Misophonia usually involves intense reactions to particular sounds, even when they are quiet; hyperacusis usually means reduced tolerance of sound itself, so ordinary sounds may seem too loud, uncomfortable, or painful. The distinction is useful, but the conditions can overlap, and neither one can be identified from a single symptom alone.

How are misophonia and hyperacusis different?

What to compare Misophonia Hyperacusis
Sound pattern Often selective: particular sounds, commonly mouth or nose sounds, trigger a strong response. Often broader sound intolerance, more closely related to sound intensity or physical discomfort.
Relationship to volume A trigger can provoke a reaction even when it is quiet or ordinary in volume. Ordinary sounds may be perceived as excessively loud or uncomfortable.
Typical response Often described as intense emotional or physiological distress. May involve excessive loudness, discomfort, or pain. Pain is not required.
Can they overlap? Yes. Misophonia can occur alone or alongside hyperacusis and tinnitus. Yes. Having one label does not rule out the other.

These are patterns, not rigid diagnostic rules. A 2022 expert consensus study and a 2022 clinical tutorial describe the challenge of defining and distinguishing sound-tolerance conditions; researchers continue to work toward reliable definitions and assessment tools. Why do certain sounds make me angry?

A highly selective reaction—such as distress triggered by a particular person chewing or sniffing—may fit the pattern commonly described as misophonia. The sound does not have to be loud. Anger is one possible reaction, but it is not a stand-alone test: emotional and bodily responses can occur in different sound-tolerance conditions.

The American Tinnitus Association’s Spring 2025 article describes misophonia and hyperacusis as potentially co-occurring, rather than mutually exclusive. Its authors also distinguish loudness hyperacusis from pain hyperacusis, underscoring that the word “hyperacusis” does not automatically mean sound causes pain. Why do everyday sounds hurt or seem too loud?

If a wide range of everyday sounds seems excessively loud or physically uncomfortable, that pattern is more consistent with hyperacusis than with a response limited to a few specific triggers. Some people experience pain, but pain is not necessary for hyperacusis. The ATA article distinguishes loudness hyperacusis from pain hyperacusis, and a 2024 study notes that pain is not a universal or defining feature.

Neither loudness nor pain alone establishes a diagnosis. The sound pattern, the perceived loudness, physical sensations, emotional response, and effect on daily life all matter. A person can also have selective triggers and broader sound intolerance at the same time. The 2024 study examined separate effects of hyperacusis and misophonia severity; its sample included 119 people with misophonia, so its findings should not be treated as a population prevalence estimate.

How to describe your symptoms to a clinician

There is no simple self-test established by these sources. If sound sensitivity causes pain, substantial avoidance or distress, or interferes with everyday activities, consider an evaluation by an audiologist or another qualified clinician. Describe the full pattern rather than choosing a label based on one reaction.

  • Which sounds trigger the problem? Are they specific sounds or many different sounds?
  • Does the reaction depend on volume, or can a quiet sound trigger it?
  • Do sounds seem too loud, feel physically uncomfortable, or cause pain?
  • What emotional or bodily response occurs, and how does it affect daily activities?
  • Do tinnitus or other sound-related symptoms occur too?

A clinician can assess these details and consider other possible causes. A normal hearing test should not be treated as proof that sound intolerance is absent.

What is known about treatment and sound management?

Management depends on the individual presentation. The ATA article describes graded sound exposure and counseling as options that may help some people with loudness hyperacusis, but sound therapy does not help everyone. It may be ineffective for some people with pain hyperacusis and can worsen symptoms. Do not force yourself to endure painful sounds or assume that continuous earplug use is appropriate; discuss a plan with a qualified clinician.

The ATA article also gives a broad estimate that “as many as 17 percent of people have difficulty tolerating everyday sounds that do not bother others.” This is an estimate for sound-hypersensitivity difficulty overall—not a prevalence estimate for misophonia or hyperacusis separately.
