A useful first approximation is autocomplete trained on a vast range of examples—but modern models are far more capable than a phone keyboard, and their fluent answers can still be wrong.
What is a language model?
A language model is a system trained to model patterns in language so it can estimate which text is likely in a given context. Many modern language models use neural networks, often built with the Transformer architecture. They do not take in text as whole words or human-like concepts directly: they process a sequence of tokens and learned numerical representations.
A token is a piece of text. It might be a whole word, part of a word, punctuation, or another text unit, depending on the tokenizer. The MIT Press survey discusses tokenization and the representations used by language models in detail: Language Model Behavior: A Comprehensive Survey.
How do language models use context?
In a Transformer, self-attention helps the model relate tokens to other tokens in the context it can see. That lets the model use surrounding words to interpret what may come next. For example, the likely continuation after “She left her coat by the…” depends on the words already present, not just on a fixed list of likely words. Context is finite: a model can only process a limited amount of text at a time. In a chat, the prompt, earlier conversation, any supplied material, and newly generated tokens all use space in that context window. Microsoft Learn describes this constraint in its LLM fundamentals overview.
How does an LLM predict the next word?
More precisely, it predicts a distribution over possible next tokens—not necessarily whole words. In causal generation, the model uses the tokens so far to score possible continuations. A decoding method then selects a token, which is appended to the sequence; the model repeats the process to produce the answer incrementally. Generation ends when the system reaches a stopping condition or a limit.

This is why the model does not compose an entire answer in one indivisible step. Each new token becomes part of the context for choosing what follows. Microsoft Learn explains autoregressive generation and token selection.
How are language models trained, and how is that different from answering?
Training adjusts a model’s parameters using examples and a learning objective. A common objective for a generative model is to predict a later token from earlier tokens. Answering, or inference, uses the trained model: the prompt is tokenized, the model generates a continuation, and its parameters are used rather than updated for each reply.

Some dialogue systems also receive additional fine-tuning to shape how they respond. Google’s account of LaMDA describes one system that was pretrained and then tuned for dialogue, safety, and quality; it is an example, not a claim that every model follows the same recipe: LaMDA: Towards Safe, Grounded, and High-Quality Dialog Models for Everything.
Are all language models next-token predictors?
No. Language-model families can use different objectives and see different context when making a prediction. These differences suit different kinds of tasks; the objective alone does not determine a model’s quality, factuality, safety, or suitability for an application.

A language model’s ability to produce plausible text does not guarantee that the text is factually correct. Fluent false output is a recognized failure mode, but the sources cited here do not establish a universal error rate or a single cause. Check consequential claims against dependable sources rather than treating confidence or polish as proof.
