus Genetics: What is DNA?
Human mtDNA is about 16,500 base pairs long and contains 37 genes. Thirteen encode proteins used in oxidative phosphorylation, a process involved in energy production; the other genes encode transfer and ribosomal RNAs. How inheritance differs
mtDNA usually follows the maternal line
The egg typically supplies the mitochondria to the developing embryo, so mtDNA is generally passed from a mother to her children, regardless of their sex. A father generally does not pass his mtDNA variants to his children. If a mother carries an mtDNA variant, transmission does not mean every child will have the same variant level or develop the same symptoms: the amount of altered mtDNA can differ. Nuclear genes follow the inheritance pattern of the particular gene
Nuclear variants affecting mitochondrial function can be inherited in different ways, including autosomal recessive, autosomal dominant, or X-linked inheritance. That is why “mitochondrial disease” does not automatically mean “inherited from the mother.” The relevant pattern depends on which gene is involved. Why an mtDNA result can vary by tissue
Cells contain many mitochondria, and mitochondria contain copies of mtDNA. Heteroplasmy means a mixture of mtDNA sequence types, such as altered and unaltered copies; homoplasmy means the copies share the same sequence. The proportion of altered mtDNA and its distribution among tissues can be relevant when interpreting a result. A percentage alone does not reliably predict an individual’s symptoms or disease severity; interpretation depends on the variant and clinical context. This tissue variation has a practical testing consequence: a variant may be detectable in one tissue but present at a low level or not detectable in another. The Mitochondrial Medicine Society consensus recommends considering another tissue when suspicion remains after a negative blood result. Urine may give more informative heteroplasmy results in some cases; muscle or another affected tissue may be considered for particular clinical questions. UK guidance notes that muscle can remain important for some variants and large-scale rearrangements. A negative blood result therefore does not, by itself, rule out mitochondrial disease, and the evidence does not mean every person needs a muscle biopsy. What clinical testing for mitochondrial disease can involve
Because disease-causing variants can occur in mtDNA or in nuclear genes that affect mitochondria, an evaluation may need to assess both. UK best-practice guidance reported more than 350 known genes encoded by either nuclear DNA or mtDNA as causes of mitochondrial disease. Depending on symptoms, age, and clinical judgment, testing may use next-generation sequencing or broader genomic analysis; the most suitable approach is not determined by the word “mitochondrial” alone. For a suspected condition, clinicians consider what needs to be detected, including the relevant genes, variant types, and tissue. A useful discussion with a specialist or genetic counselor can cover the test’s purpose, limitations, result interpretation, and possible family implications. What mtDNA ancestry testing tells you—and what it does not
An mtDNA ancestry test follows the direct maternal line: a person’s mother, maternal grandmother, and earlier maternal ancestors. People of any sex have mtDNA and can take such a test, but the result represents one lineage, not a person’s full ancestry. An autosomal ancestry test examines many markers across the genome and offers a broader estimate. Consumer ancestry testing and clinical genetic testing answer different questions. An ancestry result estimates lineage; it should not be treated as a diagnosis or as a substitute for clinical testing for mitochondrial disease. For health concerns, a clinician can help determine whether testing is appropriate and what kind of test could address the question. Start with the question the test is meant to answer. Marketing terms such as “mitochondrial” or “DNA test” do not, on their own, establish what a test examines or what a result can tell you.

- Target: Does it analyze mtDNA, autosomal markers, a nuclear gene panel, or broader genomic data?
- Purpose: Is the test estimating ancestry, or investigating a suspected health condition?
- Sample: For clinical evaluation, is blood appropriate, or could urine or another tissue be more informative for the variant being considered?
- Detection and interpretation: Can the method address the relevant variant type and low-level heteroplasmy, and who will interpret the result in context?
- Follow-up: Is professional genetic counseling available to explain implications for health and relatives?
Specimen choice and test methodology are clinical decisions, particularly when symptoms suggest mitochondrial disease. A consumer ancestry kit is designed to estimate ancestry, not to answer those clinical questions.
