If a particular sound sets off a powerful reaction even when it is not loud, the pattern is more consistent with misophonia than with sensitivity driven mainly by sound volume. Pain or discomfort as sounds get louder raises a different question—hyperacusis—while feeling overwhelmed by a noisy setting or fearing that a sound will occur can point to broader noise sensitivity or anticipatory anxiety. These patterns can overlap, and this comparison cannot diagnose you.
How do the patterns differ?
The most useful clues are what triggers the reaction, how loud the sound needs to be, and what you feel. The descriptions below summarize the working definition from the Misophonia Consensus Committee’s 2022 Delphi study and the definitions reviewed by Henry and colleagues in the American Journal of Audiology in 2022.

A person may have more than one of these patterns. Misophonia can also occur alongside psychiatric, neurological, auditory, or sensory-processing conditions, so a single symptom or checklist cannot sort out every case.
What clues can help you describe your reaction?
Rather than trying to label yourself, compare your experience across a few questions. The answers can help you explain the pattern to a clinician:

- Is it a particular sound or many sounds? A repeatable reaction to a specific pattern or cue is a closer match to the defining feature of misophonia than distress that rises with the overall noise level.
- Does loudness determine the reaction? Physical discomfort or pain as sounds get louder is a reason to ask about hyperacusis. A sound that provokes distress at ordinary volume raises a different question from one that becomes uncomfortable mainly as it grows louder.
- What is the main response? Note whether it is anger or disgust, physical pain or discomfort, broad overwhelm, or fear that the sound will occur. More than one response can be present.
- Does anticipation matter? If fear and avoidance begin before a sound happens, mention that as well as what occurs during the sound itself.
- Does it interfere with daily life? Record effects on home, school, work, relationships, sleep, or activities you avoid. Functional impact helps a clinician understand the concern.
These are prompts for describing your experience, not a diagnostic algorithm. The Misophonia Consensus Committee and Henry and colleagues both identify the need for careful differentiation among sound-tolerance patterns.
Does anxiety cause sound sensitivity?
Anticipatory anxiety can shape a sound reaction: someone may fear that a sound is coming, expect discomfort, or avoid situations because of that expectation. But anxiety should not be treated as the default cause of every strong reaction to sound. A specific sound may trigger a misophonia-like response, loudness may produce physical discomfort, or a noisy environment may be broadly overwhelming; anxiety can also be present alongside any of these.

The Misophonia Research Fund’s overview, accessed October 7, 2026, says the causes of misophonia are not presently known and that diagnosis and evidence-based treatment remain active research needs. It also notes that misophonia is not currently defined in the DSM or ICD. This means a reader should not infer a cause—such as anxiety, trauma, autism, or a particular brain mechanism—from sound sensitivity alone.
When is an assessment worth seeking?
Consider speaking with a qualified clinician if sound reactions persist, cause significant distress, or interfere with ordinary activities or relationships. You do not need to settle on a label before asking for help. A clear account of the pattern is more useful than trying to force it into one category.

- Write down the sounds or situations involved and whether the trigger is specific, broadly environmental, or related to loudness.
- Describe the main response—emotional distress, physical discomfort or pain, overwhelm, anticipatory fear, or a combination.
- Note when the pattern began, how often it occurs, what you avoid, and how it affects daily life.
- Ask the clinician to clarify terminology with you. Henry and colleagues’ 2022 review recommends aligning the terms used by patients and clinicians during assessment.
Audiology assessment may help with hearing-related concerns and distinguishing sound-tolerance patterns. Behavioral-health assessment may be useful when anxiety, avoidance, mood symptoms, or other concerns are part of the picture. Duke Health describes individualized care that can involve audiologists and other specialists depending on co-occurring concerns; this is one care model, not a referral rule for everyone.

There is no single established approach that works for everyone. Henry and colleagues’ 2022 review describes treatment as generally involving gradual, systematic sound desensitization and counseling, while also noting inconsistencies in terminology, assessment, and treatment. Duke Health describes tailored CBT and audiology input, including sound generators used to neutralize triggering noises. Oxford Health NHS Foundation Trust says there is some evidence that CBT can help. These descriptions support discussing options with a clinician, not assuming a particular method will work for you.

Ear protection, noise-cancelling headphones, or other accommodations may help in particular settings, but the International OCD Foundation cautions against treating protective devices as a stand-alone treatment. Likewise, a sound generator or consumer white-noise machine should not be considered a proven cure. If you are considering sound masking or changing how you use protection, discuss the approach in the context of your own symptoms and daily needs rather than relying on a device as a universal solution.
