What happened in the Keystone Health breach?
Keystone Health’s Notice of Security Incident says the organization identified an incident that temporarily disrupted its computer systems on August 19, 2022. Keystone reported the incident to law enforcement and worked with a third-party cybersecurity firm. Its investigation found that an unauthorized party accessed files between July 28 and August 19, 2022.
The notice says some accessed files held patient information, including names, Social Security numbers, and clinical information. It does not say that every affected person’s records contained every listed data type.
How many people were affected, and what information was involved?
Shub Johns & Holbrook LLP, counsel for plaintiffs in the case, reported approximately 235,237 affected individuals in an August 16, 2023 update. The breach notice itself does not give that count. Counsel’s report lists names, addresses, dates of birth, Social Security numbers, clinical information, and other personal identifiers. The notice specifically identifies names, Social Security numbers, and clinical information.

The two sources describe the incident from different perspectives: the notice documents Keystone’s account of the event and examples of information in accessed files; plaintiff counsel supplies the approximate affected-person count and a broader list of data categories. Neither source establishes which specific records or data types were exposed for any particular individual.
What should you do if you think your information was exposed?
The sources do not establish any individual reader’s exposure status. Keystone’s notice said it was mailing letters to affected patients and offered credit monitoring to eligible patients at the time. It directed people who thought they might be affected but had not received a letter by November 14, 2022 to contact a toll-free call center. That date and the service offer are historical; they should not be treated as a current contact route or an active enrollment opportunity.

What happened with the lawsuit and settlement?
The case was Breneman, et al., v. Keystone Rural Health Center d/b/a Keystone Health, No. 2023-618, in Franklin County, Pennsylvania. Plaintiff counsel reported that Judge Shawn D. Meyers granted final approval of the class action settlement on August 15, 2023. Counsel also reported an August 14, 2023 claims deadline, which has passed; the settlement should not be described as open for new claims.

A memorandum filed in a different data-breach case refers to the Keystone matter as a finally approved $900,000 settlement fund for approximately 235,237 class members. That figure is a secondary reference, not the Keystone settlement order. The available sources do not establish the complete benefit allocation or what any individual class member received, so the fund amount cannot be used to calculate a personal payment.
A Justia docket summary for a related federal action reports that on August 28, 2023, an order directed the lead federal action to be marked closed. That is a separate procedural entry from the state-court settlement approval.
